GLP-1s Lead to Weight Loss, Improved CVD Outcomes in Breast Cancer – Drug Topics

In abstracts presented at ASCO 2024, the annual meeting for the American Society of Clinical Oncologists, researchers discovered that glucagon-like peptide-1 receptor agonists (GLP-1RAs) were significantly beneficial in treating obesity among women with breast cancer. They also explored GLP-1RA use for anthracycline or HER-2 inhibitor-treated patients with breast cancer and diabetes, finding a reduction in major adverse cardiovascular events (MACE).1,2

Weight gain and obesity are significant problems for women receiving adjuvant therapy for breast cancer and obesity is associated with adverse breast cancer outcomes. In addition, obesity is associated with negative impact on overall physical and mental health. The GLP-1 agonists, semaglutide and tirzepatide, are effective in achieving weight loss and have been associated with decreased incidence of diabetes, stroke, and overall mortality in the non-diabetic as well as diabetic populations, wrote Fischbach et al.1

In 2023, for example, semaglutide overtook the cancer medication adalimumab as the highest-selling drug in the country, and experts predict that trend to continue through 2024.3

With ongoing trials to explore GLP-1 treatment beyond diabetes and weight loss, researchers at ASCO 2024 examined the drugs uses for patients with breast cancer. Analyzing GLP-1 efficacy in various cohorts, researchers presented their findings in the attempt of rapidly expanding GLP-1 research and data.

Researchers aimed to address GLP-1 use for weight loss and cardiovascular (CV) outcomes in women with breast cancer. There are limited data on the efficacy and safety of GLP-1 agonists in women with breast cancer, they wrote.1

Researchers examined over 92,000 medical visits for 7149 patients. Using data from the Yale Cancer Center and Yale New Haven Health Network from 2017 to 2022, they performed a retrospective analysis of semaglutide and tirzepatides association with weight loss in patients with breast cancer at stage I, II, or III.1

They gathered 5430 evaluable patients (mean age, 58 years) with non-metastatic disease and found that just 1.3% of the population was prescribed either semaglutide or tirzepatide.

The pretreatment mean weight was 95.8 kg (211.2 lbs) and mean body mass index (BMI) was 35.9 kg/m2. For patients treated with either GLP-1RA, mean weight loss was 3.03 kg (6.68 lbs), which accounted for a decrease of 1.1 for mean BMI. The mean maximal weight loss for semaglutide- or tirzepatide-treated patients was 8.89 kg (19.60 lbs), which accounted for a maximal reduction in BMI of 3.2.1

In this retrospective analysis, [GLP-1RAs] are associated with clinically meaningful weight loss in overweight and obese women with early breast cancer. Recurrence risk did not appear to be preliminarily impacted by treatment. Additional research is needed on long term safety as well as weight loss maintenance with these drugs in breast cancer survivors, concluded authors of the abstract.1

Analyzing a much more specific study population, a second abstract addressed GLP-1 use among patients with breast cancer and diabetes also receiving anthracycline or HER-2 inhibitors.

Anthracyclines or HER-2 inhibitors are associated with the potential for [MACE] among patients with breast cancer. Diabetes further increases the risk of MACE, wrote the authors.2

Researchers included adult patients with breast cancer and type 2 diabetes that were previously prescribed anthracyclines or HER-2 inhibitors. Then, patients within this population were given a GLP-1 prescription and matched with a cohort receiving no GLP-1 and instead a second-line diabetes medication.

Among the study population, 724 patients receiving GLP-1s were matched with a non-GLP-1 group (median age, 60 years for both groups). The prevalence of hypertension (81%), metastatic disease (61%), and radiation therapy (42%) was similar between cohorts, as was the use of anthracyclines and HER-2 inhibitors, they continued.2

Compared with the non-GLP-1 group, those receiving GLP-1 treatment reported a reduced risk of MACE, heart failure hospitalization, and all-cause mortality.

[GLP-1s] were associated with a reduction in major adverse cardiovascular events, incident heart failure and heart failure hospitalization, and all-cause mortality among anthracycline or HER-2 inhibitor-treated patients with breast cancer and [diabetes], concluded the authors.2

Cheaper versions of blockbuster obesity drugs are being created in India and China – Nature.com

Blockbuster weight-loss drugs such as Wegovy could soon become a lot cheaper and reach more people thanks to Chinese and Indian pharmaceutical companies. A long queue of companies is developing copies of the complex biological drugs, and some are racing to create modified or improved versions to compete in the global market.

