New Weight-Loss Surgery to Lose 20-50 Pounds
A new surgical weight-loss procedure is now available to women who are looking to slim down and lose 25 to 70 pounds.
Dr. Tom Lavin, founder ofSurgical Specialists in Louisiana, is a pioneer behind the hottest new weight-loss procedure called POSE, which stands for primary obesity surgery endoluminol.
POSE is for patientswho want to lose 25-50, maybe 60 or 70 pounds, says Lavin. Its a much different group of people than we normally approachfor bariatric surgery.
POSE is like the classic bypass operation, but there are no incisions, as everything is done through the mouth using an endoscope. The surgical tools make the stomach about 30 percent smaller, says Lavin, and the patient typically goes home the same day.
Krystal Townsend, 34, had struggled with her weight most of her life and was not happy with who she was. She weighed 229 pounds before she paid a visit to Lavin. After POSE, she says, she has a new lease on life.
It was amazing, says Townsend. I didnt have any pain or any nausea, or any of the things that you hear about with some of the other procedures that are done.

Short-Term Intensive Weight Loss Program Works for Four Years; Valid Option Seen to Bariatric Surgery
Newswise PHILADELPHIA -- June 12, 2012 -- A study by a team of clinicians and researchers at the Joslin Diabetes Center in Boston has shown for the first time that patients with diabetes who enrolled in a short-term intensive weight management program were able to lose weight and keep it off on their own for four years. The findings suggest an alternative course to bariatric surgery in the fight against type 2 diabetes.
The study, presented as a late-breaking abstract Sunday at the American Diabetes Associations 72nd Scientific Sessions, strongly counters the popular view that people generally cannot maintain long-term the weight loss achieved during lifestyle intervention programs.
The notion that most people in the medical field have is that when you apply intensive lifestyle interventions, the majority of patients gain all or most of the weight back in a year, said Osama Hamdy, M.D., Ph.D., Medical Director of the Obesity Clinical Program and Director of Inpatient Diabetes Management at Joslin and lead author of the paper.
People have been pessimistic, he added. They think they may need bariatric surgery. But we are sending an optimistic message. Think again. There is something else that can work effectively in real-world clinical practice and save money too. This is a very important observation.
The study showed that around 50 percent of the 120 patients who enrolled in the Weight Achievement and Intensive Treatment (Why WAIT) program for 12 weeks were able to maintain an average of 9.5 percent weight loss at four years, while the total group maintained 6.3 percent at four years.
Why WAIT is a multidisciplinary diabetes weight management program designed for clinical practice. The program included a change in diabetes medications to enhance weight reduction, structured dietary intervention with lower carbohydrates and higher protein and meal replacement, an exercise program, with emphasis on strength training and weekly educational and support sessions.
Although subjects in this study were followed for four years, they were on their own after the initial 12-week program. The study found that about half of the subjects did not regain the weight -- on average 24 pounds -- after four years. This group also maintained a significant improvement in their metabolic control as measured by hemoglobin A1c and the improvement in other vascular risk factors like blood pressure and lipids.
It also found that those who maintained at least seven percent of their weight loss at one year were most likely to continue the weight reduction over the long term.
This is very important information, Dr. Hamdy said. It tells us not everyone gains the weight back. In reality, these are similar numbers to the weight loss gained by some bariatric surgeries at that time frame.
Those who had lost at least seven percent of their body weight after one year lost an average of 29 pounds -- or 11.9 percent of their body weight -- at 12 weeks and maintained an average loss of 31.5 pounds -- or 12.6 percent -- at one year.

In the study, being presented at the 72nd Scientific Sessions of the American Diabetes Association in Philadelphia, patients completing four years of BYDUREON treatment experienced clinically significant improvements in A1C (1.7 percentage points) and fasting plasma glucose (-37 mg/dL) from baseline. A1C is a measure of average blood sugar over three months. Although BYDUREON is not indicated for weight loss, patients treated with BYDUREON also lost an average of 5.5 pounds from baseline.
In this study, patients treated with just one dose per week of BYDUREON for four years experienced sustained improvement in glycemic control and showed reductions in certain cardiometabolic measures. The tolerability of BYDUREON also improved over time with long-term treatment, said Christian Weyer, M.D., senior vice president, research and development, Amylin Pharmaceuticals. The durability of treatment effectiveness and tolerability of BYDUREON are critically important in managing the chronic and progressive condition of type 2 diabetes.
Patients treated with BYDUREON also experienced statistically significant reductions in certain cardiovascular risk markers, including systolic blood pressure (-1.6 mmHg), total cholesterol (-10.9 mg/dL), LDL cholesterol (-8.0 mg/dL) and triglycerides (-13 percent). No unexpected safety findings were observed with long-term BYDUREON therapy, and incidence of mild nausea, the most common adverse event during the controlled portion of the study, decreased over time (27 percent weeks 1-30; 1.6 percent weeks 180-212).
Study Details DURATION-1 compared the efficacy of once-weekly BYDUREON against twice-daily BYETTA (exenatide) injection in a 30-week, randomized, open-label study. Following the controlled portion of the study, 258 of the 295 intent-to-treat patients (87 percent) entered the extension study. A total of 68 percent of patients (n=176) completed four years of treatment, 55 percent of whom achieved the ADA-recommended A1C target of less than 7 percent. Primary endpoints were change from baseline to year four in A1C, and long-term safety and tolerability.
