The question of medication: Life modifications might be an option for children with ADHD – The Spokesman-Review
There are more kids in Spokane on ADD medication than in all of France, Dr. Leonard Sax said during our conversation about raising boys for last weeks column.
Sax, the author of the insightful Boys Adrift, was on the money. I was surprised but not shocked. America is an overmedicated country. Swallowing a pill is easier than modifying your diet and, more so, your life. Thats the case for children and adults, as well.
It takes doctors less time to hand over a prescription than spend considerable time, more than typically is allotted, during a doctors visit to assess whether a child needs Ritalin.
Some children absolutely require a drug, which helps them focus. Kids do suffer from attention deficit hyperactivity disorder.
Dr. David L. Hill, a North Carolina-based pediatrician, strongly suggests parents with a child, who might have attention issues, to be proactive.
ADHD is a real thing and if someone has it, you cant ignore it, Hill said while calling from his Wilmington, North Carolina, practice. When it comes to ADD, you can find differences on an MRI. You can find differences on various tests on function. There is a descriptive diagnosis. There are tests, which track eye movements and motor functions, which identifies whether a child has ADHD.
There is no doubt some children need medication to quell their impulses and home them in on schoolwork, but why is it that comparable countries around the world have far fewer children on ADHD medication? What should parents do when they suspect their childrens lack of attention could be a problem in school?
Before visiting a physician, parents should take a look at their childs actions and environment. Examine their diet, activity and sleeping patterns. If a child is exhausted, it impacts their attention span.
Dr. Gregory Charlop, an authority in wellness and advanced nutrition who wrote the book Why Doctors Skip Breakfast: Wellness Tips to Reverse Aging, Treat Depression and Get a Good Nights Sleep, believes rest is most critical for kids and particularly teens.
What parents might not realize is that teenagers need more sleep than prepubescent children, Charlop said while calling from his Pasadena, California, office. Sleep for teens is a big deal.
Sleep is so significant that California Gov. Gavin Newsom signed into law California Bill 328 in October that requires high schools to begin classes no earlier than 8:30 a.m. and middle schools at 8 a.m.
Its a progressive move by California, and it makes so much sense, Charlop said. Teenagers circadian rhythms shift. They naturally want to go to sleep later. Teenagers are night people. Theyre designed to go to bed later and get up later. If high school starts at 7 a.m., a tired student could be viewed as a teen who has attention deficit disorder.
Diet is as significant as sleep. Children need to eat and eat properly, Hill said. Attention will decline if a child doesnt eat. Is there enough food in the home? Is the food healthy? Avoid sugary snacks. A good balance of foods is essential for growth and attention.
Sedentary children often have energy issues. Children need to be active, Hill said. What I never understand is when I hear about how a child is punished in school. What happens often is that theyre not allowed to go to recess. You dont want to take away physical activity.
That leads to behavior issues. You want kids to run around the football field a couple of times. They need to get that energy out, or they may have trouble focusing in the classroom.
Anxiety, which can cause attention deficit, can be an issue. Is there something at home traumatizing a child, Hill said. Is there domestic violence or substance abuse? Those are reasons a child can be unsettled in class.
If your child is sleeping eight hours or more a night, eating healthy, is active and lives in a stable home and still has issues, its time to visit a medical professional.
Dr. Jacqueline Jones, the author of Medical Parenting: How to Navigate the Health, Wellness & Medical System With Your Child, believes parents should schedule a thorough examination.
Jones suggests a neuropsychological evaluation. Its a three-hour test that details how a childs brain works. The pro of the test is that it provides a detailed examination. However, the con is the test is expensive, and insurance typically doesnt cover the procedure. If you want answers and can afford the test, its a great investment.
However, if thats not so, whats best is scheduling an appointment that is extensive. Parents need to do their due diligence. You dont want your children to fail or fall behind, so if there are issues, visit a doctor to see what the next step is, Jones said.You dont want your children to be misdiagnosed.
Dyslexia, bipolar disorder and oppositional defiant disorder are examples of medical conditions that might appear like attention deficit disorder.
Sax suggests exhausting all options and doing your best to determine if your child has ADD. It isnt easy for a parent to discern whether a child has an attention deficit. Its best not to just let a child try the medication.
Sax recalls a conference he attended 14 years ago at Harvard University titled Learning and the Brain. MIT professor John Gabrielis team obtained permission to give powerful ADHD medications to children without attention issues and those with issues. The medication for ADHD improved the performance of children who were ADD addled, but the same was so for those with normal attention spans.
So if a parent were to witness their child even more focused with better grades, they would probably think the medication was necessary. Sax is convinced America is overprescribing stimulation medications.
Sax suggests trying to find a pediatrician or family physician who has training and experience to perform a sophisticated assessment of a boy in the kindergarten age range. That medical professional can determine whether that childs difficulties are due to ADHD or another problem.
Its apparent that a drug like Adderall will improve academic numbers just like the medication will enhance a baseball players performance. The drug is prohibited by Major League Baseball.
It doesnt take long for children and parents to realize ADHD medications have side effects. Appetite suppression is a common issue children have who take Concerta or Ritalin. Sleep issues, dizziness and moodiness are some of the problems the meds might cause. A number of my childrens friends have pretended to consume the medication. Ive had friends find the medication hidden in their kids rooms.
You dont want your children to be on medication if they dont need to be on it, Sax said. A parent must do whats best for their child. It takes quite a bit of work to be a parent.
But its worth every bit of the effort. Its a role that is fleeting. Thats evident for me since I have a daughter in college, and a son who will experience life at his university in the fall. I have a high schooler and another in elementary. The ride will be over soon. Part of the experience was finding out whether my boys have ADD. It was a process, but it was just part of the parenting journey.

