Millions of American men use a prescription testosterone gel or injection to restore normal levels of the manly hormone. The ongoing pharmaceutical marketing blitz promises that treating "low T" this way can make men feel more alert, energetic, mentally sharp, and sexually functional. However, legitimate safety concerns linger. For example, some older men on testosterone could face higher cardiac risks.
"Because of the marketing, men have been flooded with information about the potential benefit of fixing low testosterone, but not with the potential costs," says Dr. Carl Pallais, an endocrinologist and assistant professor of medicine at Harvard Medical School. "Men should be much more mindful of the possible long-term complications."

A loophole in FDA regulations allows pharmaceutical marketers to urge men to talk to their doctors if they have certain "possible signs" of testosterone deficiency. "Virtually everybody asks about this now because the direct-to-consumer marketing is so aggressive," says Dr. Michael O'Leary, a urologist at Harvard-affiliated Brigham and Women's Hospital. "Tons of men who would never have asked me about it before started to do so when they saw ads that say 'Do you feel tired?'"
Just being tired isn't enough to get a testosterone prescription. "General fatigue and malaise is pretty far down my list," Dr. O'Leary says. "But if they have significant symptoms, they'll need to have a lab test. In most men the testosterone level is normal."
If a man's testosterone looks below the normal range, there is a good chance he could end up on hormone supplementsoften indefinitely. "There is a bit of a testosterone trap," Dr. Pallais says. "Men get started on testosterone replacement and they feel better, but then it's hard to come off of it. On treatment, the body stops making testosterone. Men can often feel a big difference when they stop therapy because their body's testosterone production has not yet recovered."
This wouldn't matter so much if we were sure that long-term hormone therapy is safe, but some experts worry that low-T therapy is exposing men to small risks that could add up to harm over time.
A relatively small number of men experience immediate side effects of testosterone supplementation, such as acne, disturbed breathing while sleeping, breast swelling or tenderness, or swelling in the ankles. Doctors also watch out for high red blood cell counts, which could increase the risk of clotting.
Men on long-term testosterone appear to have a higher risk of cardiovascular problems, like heart attacks, strokes, and deaths from heart disease. For example, in 2010, researchers halted the Testosterone in Older Men study when early results showed that men on hormone treatments had noticeably more heart problems. "In older men, theoretical cardiac side effects become a little more immediate," Dr. Pallais says.
Some physicians also have a lingering concern that testosterone therapy could stimulate the growth of prostate cancer cells. As with the hypothetical cardiac risks, the evidence is mixed. But because prostate cancer is so common, doctors tend to be leery of prescribing testosterone to men who may be at risk.
For men with low blood testosterone levels, the benefits of hormone replacement therapy usually outweigh potential risks. However, for most other men it's a shared decision with your doctor. It offers men who feel lousy a chance to feel better, but that quick fix could distract attention from unknown long-term hazards. "I can't tell you for certain that this raises your personal risk of heart problems and prostate cancer, or that it doesn't," Dr. Pallais says.
So, keep risks in mind when considering testosterone therapy. "I frequently discourage it, particularly if the man has borderline levels," Dr. Pallais says.

Your testosterone level should be measured between 7 am and 10 am, when it's at its peak. Confirm a low reading with a second test on a different day. It may require multiple measurements and careful interpretation to establish bioavailable testosterone, or the amount of the hormone that is able to have effects on the body. Consider getting a second opinion from an endocrinologist.
After starting therapy, follow-up with your physician periodically to have testosterone checks and other lab tests to make sure the therapy is not causing any problems with your prostate or blood chemistry.
Approach testosterone therapy with caution if you are at high risk for prostate cancer; have severe urinary symptoms from prostate enlargement; or have diagnosed heart disease, a previous heart attack, or multiple risk factors for heart problems.
Ask your doctor to explain the various side effects for the differentformulations of testosterone, such as gels, patches, and injections. Know what to look for if something goes wrong.
Testosterone therapy is not a fountain of youth. There is no proof that it will restore you to the level of physical fitness or sexual function of your youth, make you live longer, prevent heart disease or prostate cancer, or improve your memory or mental sharpness. Do not seek therapy with these expectations in mind.

