Testosterone Replacement Therapy or Hormone Replacement Therapy? – Reno Hotline

If you have a confirmed testosterone deficiency, its important to identify if there are any reversible causes that can be addressed before committing to, what should be considered a lifelong therapy. Testosterone Replacement Therapy (TRT) is an involved process. In my professional opinion, the most effective method of replacement is daily Testosterone Cypionate and Human Chorionic Gonadotropin (HCG) injections. Your dose is carefully titrated to normalise your male androgen levels.

TRT is an involved process, its a financial commitment and its a time-consuming process. Two daily subcutaneous injections, admittedly with a tiny 29-gauge insulin needle, preparation of your multi-dose vial, every 3 months for Testosterone Cypionate and monthly for HCG, regular blood tests, additional blood tests after a protocol change. We have patients from all over the UK, Europe and further afield, they all have an initial face to face consultation and yearly thereafter, the rest can be managed remotely. Some of my patients travel thousands of miles, Denmark, Norway, Spain, Portugal, Dubai, the Philippines to ensure they receive Gold Standard care. Londoners often complain a trip to Poole is too far, little do they know.

TRT has traditionally been thought of as simply replacing the testosterone. It makes sense, replace the testosterone that is deficient. The issue lies with the subsequent negative effect on other important parameters administration of exogenous testosterone has on the body. Injecting testosterone shuts down the Hypo-pituitary Gonadal (HPG) axis, you know longer produce Lutenising Hormone (LH) and Follicle Stimulating Hormone (FSH) from the pituitary gland in the brain. LH stimulates the Leydig cells of the testes to produce testosterone, the FSH stimulates the Sertoli cells to produce sperm through a process called spermatogenesis.

HCG mimics LH, it is used in the treatment of male infertility. Intra-testicular testosterone is partly converted to oestradiol by the aromatase enzyme, this helps facilitate spermatogenesis. The Mens Health Clinic now has 20 pregnancies with the concurrent use of HCG alongside testosterone. Its important to appreciate that there are LH receptors all over the body, most noticeably the brain. HCG is clearly important to help maintain fertility and testicular size, but its effects are more wide ranging. Men report an improved sense of well-being and libido using HCG alongside testosterone.

I am uncomfortable with allowing an organ, in this case your testicles, to atrophy with testosterone monotherapy. It seems illogical to me that this should be accepted. I believe that irrespective of whether you want to conceive or not, you should replace this hormone. TRT should be considered hormone replacement therapy (HRT), we should be maintaining function with HCG and supplementing with testosterone to ensure your male androgen levels are normalised.

I am rather shocked and appalled that the medical community has such a simplistic approach to TRT, as one NHS Endocrinologist recently said to me either the patient wants to retain fertility in which case you offer HCG or they do not want to retain fertility in which case you treat with testosterone. This regressive and outdated attitude and approach to TRT is one of the reasons men are willing to travel from all over the world to The Mens Health Clinic, Gold Standard care.

BEYOND LOCAL: Is the keto diet a safe option for losing weight? – TimminsToday

By 2023, an additional 1.7 million Canadians will be obese, according to a 2017 study from the Organization of Economic Co-operation and Development (OECD). The report estimates 25 per cent of the population is currently obese thats about 9.35 million people.

With just three years until the OECD milestone, people are attempting to fight the battle of the waistline bulge, and many are exploring the keto diet as an option to lose weight.

A ketogenic diet is rich in proteins and fats, and it usually includes plenty of meats, eggs, sausages, cheeses, fish, nuts, butter, oils, seeds and fibrous vegetables.

The keto diet forces the body to burn ketones from fat instead of glucose from carbohydrates, and during that transition, a persons body can experience something called the keto flu. As the name suggests, it leads to symptoms like nausea, vomiting, headaches, constipation, diarrhea and irritability, among others.

Tammy Crowley of Welland, Ont., is one of them.

Shes been following the ketogenic diet for two years.

Crowley says she has lost 35 pounds and kept it off. Over the years, shes tried everything from diet programs to more drastic measures.

I dont want to specifically say any one program, Crowley says. Everyone is different, and whatever works for you is great. I just found something that finally worked for me. Ive actually, through the years, Ive had gastric bypass (surgery) for losing weight, and yes, that was great in the beginning, but you put it back on.

Its not an easy fix. Unless you learn properly whats good for you, thats the best way to go, Crowley continues. Im not running any of the other programs down. This just works for me. Ive never felt better. Im going to be 60 soon and I have more energy now that when I was in my 20s.

