Eman Ahmed, the world’s heaviest woman, will have to lose 200kg in six months – Hindustan Times

Eman Ahmed, the 500-kg Egyptian and possibly the worlds heaviest woman, will have to lose four-fifths of her weight to reach a healthy size.

Ahmed, who was admitted to Saifee Hospital on Friday, began her treatment on Monday, doctors said. As Eman is immobile, the first goal is to help her sit, eat and use a bathroom without help.

The ultimate goal is to bring Eman to a sustainable and healthy weight of under 100kg. Doctors are hopeful of achieving a weight reduction of about 150-200kg in the first six months.

Originally about 500kg, Eman has lost about 20kg during the three week period of continued monitoring and treatment, provided by a team of doctors who had travelled to her home town, Alexandria, Egypt.

Dr Muffazal Lakdawala, Emans doctor and chairman of Minimal Invasive Surgical Sciences and Research Center at Saifee Hospital, said that for the first four weeks, the goal is to treat her fluid retention issue. The ailment results in excessive build-up of fluids in the circulatory system, body tissues, or cavities in the body, causing weight gain.

The weight of fluids in her body is about 70-100kg. Once that is cleared, we will monitor her development and take a call on the bariatric surgery, said Dr Lakdawala.

Dr Hemal Shah, head of the department of nephrology at the hospital, said that while water retention doesnt usually result in an increase of more than 5-10kg, it is different in Emans case. While the body mass index (BMI), a measure of body fat based on height and weight of a normal person is about 24, Emans BMI is almost ten times higher at 252. Doctors from the hospital said she also suffers from ailments such as severe lymphedema, type 2 diabetes, hypertension, hypothyroidism, severe obstructive and restrictive lung disease, gout and sleep apnea. She is at a high risk of pulmonary embolism.Doctors said the line of treatment will follow a systematic and holistic approach. Bariatric specialists from the US, UK and Australia suggested that Eman should not be rushed into surgery.

The result of gene study will identify which of the 91 different types of genes isolated with obesity-related syndromes are causing the problem. Depending on the diagnosing of the monogenic disorder disorders caused by a single defective gene on the autosomes a call will be taken, said Dr Zoya Brar, Managing Director of Core Diagnostic. She added that Emans lipid profiles are indicative of a positive trend and a lot of drugs are now available to treat monogenic disorders.

Nutritionists at the hospital started following Emans diet starting on Monday. Carlyne Remedios, senior nutritionist said her diet will change according to the developments. For two days after she arrived, we let her continue her old diet. Now, she has been put on liquid, protein rich diet, with fibre and dairy supplements, said Remedios.

Eman, who consumed about 1500 calories daily will now receive 1200 calories. Moderately active 35-year-old women those who walk 1.5 to 3 miles daily require about 2,000 calories a day while active ones those who walk more than 3 miles need about 2,200. The difference is owing to Emans extremely low metabolism, which doesnt allow her to release calories normally.

Dr Lakdawala said one of the positive aspects of the case was ruling out any partial paralysis. Local doctors had diagnosed her with partial paralysis in right hand and right leg after the stroke she suffered two years ago. But I asked her to move her right hand and leg, which she did, said Dr Lakdawala.

Doctors will also work on her extremely low metabolism rate which is assisting the weight gain. A metabolism rate of an individual refers to the amount of calories a person frees from their body per unit time. Right now, Emans body absorbs most of what she eats. Our long term goal is to bring the rate down to 20-25%, said Dr Lakdawala. Doctors will also work on the size of her intestine to reduce food intake.

Language barriers and engineering wonders

Officials from Saifee said it took Eman two hours to travel from her home in Alexandria to the airport. Huzaifa Sherani, COO, Saifee Hospital said local engineers were not fluent in English and he could not speak Arabic. We communicated through graphs and bonded over coffee, which is called kavaa in the local language. Our intention was to break the wall of Emans bedroom, safely lift her bed and put it in the vehicle en route to the airport. About 400 cargo specialists and government officials from both countries were a part of the logistical support provided to bring Eman 6,000km from Egypt to Mumbai, said Sherani

Emans sister Shaimaa a former engineer left her job to take care of her. She has a two-year-old daughter who is currently staying with her grandmother in Alexandria. While Eman lost her father six months ago, her mother stitches clothes to earn an income. Every organisation involved in transporting and treating Eman is working on pro-bono basis to ensure that the family doesnt have to bear the burden of the extensive cost. Emans air-transport alone cost about Rs83 lakh, which the Saifee Hospital trust paid for.

Hiking seems like a romantic first date, considering all the great hiking and walking spots available in Los Angeles County. But a hiking date could take you to a remote location with a stranger, andits likely to mean spending hours together with someone you might have an immediate aversion to.

Fisher suggests keeping the first date to about one hour, and always meet in a public place during the day.

7. Go home alone

This might be the single most controversial bit of advice on thislist. (It certainly sounds boring and old fashioned.) But Thurber argued his point this way: Youre still getting to know this stranger.Whether he or she becomes the love of your life is yet to be determined thats why its always good to exercise some caution, he said.