Now, you can buy supplements that contain concentrated saw palmetto berries. The Mayo Clinic reports that saw palmetto is a popular alternative therapy for enlarged prostate (source). It has been also used as a treatment for low sperm count and low libido.
The reason saw palmetto works as a treatment for enlarged prostate lies in its ability to stop the breakdown of testosterone. As testosterone deteriorates, it turns into dihydrotestosterone.
This conversion causes enlargement of the prostate. By allowing your body to retain more testosterone, saw palmetto prevents the prostate from growing as rapidly as it might otherwise.
However, some argue that dihydrotestosterone is a more potent version of testosterone. Dihydrotestosterone is responsible for the growth of facial hair, the building of muscles and sexual function. While its true that saw palmetto can help men retain testosterone, it comes at the expense of the conversion of testosterone to dihydrotestosterone. Further research must be conducted to determine the effectiveness of saw palmetto in supporting healthy testosterone levels in men who dont have an enlarged prostate.
Boron can help the body create more free testosterone and less estrogen (source). After taking 10 milligrams of boron for a week, participants experienced increases in free plasma testosterone and decreases in estrogen (source). However, dihydrotestosterone and cortisol levels were elevated. More studies need to be done for conclusive evidence that taking boron can help boost testosterone.
Testofen is a supplement marketed for increasing testosterone levels. It contains fenugreek extract.
One study found that men who took 600 milligrams of Testofen per day for six weeks experienced increased libido and arousal (source). Participants did not experience side effects, like the sleep interruptions or mood disruption that can be caused by testosterone replacement therapy. Researchers hypothesized that Testofen may help to support healthy testosterone levels.
As you get older, your hormone levels are disrupted. In fact, the decline of testosterone and IGF-1, another anabolic hormone, increases the risk of metabolic syndrome, diabetes and mortality in the elderly (source). Magnesium consumption increases the bioavailability of testosterone.
D-aspartic acid is an amino acid that helps create testosterone. Athletes may use it to temporarily raise testosterone levels. It has also been used to treat infertility (source).
This supplement can help the brain release hormones that support testosterone production. The amino acid can also build up in the testes, leading to a slight increase in testosterone levels.
Studies have found that d-aspartic acid may be most effective at increasing testosterone levels in infertile men. In one study, infertile men who took 2.66 grams of the supplement every day for 90 days experienced a 30 to 60 percent elevation in serum testosterone levels (source).
However, another study that involved men who performed heavy resistance training while taking 3 grams of d-aspartic acid for 28 days found no change in testosterone levels (source). Other studies have had similar results.
If you have problems with infertility, this may be an effective supplement. It has not been reported to cause significant side effects.
The most common side effect is acne. This may be a result of the shift in hormones caused by the supplement. It may also cause chronic headaches.
Ginger may activate antioxidant enzymes that fight toxins in the testes (source). Researchers have found that ginger extract can increase testosterone level in infertile men (source).
If you have healthy testosterone levels, its unclear whether ginger will elevate them. However, ginger has other beneficial effects on health and sex hormones (source).
Diindolylmethane, or DIM, is a molecule that is pervasive in broccoli, kale and cauliflower. Its most significant effects are on the metabolism of estrogen (source).
It is an aromatase inhibitor and therefore prohibits the transformation of testosterone into estrogen. It also turns more powerful types of estrogen into less potent forms. If you take too much DIM at once, however, it can cause a surge in estrogen.
Creatine supplementation can elevate the levels of dihydrotestosterone in the blood. Some people recommend cycling creatine in a loading phase followed by a maintenance phase (source).

Normal Testosterone Range for TRT in Men – Aged 30+

After one month of testosterone injections of 100mg per week (.5 mil of 200mg/mil injectable oil) using testosterone enanthate my test levels are at 502. I wanted to take what was prescribed for awhile to see where that would put me. This was also near the end of the week so Im guessing that is the lower end of the spectrum, although when injecting enanthate (generic form of Delatestryl) every week the spectrum probably isnt too wide. My guess is Im anywhere from between 500 and 800 ng/dl. The government website Medline Plus puts the normal range for men at 300 to 1,200 ng/dL. Given that this includes 25-year-old guys at the peak of their physical condition, I think maintaining somewhere between 500 and 800 is going to work out well for me healthwise. I feel good in this range.
Now with that said, I am hereby embarking on the Blast phase of my year, where I will be taking double that amount for several months before tapering back down to 100 mg per week. I believe at 200mg per week I will still be within safe-enough levels to avoid any major side effects (hopefully) associated with steroid use, as bodybuilders frequently start at a minimum of 500mg per week (up to more than 2 grams in some cases) in addition to several other androgenic / anabolic steroids like nandralone, stanozolol, dianabol and equipoise. At this point one needs to take a whole host of other medications to combat the side effects, both during the cycle and during the post-cycle-therapy (PCT) phase, including things like tamoxifen citrate (generic Nolvadex), clomophene (Clomid), and anastrozole (Arimidex). Since I dont plan on having any more children, I am not even taking HCG. Right now its just testosterone, although I do plan on having some Nolvadex and/or Arimidex on-hand just in case I feel any estrogen-related side effects.
So thats the plan. If Im going to be on testosterone for the rest of my life, I might as well enjoy the benefits of being able to have a great sex drive and physique for the rest of my life without having to hassle with the crashing testosterone levels experienced at the end of normal testosterone cycles for men who arent on lifelong TRT.
Please keep in mind that A: I am not interested in being a bodybuilder and do not need to take 500mg of testosterone to be in good shape. B: I want to avoid as much ancillary medication as I can without experiencing estrogen-related side-effects. C: I do plan on tapering from 200 mg/week, to 150 mg/week for one month before going to the normal 100 mg/week to give my body time to metabolize some of the estrogen and catabolic hormone build-up while staying in the proper balance of having more testosterone in relative amounts to the other hormones.