The effectiveness of the keto diet as part of a long-term lifestyle change is still unknown, but there are medical uses for the regimen. For example, the keto diet is being used to help treat children with epilepsy.

Jennifer Fabe, a registered nutritionist with the Division of Pediatric Neurology at McMaster Childrens Hospital, says the therapeutic ketogenic diet has been used as a treatment for epilepsy for nearly a century.

However, she stresses, it is customized for each patient and only used when people meet certain medical criteria. Fabe says the diet is monitored regularly for effectiveness, tolerance and side effects at home and by a medical team.

Fabe is also the president of Matthews Friends, a charity that specializes in ketogenic dietary therapies for children and adults with drug-resistant epilepsy.

Dr. Andrew Mente, an epidemiologist with the Population Health Research Institute at McMaster University in Hamilton, Ont., says the keto diet can also have benefits for those with Type 2 diabetes.

Conceptually, it is a diet that is high in fat, adequate in protein and very low in carbohydrates, Mente explains. And conceptually, this diet would make sense for people with carbohydrate intolerance, i.e. people with Type 2 diabetes, in controlling their glucose levels and possibly even reversing their diabetes.

There is evidence that when people are assigned to a ketogenic diet if they have Type 2 diabetes, they are able to normalize their glucose levels and possibly reverse diabetes, he adds. In people from general populations, generally healthy populations without diabetes, the evidence is less clear. So, what we would need going forward is more study to investigate both the effectiveness and safety of a ketogenic diet in general populations.

Mente says the ketogenic diet is one approach for Type 2 diabetes, but its not the only diet for people with Type 2 diabetes. Its too soon to say what the long-term impacts of the diet will be on both those with Type 2 diabetes and the general population.

According to Mente, the best idea for those without medical issues requiring a ketogenic diet is to eat a balanced diet.

A nuanced approached is seldomly promoted. Everybody seems to take an extreme position on a variety of matters, and diet is no different, Mente says. For general populations, I would say a nuanced approach is more appropriate going forward until we get the evidence (on the long-term impact of keto) because we want to make recommendations that are evidenced-based.

But what about using the keto diet for weight loss?

Dan Tisi, registered dietitian at St. Josephs Health Care in Hamilton, says there are some pros and cons to losing weight with the keto diet. If theres a medical indication there are some benefits, like epilepsy, then the keto diet can have a positive impact. However, for weight loss, Tisi views it as another tool in the toolbox.

With ketogenics having an emphasis on certain fats and proteins, people need to be aware of the nutrition that may be missing if they are on keto, Tisi says.

A lot of people will have to do supplementation of some kind with higher-quality supplements because there tends to be a B vitamin deficiency. Electrolytes are the most common thing thats missing, usually.

They are also responsible for things like the keto flu that people experience in the beginning, especially the electrolytes part, and magnesium has to be added to that as well. Although, most North Americans are already deficient in magnesium, Tisi explains.

Its best if you are going ahead with keto to do it with the assistance of someone that can really assess your situation and help you determine if this is the tool you should be reaching for.

For someone like Crowley, who has struggled with weight issues for most of her life, she is not willing to easily let go of something that has worked.

She says she is aware that some cardiologists have concerns about ketos emphasis on full fat and high protein, and so does she. Crowleys father died at 51 from a heart attack, and she says that plays on her mind. She says she did a lot of research before starting keto, and Crowley is willing to shoulder the medical risk.

Id rather feel as good as I feel right now and only live another five years than live another 10 years and feel like crap.

REED CITY This is the time of year when folks are striving to keep their New Year resolutions. And among the favorite resolutions are the ones concerning exercise and losing weight.

Debbie Yost, who owns and operates RC Fitness in Reed City along with husband Tony, says there is always an influx of new clients in January and February and shes naturally happy to see that, though not everyone who starts on the path of fitness continues to follow through.

This is a popular time for a lot of people to start exercising and making healthy lifestyle choices, said Debbie, a native of Hart, Michigan who is going into her seventh year keeping the doors of RC Fitness open to the public with her husband.

Some people are starting afresh, she noted, and others are rededicating themselves to a resolution they have made at other times in the past.

What matters is making the decision.

People who start exercising and making better decisions about nutrition and diet never have regrets if they stay with it, she said.

It doesnt just make them feel better mentally and physically; it helps them in so many different ways with things like diabetes and medications. The benefits of fitness are endless.

Debbie and Tony and the staff at RC Fitness have seen many success stories through the years. It all starts with making the decision to say yes to a healthier lifestyle.

Its for people of all ages no matter their present level of fitness, she said.

