These men, in particular, were supplemented daily with 459 mol/d of zinc for six months.

Why bald men might be more susceptible to severe COVID-19 – 9News

The COVID-19 virus could be more prevalent in bald men due to factors linked to testosterone levels, new research has shown.

Testosterone produces a hormone called dihydrotestosterone which is responsible for balding.

The same hormone could also make people more susceptible to COVID-19.

"In terms of baldness, studies in Spain shows that bald men were over represented in terms of having severe outcomes of COVID," infectious disease expert Sanjaya Senanayake told Today.

"That might sound weird but there is a biological basis for it because bald men have a high levels of dihydrotestosterone which incidentally helps COVID get into cells."

A converse study conducted in Italy showed men with prostate cancer in hospital who were taking an anti-testosterone for cancer were four times less likely to get infected.

People with certain blood types may also be more susceptible to having severe symptoms if infected with COVID-19.

"With blood groups it looks like blood group O is more protective against COVID and A is more severe," Prof Senanayake said.

"It could mean that blood group O is less likely to cause clotting."

Researchers are also beginning to identify new and peculiar symptoms of the virus including blistered feet and a loss of taste.

"The loss of smell can occur with quite a few viral infections that affect the upper respiratory tract and coronavirus as a group can be responsible," Prof Senanayake said.

"It tends to effect the olfactory nerve, which is your nerve for smell.

"It can also cause a lot of inflammation in the nose and that can effect smell."

One patient being treated for COVID-19 in Wuhan reported a change in his skin colour, however Prof Senanayake said this was more likely to do with the antibiotics opposed to the virus itself.

"Antibiotics, although they are wonderful at treating infections they all have side fix and everyone knows about getting diarrhoea and an itchy rash and it can ever weird things like change of skin colour," he said.

Testosterone levels are about 10 times higher in men than women. While it does have important functions in women, its role is quite different so this article will focus on testosterone in men.

Testosterone and development

Testosterone is the most important male sex hormone (androgen); its needed for normal reproductive and sexual function. Hormones are chemical messengers made by glands and carried in the blood to act on various organs.

This particular hormone is important for the physical changes that happen during male puberty, such as development of the penis and testes, and for features typical of adult men, such as facial and body hair. Testosterone also acts on cells in the testes to make sperm.

Its important for overall good health; testosterone helps the growth of bones and muscles, and it affects mood, libido (sex drive), and certain aspects of mental ability.

Levels dont have to decline as you age

The hormone is present in the body from the early stages of fetal life to old age. At the earliest stage of development, it helps the fetus develop a male body and a male brain (there are gender differences, or sexual dimorphism in the human brain).

Levels are highest between the ages of 20 and 30. As men age, testosterone levels fall by about one per cent to two per cent every year, although recent research suggests this may not be true of all men as they age. It seems a large part of the drop in testosterone levels in older men is due to chronic conditions, such as obesity and diabetes.

If men remain very healthy into old age, their testosterone levels may stay the same as when they were younger.

Effects of too little and too much

Low testosterone is usually caused by a genetic disorder (such as Klinefelters syndrome, the commonest chromosomal disorder in males that leads to poor testicular function) or damage to the testes or, in rare cases, a lack of certain complementary hormones made by the brain.

Its thought that about one in 200 men under 60 years of age and about one in 10 older men may have low testosterone levels, but exact numbers are not known.

Low testosterone levels have a variety of effects at different ages. In young boys and teenagers, it means the testes and penis dont grow properly and theres poor development of muscles and facial and pubic hair. Boys with low levels of the hormone may be taller than their peers and their voice may not deepen.

In adults, low energy levels, mood swings, irritability, poor concentration, reduced muscle strength and changes in body fat distribution, and low sex drive may result from low testosterone. But thats not to say that low levels of the hormone are the only possible cause of these symptoms.

Research suggests that men with low testosterone may have a higher risk of chronic conditions, such as stroke and heart disease. Older men with low testosterone also have thinning bones and that puts them at risk of fractures.

Too much testosterone can cause problems too. Although people link the hormone with aggression, this hasnt held up under scrutiny. Rather, research has shown testosterone levels are associated with quite different traits, such as care-giving and empathy.

Supplementing it; a good or bad idea?

Longevity is highest for those men with testosterone levels in the mid-range not too high or too low and recent research supports the idea that too much or too little testosterone is best avoided.

For men with a clinical diagnosis of low levels, testosterone therapy can bring the amount of the hormone in their blood back to normal and restore and maintain good health. In boys, it can restore sexual development.

But in men with normal testosterone levels, taking supplements of the hormone is not appropriate and can cause problems. Taking testosterone can lead to a reduction in the size of the testes, and it can slow or stop sperm being made. And it can take many months to go back to normal once the man stops taking testosterone.