The Daily Mail reports that after decades of working with Anna Wintour, Talley is taking aim at Wintour for her cruelty and mercilessness, making it clear that their friendship is over. Talleys memoir dives into his time with the woman who inspired Miranda Priestly in The Devil Wears Prada.
Known for her signature sunglasses and bob haircut, Wintour has been the editor-in-chief at Vogue since 1988. She showed interest in fashion early and had a long career in the industry and journalism before landing the top job at Vogue. Wintour started working retail in London before stints with publications like New York Magazine and House & Garden. In addition to her more than full-time position with Vogue, Wintour hosts the annual Met Gala. The event is widely regarded as the Super Bowl of fashion.
Outside of work, Wintour has been married twice. She had two children with her first husband David Shaffer, but the pair divorced in 1999. Wintour is currently married to businessman Shelby Bryan.
Wintour famously has a reputation as a cold, unforgiving person. Her icy exterior earned her the unkind nickname Nuclear Wintour. On top of that, she is rumored to be the inspiration behind Meryl Streeps character in The Devil Wears Prada. The rumors are generally believed; Miranda Priestlys meticulous appearance and taxing management style seem to be directly ripped from Wintour. The Daily Mail reports that one of her two assistants did have to go to her home every morning with a copy of the big book, a mockup of the current issue of Vogue, with flowers, gifts and all her clean clothes.
While that morning routine is unthinkable for most people, possibly the most unrelatable aspect of Wintours life is that her kitchen is always clean because she never cooks. While Wintour maintained her aloof position for decades, her image recently softened slightly thanks to The First Monday in May and appearances with Taylor Swift on Vogues Go Ask Anna segments.
Whatever friendlier image Wintour had been cultivating has been shattered by the claims made by Talley in The Chiffon Trenches. The longtime friend and colleague says that Wintour has left him scarred, both emotionally and psychologically. Talley counts himself among the many Wintour has left in a frayed and tattered heap during her powerful rule.
Talley tells readers that during his ongoing struggle with his weight, Wintour called him and told him to go to the gym. After Talley unsuccessfully tried to lose weight with a diet where he only ate cabbage, Wintour reportedly staged an intervention for him and told him his weight was out of control. Finally, Talley was sent to rehab at the Duke Diet and Fitness Center. While this could all be construed as the actions of a concerned friend, Talley says that Wintour is not capable of human kindness.
In addition to Wintours focus on his weight, Talley details how their professional relationship fell apart. Allegedly, Wintour froze Talley out, unceremoniously canceling his podcast and treating him to a sphinx like silence. Talley says this happened because Wintour deemed him too old, too overweight, too uncool for her.
Though the memoir isnt due out until September, what we have seen is already making waves in the fashion industry. The Daily Mail quotes designer Betsey Johnson as saying, I think its great that Andr has called Anna out. Except for The Devil Wears Prada, nobodys had the balls to say how she really is because everyones scared of Annas power.
While this is not the first falling out between Wintour and her former right-hand man, it seems likely to be the last. However, Talley said, My hope is that she will find a way to apologize before I die.