We do an assessment when you come and see us and help you get started on the right path. You start slowly and build up from there. People are surprised how much just 15 to 30 minutes of cardio three times a week can make them feel better about themselves. The big thing is they have a plan moving forward now and they learn to enjoy it and look forward to coming in.

Debbie said they show the new clients how to operate and properly read the panels for the treadmill and other equipment and also how to gauge their heart rate and learn how to burn calories.

If youre sweating then youre benefiting, she said.

The staff also provides instruction for the weight machines because building muscle is as important to overall fitness as doing cardio. The big thing is striking the proper balance between the two as well as proper technique.

Another critical aspect to fitness that is too often neglected is the nutritional side of it, especially for those who want to lose weight as well as get fit.

Exercise is 20 percent of it and nutrition is 80 percent, Debbie noted.

You can exercise and still not lose any weight if you dont change your unhealthy eating habits at the same time. You cant outrun your fork.

Neuron Found In Mice Could Have Implications For Effective Diet Drugs – Science Times

Have you ever eaten something, got sick and then you did not want to eat that food again because of how it made you feel? That is because a signal from the gut to the brain produced that sickness, thus creating a taste aversion.

The conventional wisdom renders that there is one circuit in our brain that suppresses eating, it comes from the stomach and it makes you feel sick if you activate it too hard. Eating a portioned meal makes your body happy, though, even while you are stimulating a signal your brain to stop eating. This is according to Martin Myers Jr., M.D. Ph.D., the Michigan Diabetes Research Center's director. He said that there could be a circuit that can stop normal feeding without adverse effects.

Now, a Cell Metabolism study may have discovered the second circuit in mice. The director of the Michigan Nutrition Obesity Research Center, Randy Seeley, Ph.D., together with Myers and other researchers sought to better understand which part of the brain curbs the appetite and which the neurons work on making mice want to eat more or not at all.

The gut-brain signal that suppresses the appetite is triggered by a type of neuron, containing CALCR or calcitonin receptor, which lives in a structure of the hindbrain called the medulla. These neurons did not need to be active in the brain for gut sickness to cause an aversive response.

Myers said that this study suggested that they might be able to dissociate the brainstem systems that stop feeding on those that cause nausea, and his group who found that they could genetically activate those CALCR neurons could do that.

Singling out the responsible neuron

Since there are neurons that can suppress eating but can also cause aversive effects, that means that there are different types of neurons, or circuits, in the brain that can terminate feeding with differing emotional responses. When the researchers made adjustments and inactivated the neurons called CALCR, the researchers were taken aback when they made another discovery. It then contradicted the idea that the brain only controls short term meal sizes and consumption.

By turning the neurons off, it did not only interfere with the suppression of feeding by gut signals, but it also caused an ongoing increase in food intake. The mice became obese, thus suggesting that the brainstem systems do not only control meal size, but the amount of food consumed long term. This created a predisposition to obesity because of the energy imbalance in the mice.

Similarly, activating CALCR neurons decreased the food intake of the mice and the bodyweight without any aversive gut effects. In the research, Myers and fellow researchers found another neuron, called the CCK, and it also decreased food intake and body weight however it created an aversive internal response, not like the CALCR neurons. The difference between the two neurons, CCK and CALCR, was found in their circuits.

Myers said that CCK activates what we would call a yucky circuit. The neurons activate a certain cell, the CGRP cells, which then create that sick feeling. Unlike CCK. activated CALCR neurons follow a yummy circuit, thus activating non-CGRP cells.

Potential implications in humans

Obesity and being overweight affects more than one-third of the adult population in developed countries, which can then lead to diabetes or other serious health conditions such as heart disease, diabetes and more.Unfortunately, a lot of diet drugs work, but they make people feel nauseous after they take them Obesity is still one of the conditions that are difficult to manage pharmaceutically.

This is because the treatment options have limited therapeutic utility. A drug that turns on the CALCR and turns of the CGRP could benefit those who are suffering from obesity because it suppresses feeding and it creates a long term control of food intake and body weight.

Myers also said that if they could figure out a drug for people with obesity that can suppress food intake to produce long term weight loss without the negative side effects, it could absolutely change people's lives.

Weight Loss Drug Belviq Tied To ‘Possible Increased Risk Of Cancer,’ FDA says – Science Times

The FDA or the US Food and Drug Administration warns the public that the prescription weight loss medicine called Belviq might be linked to an increased risk of cancer. The findings were the result of a clinical trial that assessed the safety of the drugs Belviq and Belviq RX, or lorcaserin.