There is huge potential for companies from India, China, that can help create access to these drugs, says Abhijit Zutshi, chief commercial officer of the pharmaceutical giant Biocon, headquartered in Bengaluru, India, who oversees its generics business and is based in Woodbridge, New Jersey.

These companies will bring drugs to patients who need it, at a cost that can be afforded, says Guy Rutter, a cell biologist at the University of Montreal, Canada.

Globally, one billion people have obesity or are overweight and hundreds of millions of them live in India and China. Lei Qian, vice-president of clinical development at Innovent Biologics in Shanghai, China, says that demand for anti-obesity drugs is very strong in the country.

In the past few years, a new class of weight-loss drugs has taken the world by storm. They work by mimicking a naturally occurring hormone called glucagon-like peptide 1 (GLP-1). GLP-1 not only regulates blood sugar levels, but also binds to receptors in the brain that control appetite and receptors in the gut that slow digestion and contribute to a feeling of being full.

In 2014, the US Food and Drug Administration (FDA) approved the first class of GLP-1 drugs for weight loss. Liraglutide, marketed as Saxenda by Novo Nordisk, based in Bagsvrd, Denmark, is a daily injection that can trigger an average body-weight loss of 8% after a year.

Obesity drugs have another superpower: taming inflammation

The next generation of these drugs required fewer injections and led to greater weight loss. In 2021, the FDA approved semaglutide, sold as Wegovy by Novo Nordisk, as a weight-loss treatment, and in 2023 it approved tirzepatide, marketed as Zepbound by Eli Lilly, based in Indianapolis, Indiana. Tirzepatide mimics GLP-1 as well as another hormone that stimulates insulin secretion, known as glucose-dependent insulinotropic polypeptide (GIP).

Semaglutide and tirzepatide are weekly injections that can induce an average body-weight reduction of between 12% and 18%. In addition to their slimming effects, these drugs are marketed under different brand names, such as Ozempic, as treatments for type 2 diabetes. Clinical trials have also shown that they can reduce the risk of kidney failure, tame inflammation and protect against serious heart problems.

But the drugs are expensive a months worth of semaglutide or tirzepatide injections can cost upwards of US$1,000. One way that Indian and Chinese pharmaceutical companies plan to slash that price is by developing biosimilars, cheaper versions of expensive brand-name drugs. Unlike generics, which are exact copies of chemically synthesized branded drugs, biosimilars very closely resemble their reference product and are derived from modified living organisms, such as yeast.

Companies are preparing to release biosimilar versions of GLP-1 drugs when patent protections lift in different markets. In China, the patent for liraglutide has already expired, and the one for semaglutide will expire in 2026 in India and China.

Companies are also innovating in how they synthesize the drugs, fill the vials and manufacture the injection devices, says Zutshi, whose company is focusing on developing biosimilars for the global market.

How much the cost will drop is yet to be seen, says Zutshi. It could be cut in half, or be one-tenth the current price, he says. If drugs are cheaper, more people will use them, thus expanding demand, he adds. This, in turn, could have a ripple effect in which companies reduce the price of existing weight-loss drugs in anticipation of the competition coming.

No weight-loss biosimilars have yet been approved in India, but Glenmark Pharmaceuticals in Mumbai, India, launched Lirafit, a liraglutide biosimilar, in January for treating type 2 diabetes. Glenmark said in a press release that, at some 100 rupees a day (US$1.20), Lirafit costs 70% less than existing therapies. Biocon has also developed a liraglutide biosimilar, which was approved by the UK drug regulator for type 2 diabetes in March but their product is not on the market yet. Biocon says it is developing semaglutide biosimilars to launch in select countries as soon as 2026.

Chinas drug administration has approved two GLP-1 drugs produced by Chinese companies for weight loss. The first is a liraglutide biosimilar marketed as Liluping, made by Huadong Medicine in Hangzhou. The second is beinaglutide, marketed as Feisumei by Benemae Pharmaceutical Corporation in Shanghai.

Several companies in China are ready to pounce on the semaglutide market as soon as the patents expire in 2026. Half a dozen companies have launched late-stage clinical trials of semaglutide injections. One of them, Hangzhou Jiuyuan Gene Engineering, has completed late-stage clinical trials for its semaglutide biosimilar for treatment of type 2 diabetes, and could be the first to enter the Chinese market. A pending court decision on whether the semaglutide patent applies in China could see these drugs enter the market even sooner. Late-stage trials for the companys semaglutide biosimilar targeting weight loss are expected to start this year.

The arrival of semaglutide biosimilars will be a game changer and competition will be severe, says Lei.