The simple fact that food is so easy to access when you're at home can lead people to eat more, says McMillan, but stress and anxiety are also in oversupply at the moment, and these too can lead us to seek comfort in the fridge. What's more, it's not your imagination that sugary and fatty foods in particular make you feel better.
These increase the feel-good chemicals in the brain so they make you feel better in the short-term, says McMillan. But of course long-term that is a nightmare for physical and mental health if too much of it goes on.
Make eating well easier by stocking your fridge and pantry with healthy, delicious foods.
Grazing your way through the working week, even if the food is relatively healthy, is not ideal. For the vast majority of us, snacking too often just means we eat more, are never truly hungry or truly satisfied, and we fail to get the balance of foods we need," says McMillan. "We also want to give our gut a break between meals to digest food.
Try to stick to a regular eating routine and have a 12-hour fast between dinner and breakfast, as a minimum.
While it's important to eat well McMillan says it's also important to be kind to yourself in these difficult times.
This is not about trying to be Insta fit or a diet guru looking fabulous at home every day, she says. Go easy on yourself. In my world, coronavirus or not, guilt has no place with food or eating.

Try not to label foods as good or bad as that implies you are good or bad when you eat them.
Instead, see foods in a positive light and think more about what you want to be eating more of real whole foods and give less attention to what you think you shouldnt be eating. McMillan recommends leaving your work space to eat in order to enjoy your meal without distractions.
Try to resist the temptation to stock up on treats you know you are at risk of overeating, says McMillan, and keep food items away from your work space.
If the urge to continue eating treats such as chocolate arises, ask yourself if you really want it, and wait 10 minutes, distracting yourself with another activity. If the urge is still there, serve yourself a measured amount and put the rest away, out of sight.
Healthy eating is at the core of looking after your mental and physical health. We are going to be in this situation for a long time, lets be honest, and bad eating for months can do a lot of damage.