There is some controversy around studies suggesting that older men taking testosterone have an increased risk of cardiovascular disease but the jury is still out. What we do know is that theres no good evidence for the much-publicised benefits of testosterone supplements in old age, except for men with clinically diagnosed low testosterone.

Andropause: Not the same as menopause but needs to be addressed – The Star Online

First of all it is important to make clear that andropause isn't the male version of menopause, says consultant urologist Dr Datesh Daneshwal.

"While all women go through menopause, all men do not experience andropause. It's not supposed to happen to every man and it only affects a very small percentage of men," he says.

Andropause is a condition where men experience a decrease in their testosterone levels that is not normal. While all men do start losing testosterone after the age of 30, the decrease isn't very significant (about 1% a year) and should not affect their well-being. Very different from menopause which signifies a sudden decrease in a womans hormone levels that causes ovulation to cease.

But sometimes, when a man's biochemical dip is more drastic, it can cause a broad range of symptoms that affect their overall health and temperament.

These symptoms include a decline in muscle mass and strength, a buildup of body fat, a loss of bone density, flagging energy, emotional instability, deranged sugar and cholesterol levels, lowered fertility and fading virility.

"A drop in testosterone coupled with these symptoms is what is classified as andropause. When it interferes with your well-being and health," explains Dr Datesh. "Many people have a misconception that testosterone is purely for sexual function which it is not. It is the elixir that keeps the brain sharp, affects one's mood, muscle bulk and weight management and also collesterol and sugar levels. Low testosterone can cause insomnia and depression and, of course, sexual dysfunction too. Men need a certain level of testosterone to function well in all aspects of their life and if it dips too low, their health is at risk," says Dr Datesh

Getting help

David was 38 when he started noticing the symptoms.

"I was experiencing a gradual but progressive decline of erectile function, decreased libido, frequent and more prolonged periods of low moods, decreased energy levels and frustration, sadness and despair that my interest in the opposite gender was not what it once was," says David who is now 61.

Although he initially started taking Viagra to help with his sexual dysfunction, David decided to consult a doctor when the symptoms persisted.

"For many years, l absorbed myself in my work and largely shrugged off and just accepted my symptoms without question. ince retiring a few years ago, l have had the time to become more aware of the physical and psychological fallout these symptoms have been having on me. The biggest psychological symptom was the diminishing confidence that l keenly felt in pursuing new relationships with women (l am, and always have been, a single guy) not least because l was conscious of the increasing difficulty of consummating such relationships.

"On a physical level, l felt myself becoming more anxious and subject to low moods as well as experiencing decreasing energy levels. I felt that l owed it to myself to get my symptoms properly evaluated by qualified professionals in this area," he said.

David was diagnosed with hypogonadism, a condition where the body doesn't produce enough testosterone which can occur from birth or later in life, often from injury or infection. He is currently on testosterone replacement treatment as well as medicine for erectile dysfunction which he says has helped him tremendously.

"Physically, l feel more energised, l have a stronger libido and my erectile functioning difficulties have been greatly improved, with further improvements anticipated after further testosterone injections. Psychologically, my confidence in interacting with women has been greatly boosted l think this is the most dramatic change l have notice.

"My advise to other men who are going through this is to not ignore the symptoms but to take action! Go and get specialist advice initially from your doctor and better still from a Urologist who has special expertise in this area. Please do not be embarrassed about talking through your symptoms with a doctor or urologist, particularly around erectile dysfunction. Remember that these medical people have seen and heard it all before many, many times. ln most cases your symptoms are likely to be physiological rather than psychological and can be resolved with treatment," says David.

The stigma is real

Although there is a lot of literature available or andropause, men rarely seek medical help even when the symptoms can be affecting their life significantly, notes Dr Datesh.

"There is a stigma associated with this, unfortunately. And that's because we immediately associate testosterone with sexual function. That's why we need to raise awareness about how men need healthy levels of testosterone to lead a healthy life all around. If you are over 40 and are not feeling yourself notice that you put on weight, lose muscle bulk, emotional instability, find you are no longer attracted to your partner anymore, come and get tested. All it takes is a simple blood test and if it is because of low levels of testosterone, there is a solution now. You can get treated and change your life not just for yourself but for your family too," says the urologist from Prince Court Medical Centre.

Hormone replacement therapies for men come in the forms of pills, injections and gels although the most effective and popular option in Malaysia are thelong-term injections that are adinistered every three months.

"Hormone replacement is a science though and cannot be administered simply without ruling out a few things. First and most importantly, we have to make sure that the patient has no signs of prostate cancer because testosterone is fuel for prostate cancer cells. Second, while testosterone administered externally may help with a man's virility, it may affect his fertility. These things have to be discussed with the patient before he starts on treatment," says Dr Datesh.

How long a patient has to be on them really depends on the individual, he says.