Beyond biosimilars, companies in India and China are also developing new obesity drugs. For example, Mumbai-based Sun Pharmaceuticals has developed a molecule called GL0034 that has slight tweaks to its chemistry that distinguish it from semaglutide and alter the way it interacts with insulin-secreting cells and nerve cells, says Rutter, who is a consultant for the company. Not only is the ambition to have a cheaper product, but to have something which is as good, if not better than semaglutide, he says.

Beyond Ozempic: brand-new obesity drugs will be cheaper and more effective

Early-stage clinical trials of GL0034 have been very encouraging, says Rajamannar Thennati, executive vice-president for Sun Pharma, who is based in Vadodara, India. The results in three dozen participants show that high doses can lead to body-weight reductions of up to 10% after two months, according to a press release, but Thennati says it could take up to five years for the molecule to hit the market.

Sun Pharma is also in the early stages of developing a dual-target drug to compete with tirzepatide. Its structure is sufficiently different to be proprietary, says Rutter.

Another improved drug could arrive in the Chinese market even sooner. South Africa Runs Out of Insulin Pens as Global Supply Shifts to Weight-Loss Drugs – The New York Times

South Africas public health care system has run out of the human insulin pens that it provides to people with diabetes, as the pharmaceutical industry shifts production priorities to blockbuster weight-loss drugs that use a similar device for delivery.

Novo Nordisk, the company that has supplied South Africa with human insulin in pens for a decade, opted not to renew its contract, which expired last month. No other company has bid on the contract to supply 14 million pens for the next three years, at about $2 per pen.

Current manufacturing capacity limitations mean that patients in some countries, including South Africa, may have limited access to our human insulins in pens, said Ambre James-Brown, a spokeswoman for Novo Nordisk. The company did not reply to questions about which other countries are affected.

Novo Nordisks drugs Ozempic and Wegovy, which are widely prescribed in the U.S. for weight loss, are sold in single-use pens produced by many of the same contracted manufacturers who make the multidose insulin pens. A months supply of Ozempic in the United States costs about $1,000, far more than insulin.

Novo Nordisk dominates the global market for insulin in pens and has supplied South Africa since 2014. Eli Lilly, the other major producer, has indicated in recent months that it is struggling to keep up with the significant demand for its weight-loss drug Zepbound.

This is because of the global demand for Ozempic and these drugs, said Khadija Jamaloodien, the director of sector-wide procurement for South Africas health service. Theyre shifting the focus on the more profitable line.

The #1 Habit You Should Break to Lose Weight, According to a Dietitian – EatingWell

There are many healthy habits that can help you lose weight, like eating more vegetables, getting adequate protein, keeping a food journal and moving your body more. On the flip side, some habits can keep you from seeing the scale moveeating mindlessly, turning to food when you're stressed and drinking your calories, to name a few. But there is one habit that is deeply ingrained in lifelong dieters. And while most people think it's helping, it could be the number one reason you aren't seeing lasting results.

The number one habit you should break to lose weight is not eating enough. Yes, you heard that right. Eating too few calories, especially at breakfast and lunch, is keeping you from losing weight and keeping it off.

If you're a lifelong dieter, you've unfortunately heard countless times to "eat less and move more." And while a calorie deficit is required for weight loss, this advice is more often than not taken to extremes, leading people to eat 1,200 calories or less per day while also trying to burn 500 calories or more a day. If you haven't figured out already, this combination does not work for lasting weight loss.

Drastically cutting calories and overexercising is not the way to lose weight sustainably. While you may lose weight initially, you won't be able to keep it off because you'll be so hungry you end up eating back the calories and putting the weight back on. Plus, you'll be miserable in the process (read: hanger, headaches, tired and cranky).

Even if you aren't currently on a diet, you may still be intentionally or unintentionally eating too little at meals out of habit. Years of hearing "eat less" has conditioned you to find the lowest calorie options around. The emphasis on quantity of food at the expense of quality can get you into trouble.

It looks something like this: a non-fat yogurt for breakfast, a salad at lunch with chicken and then from 3 pm on things tend to go downhillcheese, crackers, endless grazing, a semi-healthy dinner, then dessert or more snacks post-dinner. In an attempt to start your day by keeping calories low, you end up so hungry by the afternoon that you end up eating more calories than your body needs by the end of the day.

Frequently eating later in the day can also keep your body in fat-storing mode instead of fat-burning mode. To understand this, we have to review blood sugar.