This wont last forever. We will be more free to roam around. Its likely going to happen by summertime, and it will be nice to get to see people we havent in a while. Well, except it will be weird, knowing our friends might look at us and notice how much more plump we have become.
1 Timothy 4:8 says exercise has some value. Some translations say little value. Still theres some good in exercising. We should do more than just sitting and laying around these days. However, I would like to help you out with some very practical things which can be huge in helping you become smaller.
But first, I need to remind you about one particular fruit of the Spirit. Galatians 5 has the whole list, but one term in it is vital in this discussion. Christians should exemplify the fruits of the Spirit. What that means is if youre a Christian, you should demonstrate your faith as you let the Spirit guide your behaviors. Jesus said you can tell a legitimate Christian by the fruit they produce. What fruit of the Spirit am I talking about? Answer: self-control.
Im about to suggest a very simple solution which really works, but it requires self-control. Non-Christians, you, too, can benefit from what Im about to tell you, but still, youll need self-control.
Besides pastoring and counseling, Ive spent years as a weight-lifting coach and personal trainer. I can tell you that trends come and go in weight loss methods. Yet, there is a very real, tried and true way that works for all of us.
It is always advisable to check with your doctor before you try any new exercise or diet plan. Some people need to be extra careful because they have underlying health issues which complicate things a bit. Still, the overall concept Im about to share is functional, practical, and simple.
Believe it or not, you dont even have to exercise more. Score! Seriously, you dont. However, it can help if you do.
Ive watched overweight people struggle in the gym, and know the struggle is real at home, as well. People get it in their heads they have to work out long and hard in order to be successful in weight loss. That simply is not true. There is no reason to burn yourself out of the idea of healthy living by overdoing the exercise part. Do I recommend exercise? Of course I do. However, there are those who struggle so much they need to lose some weight first. Then, it wont be such a huge task to exercise, later.
Weight loss is as simple as a mathematical equation. Im not joking. Stay with me, and youll see. It works. Ive seen it work with hundreds of people. It can work with you, as well.
The math works like this. 1 pound = 3500 calories. For every pound you need to lose, you need to create a 3500 calorie deficit per day, week, or month. So, if you want to lose 10 pounds in 2 months, you need to create a deficit of 35,000 calories in 2 months.
Lets say a person wants to lose 20 pounds, and would like to do so by the end of July. As of this writing, thats a little more than 13 weeks away. Lets also say this person has been drinking 64 ounces of Dr. Pepper since the quarantine started. 64 ounces of Dr. Pepper is 800 calories. If this person doesnt exercise any extra, but switches to Diet Dr. Pepper, he/she would be creating a calorie deficit of 800 per day (Diet Dr. Pepper has 0 calories.). Look at the math, below, to see how fast he/she can lose 20 pounds simply by switching drinks like that.
800 x 7 = 5600 calorie deficit per week. 5600 x 13 weeks = 72,800 calorie deficit. If losing 20 pounds is the goal, 3500 calories x 20 = 70,000. So, even though he/she would only need a calorie deficit of 70,000, he/she will have created one of 72,800. That means this person will have lost the 20 pounds before the full 13 weeks, and will have lost nearly another pound by the end of the 13 weeks. Boo yah!
Do you see how simple the math works? Its not about metabolism. Thats already factored in. Its a matter of creating a calorie deficit, and it works for any adult with no underlying health conditions, and even with many who do. Its universal between males and females, as well.
A word of caution for adults: Dont reduce your daily caloric intake to below 1000. That can trick your body into thinking it needs to store more. So keep it over 1000 per day, and if youre going to go that low, you should be supplementing with a multivitamin.
A word of advice in keeping balance in your diet: A good rule of thumb if youre tracking what you put into your body is 1 gram of fiber for every 100 calories. There are several smartphone apps out there which can help you track your calories and more. Always count everything you consume. That includes gum (even if you spit it out, and you always should), supplements, drinks, or whatever. If you dont want to or cant use a smart phone, use a note card or something similar. Personally, since I do exercise, I track my protein, fiber, and calories.
If you need to determine how many calories you have been or should be consuming to maintain or lose weight, online calorie calculators are, also, readily available and free. These work for children as well. The dont go under 1000 calories per day does not apply to children.
Many of us who have been overeating during this pandemic can actually lose as much as a pound per week. Thats some of the simplest math. If you can safely create a calorie deficit of 500 per day, you will lose 1 pound per week (500 x 7 = 3500 calories, which = 1 pound).
For those who believe they are ready to exercise, there are, also, calorie burning calculators online which can help you figure out how much exercise it takes to create more of a calorie deficit. Ill warn you, it takes a lot of exercise to create calorie deficits. However, for every pound of muscle you add to your body, you will burn an additional 50 calories just sitting still in a given day. This is why people with more muscle can eat more without getting fat.
For example, using a calorie burning calculator, I can know that a man at my age, height, and weight, will burn around 300 calories, bicycling on the trails in my neighborhood for an hour. If I wanted to lose a pound per week, all I would have to do is create a 200 calorie deficit per day by changing eating habits, and bicycle an hour a day on the trails in my neighborhood. Obviously, its a whole lot easier, just creating the deficit in my diet. However, if I wanted, I could do that and add a 2 hour bicycling regimen and easily lose 2 pounds per week.
It really is that simple. A 3500 calorie deficit = a 1 pound loss. Self-control or lack thereof will determine how well our jeans fit at the end of this stay home mandate.

The Gut Health Doctor continued: Unlike other types of carbohydrates, our human cells cant digest fibre. Its sole purpose is to feed our gut bacteria, which, in turn, produce beneficial compounds that help to balance our blood sugars and tell our brains that were full.
At the moment in the UK, were only getting 19g a day of fibre on average, out of the recommended 30g daily. Its a good idea to increase your fibre intake to help with blood sugar balance.
Healthline says that eating more foods rich in fibre can be an effective strategy to help lose weight.
However, it wont lead to long-term results unless you pair it with a lasting lifestyle change.
A long lasting lifestyle change refers to daily exercise, getting plenty of sleep and nourishing the body with good whole foods.
Dr Rossi suggests aiming for two pieces of fruit, five portions of vegetables, three portions of wholegrain, one to two portions of nuts or seeds daily.