I have had patients who, after a couple of doses, were feeling better and able to resume an active lifestyle which can stimulate the body's ability to produce it's own testosterone," he explains.

Men experiencing the symptoms of andropause are advised to make healthier lifestyle choices. Aerobic exercise to release endorphins that improve mood, strength training to increase blood flow and strengthen the hear as well a healthy diet and a decrease in alcohol and tobacco intake.

Muscle-building exercises, he says, can encourage the body's own production of testosterone.

"But a person experiencing andropause will not have the energy to do weight training - they'd need a boost of hormones get they them feeling energised enough to start exercising and building their body up," he says.

If you measure your manliness according to your testosterone levels, sadly the answer is likely no, according to current statistics comparing men today with the previous generations at a similar age. Naturally there is more to a man than his testosterone score, but testosterone plays a powerful role in how we feel as men. It is our dominant hormone responsible for the development of our sex organs along with playing a major role in so many of our health characteristics including muscle mass, bone density, libido, wellbeing and moods.

Last year the lab I use here in Phuket lowered the testosterone normal reference ranges (the range that is considered normal and okay) from previously 280-800ng/dl to now 193-740ng/dl nearly a 100-point decline. Why? The standard normal ranges for blood tests are often the result of plotting all the test results for normal people into a bell curve and identifying the outlying 5%, suggesting a strong testosterone decline, which is a worldwide trend.

Testosterone trends and statistics

Low testosterone has really become one of the biggest mens health and wellbeing issues today, with approximately 30% of men worldwide affected by low testosterone.

A global estimate of total testosterone sales from 41 countries indicates increases from US$150 million in 2000 to US$1.8 billion in 2011. This figure has a lot to do with more advertising and exposure via the internet to sell testosterone but likely also represents the greater awareness of men realising an issue and looking for a solution.

Low testosterone issues

Recent studies have demonstrated that low testosterone in men is strongly associated with metabolic syndrome, type 2 diabetes, cardiovascular disease.

But along with these issues is the often silent mental health issue that goes with low testosterone, namely depression. Studies are very clear in showing the relationship between low testosterone and depression.

One study from over 800 men showed a clear and significant inverse relationship between serum testosterone levels and depressive symptoms. As testosterone goes down, depression goes up.

The Massachusetts Male Aging Study (MMAS), which included 1,709 men aged 40 to 70 years, showed that each unit increase in the concentration of total serum testosterone results in a 10% reduction in the risk of depression.

So what does a guy look and feel like with low T? Well simply not himself low energy, vitality, low sex drive are classic symptoms. He easily gains fat around the midsection, that male pregnant look and its hard to lose it even if he tries with diet and training.

It is critical for those wishing to supplement with testosterone to do so only under medical supervision. Labs need to be done regularly to see you are in the right range for your body more is not better. Over-supplementation can cause many problems including increases in estrogens (due to testosterone converting/aromatising to estrogen), which is bad news for a male body.

If you are not checking you are just guessing

My recommendation for all men over 30 is every year please get a physical with labs included and insist on them adding testosterone to keep an eye on it. You really have to insist as its not often done unless you say you have a low libido. If possible ask for also SHBG and albumin (to work out free and bioavailable testosterone), Estradiol, and DHEA. Testosterone is the only hormone people use in masculinizing hormone therapy, says Amy Weimer, MD, a primary care physician with a clinical interest in transgender care at UCLA Health.

Depending on the dose, frequency, chosen method, and the person's genetics, you may start to see masculinizing changes within the first week or a few months. Here's a timeline showing some of the changes you can expect:

2. Hair growth. Existing facial and body hair will thicken, darken, and grow at a faster rate. More hair will grow on the chest, back, arms, legs, and rest of the body. You'll start to grow a more pronounced mustache and beard too. Some may also experience male pattern baldness if it runs in the family.

5. Body fat redistribution. Taking testosterone decreases the body's fat mass, according to a 2018 study published in the European Society of Endocrinology. This is most noticeable in the hips, legs, arms, and face.

Use condoms (either external or internal) and dental dams every time you have sex to reduce the risk of STDs and pregnancy. Those who go off testosterone therapy may become pregnant again with the return of menses, but you should talk to your doctor before attempting to get pregnant.

10. Genital changes. The cells that make up the clitoris will try to grow into a penis. Unlike a penis, the urethra will remain separate. However, the clitoris may grow one to two inches, though it can grow larger depending on your genetics.

The vaginal walls will also become thinner and more prone to tearing after starting testosterone. Natural lubrication also decreases. Some trans men also report pelvic pain, as testosterone could affect the uterus and pelvic muscles.

Excess red blood cells could obstruct how well the body absorbs oxygen in the blood, says Safer. It could also lead to an increased risk of blood clots.

However, these physical changes will take time, and the exact process may be different depending on the individual and their metabolic and genetic makeup.