When you eat, food is broken down into glucose (sugar). This causes a rise in blood sugar, which stimulates the release of insulin, a hormone that takes glucose from the blood to cells for energy. Extra glucose is stored as glycogen in the muscles and liver. If there is any leftover after that, it is stored as fat. Therefore, insulin is released every time you eat. If you are eating more calories than your body needs, your body will store fat.

Changing any habit begins with your mindset. In this case, you must change your belief that you always need to eat less, even if it leaves you hungry. Hungry people aren't successful with losing weight and keeping it offsatisfied people are.

When you build balanced meals that leave you full, you won't constantly think about food between meals. Constantly thinking about food is not a lack of willpower, by the way. It's just your brain's built-in mechanism to alert you that you need to eat more.

Eating balanced meals that satisfy will allow your blood sugar to rise steadily, instead of spiking, and keep you energized for hours. It will then naturally come down versus crashing.

So, how do you do it if you are also trying to lose weight? The goal is to feel comfortably full while also maintaining a small calorie deficit every day so you can see results. This process will take some time and go slowly, but it allows you to enjoy your life while getting healthier. And it will be less likely to slow your metabolism down.

Fiber, protein and fat digest more slowly and help keep you full. Plus, they're found in healthy foods like nuts, seeds, fruits, vegetables, chicken, salmon, whole grains and yogurt. Eat every 3 to 4 hours instead of picking and grazing all day. This prevents the constant release of insulin and allows your body to burn fat instead of storing it.

For lunch and dinner, aim to make half of your plate vegetables, a quarter of your plate whole grains, and the last quarter of your plate protein. Then add some healthy fat, like olive oil or nut butter. Filling half of your plate with vegetables fills your stomach with fiber, so you feel full with fewer calories.

One common mistake is skipping a whole grain at lunch due to fear of carbs (complex carbs are your friend!). But this can backfire and lead to eating less healthful carbs in the afternoon. Try adding a half cup of cooked quinoa or farro to your lunch salad or make a sandwich using a whole-wheat tortilla. You'll be surprised at how much more satisfied you will feel.

Remember, because protein, fat and fiber are digested slowly and suppress hunger hormones, they keep you full longer. This slows the rise of blood sugar and the release of insulin.

‘My wife’s trying to loose weight but she keeps making the same mistake’ – Yahoo News UK

Shedding weight is no easy task.

While the concept seems straightforward consume fewer calories than you burn it can be challenging to adhere to, particularly for those who enjoy a good snack!

A man, who chose to remain anonymous, shared on Reddit that his wife was struggling with this exact issue. Despite attempting various diets, she continually making the same mistake.

He shared her struggle on the popular forum, writing: "Ever since the 2nd delivery she has been about 50lbs (three and a half stone) overweight. She is no longer breastfeeding and kids are sleeping well now, and our life is pretty stable. She often mentions that she wants to lose weight and has tried a few things off and on like Noom, my fitness pal, but is not very consistent.

"She does a lot of things that I feel are incompatible with losing weight. After eating a normal-sized dinner where she serves the same portion that I eat (I am naturally skinny, and not trying to lose weight), she will frequently have a late snack around 8:30pm or 9pm like a bowl of cereal, piece of toast, or ice cream.

"She also does not seem be able to deal with any amount of hunger. She has made comments like 'I need a snack before dinner, all I ate for lunch was an apple, a bowl of yogurt with Granola, and cheese with crackers'. To me, that sounds like an adequate lunch for someone who is trying to cut calories."

The husband expressed his concerns about his wife's late-night snacking and her fondness for baking, noting that she became "very defensive" when he broached the subject. Despite trying to support her by cooking healthy meals, he feels frustrated when she indulges in sugary treats afterwards.

Seeking guidance, he turned to the internet community, sharing: "I try to be supportive. I haven't made any comments since that one a few years ago. I cook healthy dinners like chicken or fish with lots of veggies. But I feel like she is always setting herself up for failure. Also diet advice from a naturally tall skinny dude is impossible to be empathetic."

He continued: "But after making a nice healthy dinner and then seeing her eat a big bowl of cereal three hours later makes me want to say some snarky remarks about why she isn't losing weight. Does anyone have a similar experience? Anything useful I could say? Or should I just continue to hold my tongue? ".

Fortunately, the online community came through with suggestions. While some advised him to encourage his partner to see a professional, others recommended he refrain from commenting on her weight.

One commenter advised: "You could suggest an appointment with a dietician to help her create a meal plan that works better for her tastes and unique hunger patterns, and who can also help her get a handle on any sort of emotional eating issues. But I don't think you pointing out what she's doing wrong is going to be productive."

However, another individual cautioned: "Omg there must be a course on this what not to say to your wife!"