Over a week has passed since Ramadhan started in Brunei Darussalam, and since then, Muslims have been fasting every day from dawn to sunset.
During this time, perhaps youve heard your friends or family members say, Im going to lose weight this Ramadhan.
Losing weight, if done in a healthy way, comes with many health benefits such as decreased risk of diabetes, lowered blood pressure, improved mobility, as well reduced back pain and joint pain, among many others.
Several studies suggest that weight loss is also associated with positive changes in our lifestyle and psychological well-being such as better sleep, improved mood and increase in self-confidence.
As such, its important to look after our health by managing our weight.
OBESITY
Overweight and obesity are defined by the World Health Organization (WHO) as abnormal or excessive fat accumulation that may impair health. Obese people are at higher risk for potentially serious health conditions such as type 2 diabetes, stroke, joint pain, sleep apnea and breathing problems.
Based on the Brunei National STEPS Survey on Non-Communicable Disease and Risk Factors 2015-2016, 28.2 per cent of people in Brunei Darussalam are obese. For people aiming to lose weight, fasting during Ramadhan can be the first step of their weight loss journey.
To lose weight this Ramadhan and after Ramadhan ends its useful to understand how weight gain and weight loss works.
ENERGY IN, ENERGY OUT
The WHO website states that the fundamental cause of obesity and overweight is an energy imbalance between calories consumed and calories expended.
To put it simply, consuming more calories than the amount you burn will make you gain weight over time.
Think of calories you eat as energy in, and calories you burn as energy out. While energy in mainly comes from the food we eat, there are many ways for us to burn calories energy out even when we do nothing.
The simple act of just breathing burns calories, because our bodies are constantly at work to make sure all parts of our bodies are functioning properly.
Of course, working out burns calories too, and comes with many health benefits to boot. When it comes to losing weight, both energy in and energy out must be taken into account.
For a person to lose weight, energy out must be greater than energy in. This can be done by increasing physical activities or eating less, or doing both at the same time. Limiting food intake can be an effective weight loss tool, and several studies have researched the weight change during and after Ramadhan.
STUDIES: RAMADHAN FASTING AND WEIGHT CHANGE
According to a study published in the Journal of Public Health, out of a sample size of 202 people, 46 per cent lost over one kg after a month of fasting during Ramadhan. Another 16 per cent lost 0.5kg-one kg.
Meanwhile, 21 per cent maintained their weight within a range of 0.5kg. The remaining 17 per cent gained at least 0.5kg, showcasing the possibility to gain weight during Ramadhan by overeating. But its also possible to lose weight this Ramadhan, as shown by the study results, and its important to remember that just regulating the food intake during Ramadhan isnt enough.
Even harder than losing weight is what comes next keeping the weight off.
In the same study, 87 participants who recorded their weight a month after Ramadhan ended gained back the weight they lost.
A recent study published in Nutrients, a peer-reviewed open access journal of human nutrition, mirrored the results of the study mentioned above.
Although the study participants lost weight at the end of Ramadhan, they also gained back the weight within five weeks after Ramadhan ended. LONG-TERM WEIGHT MANAGEMENT
Eating less and eating healthy is easier said than done, but it is doable, and is perhaps more manageable during Ramadhan when we limit our food intake between sunset and dawn. That can be the first step to weight management.

BULLETPROOF's Noel Clarke admits his desire to lose weight came halfway through season 2, came after spotting "neck rolls" on camera.
After watching the scenes theyd previously filmed for the show's new series, they were prompted to hit the gym and start toning up.

Noel, 44, along with his partner in catching those who do the crimes Ashley Walters, 37, spoke to Metro about why they started to exercise midway through shooting the new series of the cop drama.
Clarke said: "Theres a little scene in episode one where Im mocking Munroe (played by Jason Maza) and Ive got neck rolls. Ive got f***ing neck rolls, like three hot dogs at the back of my neck."
Ashley chimed in: "In the first scene, theres a part where I come in and when Im walking in my stomach is like that [out]."
Kidadulthood star Clarke then mentioned: "He did this really cool stunt where he tackles a guy off the motorbike and he stands up all cool and his belly is out there, so we both looked at each other like, nope, we cant have this."