Juice cleanse

Any suggestion that the human body can be detoxed with a juice cleanse is incorrect. We are naturally designed to remove toxins using our liver and kidneys a juice cleanse won't perform such a detox.

Those who promote juicing often claim that drinking juice is better than eating whole fruits and vegetables because removing fibre makes nutrients easier to absorb. However, there is no scientific research to support this claim.

Antioxidants are often bound to the fibre in fruit and vegetables and countless research studies suggest that the lack of fibre in juices significantly reduces any supposed health benefits.

Consuming well under 1,000 calories per day from juices will likely result in a stark calorie deficit and weight-loss will be inevitable. But this is extremely difficult to sustain for more than a few days and while a juice diet may help initially with slimming, any severe calorie restriction can slow down your metabolism long-term.

Juices should not be used as meal replacements. They do not contain sufficient protein nor healthy fat - both of which are required by the body. In fact, drinking juice over a long period of time could increase the risk of metabolic syndrome, liver damage and even obesity.

Veganism

Alkaline diet

This weight loss scheme is based on the claim that modern diets cause our body to produce too much acid, which is then turned into fat, leading to weight gain. High acidity levels have also been blamed on conditions such as arthritis, osteoporosis, tiredness, and kidney and liver disorders.

An alkaline diet reduces the number of acid-producing foods you eat, such as meat, grains, refined sugar, dairy, caffeine, alcohol and processed foods, in favour of alkaline foods.Ultimately, the diet will consist of a great deal of fruit and vegetables and a small amount of protein.

While the advice to cut back on processed foods, alcohol, caffeine and refined sugar is good, any weight loss associated with this diet is largely down to the fact these unhealthy foods have been cut out, rather than anything to do with acid and alkaline.

The human body regulates its own acidity levels, regardless of what you eat and drink.

Unlike so many other diets, the alkaline diet is not unhealthy, but its claims are simply not supported by science and human physiology.

Meal replacements

The idea is to restrict calories by eating a range of bars, soups, and shakes, as well as having a few low-calorie meals.

Like with any calorie-restricting diet, weight loss can be dramatic. Despite some products being fortified with vitamins and minerals to ensurenutritionisn't lost, the very mindset of giving up normal meals and swapping them for a snack bar or a shake can be boring, socially isolating and difficult to maintain long-term.

And while fortified products are a welcome addition to any restrictive diet, the inclusion of sugar and artificial chemicals is definitely not, with studies suggesting that they will limit any health benefits from vitamins and minerals.

A blackcurrent and apple shake, for example, contains 44.1g of sugar per 100g significantly above your recommended daily intake.

Low calory meal plans

Low-calorie weight loss programmes are based on your Body Mass Index (BMI) score, which will dictate how many meals a day you have.

This diet does advise you to see a GP before you start and can fit into a busy schedule - but it has many drawbacks. Restricting carbohydrates and fibre will result in side effects such as tiredness, dizziness, and constipation, making this plan unsustainable long-term.

It could also impact on your mental well-being. Without any allowance for spontaneous eating out, they are unsustainable as diet plans.Any low-calorie diet that involves eating 1,000 calories a day or less should not be followed for more than 12 continuous weeks and should require professional medical supervision.

Low fat meal plans

These diets encourage the swapping of high-fat foods for low-fat foods which are naturally filling. But this idea is out-dated, with healthy fats now considered a crucial element in our meals.

On these diets, some items are considered 'free foods' which can be eaten in unlimited amounts including fruit, vegetables, pasta, meat, fish and eggs. No calorie counting is involved and no foods are banned, and it's said you could lose one or two pounds a week.But without any education surrounding portion sizes and nutritional intake, many will find it difficult to understand what to do when the programme ends and struggle to keep the weight off long-term.

There would also be a temptation to fill up on 'free foods' such as processed meats without any concern for the salt, sugar and other unhealthy additives contained in them.With many carbohydrates restricted, these diets will only worsen some people's obsessive fear of supposed 'bad foods'.

There are many reasons why someone may want to lose weight fast, such as a wedding or a beach holiday. However, it is important to take a healthy approach to weight loss and to avoid putting the pounds back on again after a fad diet.

The tactic is not without its pitfalls. The British Dietetic Association carried out a survey in 2004. A third of people questioned said they had ended up heavier than before they went on a diet. The BDA says that is probably because fad diets are so popular these days.

If it sounds too good to be true then it probably is. Be wary of diets that insist on avoiding whole groups of foods. Diets that tell you to eat only specific foods (like cabbage) are another sign of a fad diet. Be sceptical about claims that certain foods (such as grapefruit) have magical fat-burning qualities. You should also be careful about any diet that focuses on how good you will look rather than on how much more healthy you will be.

You can safely lose over 2lbs (1.3 kilos) a week at home with a healthy diet and lots of exercise, says dietician and weight loss counsellor Katherine Tallmadge.

In fact, having a goal like looking good at a wedding or reunion can be a great motivator, as long as you follow a weight loss plan that you can keep up after the special event. However, you need to plan ahead and allow enough time to make changes to your shape.

Losing weight is a simple concept: you need to burn more calories than you eat. The British Nutrition Foundation recommends a sensible weight loss of 1-2 lbs (approximately 0.5-1.0 kg) a week.