The FDA stated that at this time, the cause of the cancer is not certain, and they can't conclude that lorcaserin contributes to the cancer risk. But they wanted to make the public aware of this potential risk. They are continuing to evaluate the clinical trial results and they will communicate their conclusions and recommendations when they have completed their review.

The FDA went on to note that health care professionals should consider if the benefits of taking lorcaserin exceed the potential risks for a patient and that patients that are currently taking the drug should talk to their doctors about the potential increased risk of cancer. The drug manufacturer, Eisai Inc., responded to the announcement of the FDA in a written statement.

The statement said that patient safety is Eisai's priority and they will continue to work closely with the FDA to evaluate the clinical trial results. They recommend patients to talk directly to their health care professionals in order to make the best decision about their medical treatment. As shown in the FDA's communication, at this time, the FDA and Eisai can't conclude that Belviq increases the risk of cancer.

Health risks of taking Belviq

Lorcaserin was approved by the FDA in 2012. It is an oral medication, and it was made available by prescription the year following to help weight loss in adults who are overweight and obese and have weight-related medical problems. Lorcaserin, also available as Belviq, which is taken twice a day, and Belviq XR, which is an extended-release once a day tablet, works by increasing the feelings of fullness so you eat less.

According to the FDA, when approving lorcaserin, they required the drug manufacturer Eisai Inc., to conduct a randomized, double-blind, placebo-controlled clinical trial in order to evaluate the risk of heart-related problems. In the trial, conducted in about 12,000 people over five years, more patients taking lorcaserin were diagnosed with cancer compared to those who took a placebo, which was an inactive treatment. The FDA said that their evaluation of this potential signal is ongoing, and at this time it is not certain if lorcaserin increases the risk of cancer.

Weight loss drugs

The weight-loss drugs that were approved by the FDA for long-term use include orlistat, whose brand name is Xenical, lorcaserin, whose brand name is Belviq, and liraglutide, whose brand name is Saxenda, as well as the combination drugs naltrexone-bupropion, whose brand name is Contrave and phentermine-topiramate, whose brand name is Qsymia.

In a recent study, these drugs helped overweight or obese people lose at least 5% of their body weight at the end of a year, and that is at least 10 pounds, compared with a placebo. Saxenda and Qsymia were linked to the highest odds of getting that amount of weight loss. For a lot of people, losing 5% to 10% of your body weight is linked to improved blood pressure, triglycerides and blood sugar, these factors are known to lower the risk for heart disease and diabetes.

I had to get this fat to be grateful for what Ive discovered. At 43, and 18 stone, Ive gained freedom: from crash diets; from constant panic the weight might return; from worry my life will end if Im not the right weight; from constant thoughts of forbidden foods, until the inevitable binge, and guilt that would follow.

I also see my world hasnt ended. My partner still loves me. Im still a decent radio presenter, working at talkRADIO, and my dogs still wag their tails when I arrive home.

It started in my childhood. A misdiagnosis of a heart problem meant I was put on my first diet aged eight, which meant constant calorie counting and supervision of all food. I rebelled by secretly eating cakes and chocolates, and by 16 I was 17 stone with a relationship with food based on secrecy, guilt, and anxiety.

Ruthlessly strict calorie counting

I first lost weight in my twenties, through ruthlessly strict calorie counting, training, and being militant with both my regime and diet. But the more I deprived myself of the food I wanted, the more Id think about it until eventually Id give in. Id convince myself that I could have a chocolate bar as a treat. But once Id eaten that one, Id end up having another. And another.

Inevitably, my weight crept up. Just half a stone at first. Then Id panic and go back on a strict diet of misery and deprivation, and that half a stone would come off. This cycle of starving myself versus stuffing myself would continue in my thirties until it turned from half a stone repeatedly gained and lost, to a stone, two stone, and before I knew it, five or six stone at a time.

I went to the doctor about my yo-yo dieting, but in my experience the NHS arent interested until you need a gastric band. And along with every diet I did came a punishing workout/gym regime, either on my own, or, when I started doing well, with personal trainers. Ive had about 10 PTs over the years and spent thousands, all to end up back at square one.

Meanwhile, the diets I was choosing became more and more extreme. Id use low calorie, low carb, Atkins, Dukan, food combining, fasting, detoxes, shakes, soups and simple starvation. Despite all this I would always end up bingeing, and always, ultimately, end up gaining more back.

Plagued with thoughts of banned food

My most recent four-stone loss came in 2016 from a shake- and soup-based ketogenic diet. I spent months barely eating real food, and when I did, it was just protein. I looked great, but I was constantly fearful that Id gain weight and was plagued with thoughts of the foods Id banned myself from eating, until one day, after a year of deprivation, I gave in.