The duo are clearly happy with their new and improved bodies and took to Instagram to show off their hard to work to their fans.
After the pair scenes finished shooting scenes in Malta, they returned to the UK and began their transformation, which consisted of a diet plan and working out regularly.
Noel revealed: "You see the differences in episode one, if you binge-watch it, youll see the difference."
Revealing when he decided he wanted to make a change, Top Boy star Ashley exclaimed: "I just remember waking up and thinking, 'Im fat!!! Nooooo!'"

Both Noel and Ashley stressed how hard they fought to get roles such as the ones they occupy in Bulletproof, as they were sick of being portrayed negatively and wanted to feature in a role that youngsters can try and emulate.
Noel explained: "We want to show young black youth and young working-class youth that actually, you dont always have to play the criminal. Theres more that you can do."
Ashley added: ""We were sick of being offered the same roles. I love those shows. I would never shun it, but at the same time, it was good for me to portray a character that maybe my kids can aspire to."
Bulletproof airs Fridays on Sky One at 10pm.

Claire Brough used to hate her legs. They were, she felt, the biggest part of her, and she had really bad cellulite.
Now theyre like a long-lost friend shes become reacquainted with, admitting shes learned to love them as they carry her through countless half-marathons.
Thats just one of the health benefits the 53-year-old mum-of-four has enjoyed after years of despairing over diets that didnt work and failed weight-loss plans.
But it all clicked for Claire the day she walked through the doors of a Slimming World group 22 years ago. She lost 2st 7lb, dropping from 12st to 9st 7lb, a weight she has maintained for over 20 years, enabling her to show off those pins in size 10 shorts.
This month she celebrates 21 years as a consultant, living proof the system can provide sustainable weight loss and a happier life.
When I had my fourth child, the doctor said I was like a different woman, she said.
Since then I have had people referred to my group by my GP.
Claire, from Kidsgrove, was attracted to Slimming World after learning she could still eat pasta and chocolate and lose weight.
She added: I will go through my Take 5 book and make a shopping list based on the recipes in there. I can eat really cheaply. I love things like Slimming World egg and chips. I also love using my slow cooker for things too. It doesnt have to be expensive.
But Claire also makes time for treats known as syns in Slimming World terminology.
She said: Its still important to have a treat. I have my goody drawer, which includes things like marshmallows, dark chocolate, popcorn, choc shots and hot chocolate.
I like to make myself little desserts with quark, frozen berries and choc shot. I can feel like Im having a treat but its a lot healthier.
She also cut down on alcohol, swapping a bottle of wine for water infused with cucumber and making her own ice lollies with sugar-free squash.
Claire said: I always tell my members to buy smaller bottles of wine, and the smaller cans of cocktails. Having a smaller portion is better because you can still finish it but you obviously havent had as much.
I think this is the first Easter I havent bought a bottle of Jack Daniels. I had a G&T that my daughter bought me for Mothers Day and that was it I just wanted a cup of tea after.
I also used to love Diet Coke but now I cant stand it. I really try to stick to water.
Claire also fell in love with the gym after a foot injury left her unable to be as active as she was used to. She said: I just thought, if I dont do anything, Im going to pile the weight back on and feel bad.
So I joined DW Fitness, in Tunstall, to use the pool. I saw a lady with bluetooth headphones and so I ordered some. Im a big music person festivals are my thing so with the headphones I could move about in the water. That got me through the winter.
Claire is also very aware of her mental health and loves to keep a positive mindset in everything she does, praising the Slimming World podcast for keeping her motivated.
She added: Listening to good music and going on walks is important to me - especially during our current lockdown. I also love yoga!
During the coronavirus lockdown Slimming World groups are unable to meet physically but support is still there online, with a half-price weekly fee and free online group sessions.
Claire and other consultants are still looking out for their members: When groups had to close, it felt like Id been punched in the stomach. I didnt know what to do.
But I have been putting on weekly Zoom sessions. They are about 40 minutes long and we treat it like a normal group session. Members get everything they normally would apart from walking through the doors. I even encourage them to bring a piece of fruit!
They can text me their weight and still set themselves a mini target for the next week. I can also give out our weight loss awards and post them out to members and we still do slimmer of the week.