I ate one of the blueberry muffins Id walked past in the office canteen, and resisted, countless times before. Biting into that delicious sugary sponge brought back every emotion around food Id been suppressing, guilt, anger at myself for ruining it all and a promise to be stricter tomorrow.

Within months, Id gained back the four stone, and more, to put my weight at the highest its ever been.

Weary of it all and in my worst ever mental state, I finally chose to spend money on my brain, rather than my belly, starting cognitive behavioural therapy to get to the root cause of why, when Id done so much to lose weight, I was once again so fat? Had I not had the money for my own CBT, Id still be in the mess I was in.

CBT revealed that crash diets were the problem

One of the main lessons of CBT is to challenge all-or-nothing thinking, and encourage rational, instead of negative pre- programmed thoughts. I finally realised that far from helping me, every single restrictive crash diet was actually making me eat more.

The moment Id stray, the immediate negative narrative and subsequent resolution to be even more restrictive the next day meant I was actually giving myself the green light to eat more. I now challenge that narrative that I am worthless when I am bigger and that my life should stop when I am. If I hadnt got to this weight, how would I have discovered this?

Its not always easy. I sometimes have to use all my CBT knowledge to ignore the latest diet trend, and remind myself, its not a solution.

And something else has changed, too. By knowing I can have that muffin and the world wont end, I dont want it as much. For the first time, I have a biscuit jar, which actually has biscuits in as I dont eat every single one. As a result of ditching fads, my weight, although high, has remained constant for two years. Ive spent 30 years either gaining or losing and now, for the first time ever, I am a steady, albeit high, weight.

There's a place for treats in a healthy diet

Whats also encouraging is that there are now successful fitness professionals employing the non-restrictive philosophy, such as James Smith, whose best-selling Not a Diet Book challenges the diet industry.

Why would we have a life without the foods we enjoy? he tells me. I dont think we should make a case for consuming treats, but make one against those who say you never should. I want people to reduce their food intake slowly and have the muffin if they want it. I tell my clients, one hot day doesnt make a summer. A treat or two wont ruin your long-term plan, its part of human nature. Its normal. And if they have one, just do a few extra steps the next day.

So now Im not on anyones strict plan, Im eating well, but having that muffin if I want it. I am reading about sustainable long-term methods which arent too strict. I now get it. I am vlogging and Instagramming it as I want others to know they can do it too. I hope I succeed, as I want to be healthy and feel better. But if I dont, Ive had success already. Because I know I will have days when I am not perfect. But those days wont mean Ive failed.

Slow and steady wins the Prius race | Transportation – The Union Leader

Do Prius owners really think they need to set the (slow) pace for everyone else?

Very often, at the head of a line of slow-moving vehicles is a Prius, or one will pull out in front of another car and slow down every car behind it.

Is it because the car cannot accelerate, or is it because the owners of that particular car feel the need to control others?

I don't think it's either one, Susan.

We know a famous researcher named Dr. David Giovannoni. We call him Dr. Joe Baloney. He once explained to us that humans (and organizations) achieve what they measure.

So, for instance, if you want to lose weight, what would you do? Well, you might cut out sweets, drink water instead of soda and maybe start to exercise (Note: we're talking about you, not me).

Then you'd get on a scale. And if the number on the scale was lower than it was the week before, you'd keep doing those things, because you'd know they're working.

You achieve what you measure.

Now, who buys a Toyota Prius? Someone who wants to get better gas mileage than the rest of us mortals. And guess what a big screen on the dashboard of the Prius does? It measures mileage. Constantly. By watching the numbers change on that screen, Prius drivers learn very quickly that by accelerating very gently, they can coax their mileage up from 53.8 mpg to 54.1 mpg.

That makes them feel good. So they learn to feather that gas pedal and take 25 seconds to get from 0-60 while you're cursing at them through your windshield.

It's not that the Prius can't accelerate. It will accelerate like pretty much any other car if you put your foot down.

And it's not that they feel a need to be controlling. They're just pursuing their personal goal of maximizing their mileage -- because they care about it, and they're constantly measuring it.

Likewise, if you measured your blood pressure, Susan, you'd probably find that it goes up when you're stuck behind a Prius.

If you took a few deep breaths and said to yourself, "How nice, there's a person who cares about the planet and is making the world a better place by wasting less fuel," you'd see your blood pressure go down.

Then you'd throw the blood pressure monitor out the window and pass the Prius with a few choice hand gestures.
