Four strategies for keeping weight off once it’s gone – The Reflector

Losing weight requires hard work and determination. Oftentimes, men and women hoping to lose weight must commit to healthier lifestyles than the ones theyve grown accustomed to. Abstract: Antipsychotic-induced weight gain is a major management problem for clinicians. It has been shown that weight gain and obesity lead to increased cardiovascular and cerebrovascular morbidity and mortality, reduced quality of life and poor drug compliance. This narrative review discusses the propensity of various antipsychotics to cause weight gain, the pharmacologic and nonpharmacologic interventions available to counteract this effect and its impact on adherence. Most antipsychotics cause weight gain. The risk appears to be highest with olanzapine and clozapine. Weight increases rapidly in the initial period after starting antipsychotics. Patients continue to gain weight in the long term. Children appear to be particularly vulnerable to antipsychotic-induced weight gain. Tailoring antipsychotics according to the needs of the individual and close monitoring of weight and other metabolic parameters are the best preventive strategies at the outset. Switching to an agent with lesser tendency to cause weight gain is an option, but carries the risk of relapse of the illness. Nonpharmacologic interventions of dietary counseling, exercise programs and cognitive and behavioral strategies appear to be equally effective in individual and group therapy formats. Both nonpharmacologic prevention and intervention strategies have shown modest effects on weight. Multiple compounds have been investigated as add-on medications to cause weight loss. Metformin has the best evidence in this respect. Burden of side effects needs to be considered when prescribing weight loss medications. There is no strong evidence to recommend routine prescription of add-on medication for weight reduction. Heterogeneity of study methodologies and other confounders such as lifestyle, genetic and illness factors make interpretation of data difficult.

In Sickness and in Health: it’s time for Nick to get back in the water – iNews

One of my proudest achievements as an adult used to be the fact that Id learnt how to do proper, face-in-water swimming. Or how to swim, as most other people would see it. Until my late twenties, I would do the kind of breaststroke favoured by ladies of a certain age who didnt want to ruin their hairdos. Front crawl was off the agenda, because I didnt like putting my face underwater, and back crawl was too alarming because if I splashed too much, water might find its way up my nose.

Its thanks to Nick that I finally got my head around swimming. He used to be the captain of his schools swimming team and was brilliant in the water. He patiently bobbed about with me in the shallow end of our local baths and taught me how to exhale while submerged. Over the following months, I mastered a few strokes and started lane swimming. I even bought myself a swimming cap so that my fringe didnt impair my front crawling.

As I became fitter (and fed up with some peoples pool etiquette), I started going to the gym, walking and running, and when our local pool shut for refurbishment, I forgot about it. Years passed, and then Nick was run over. Since then, my proudest achievements have been bound up with looking after him; learning to live alone, getting back on the road after years of not driving; doing battle with the care system and the like. Even low-grade athletic prowess has been beyond me, despite wanting to get more active for my own sake as well as to be strong enough to look after Nick.

Then, a few weeks ago, not knowing what to do with myself after work and, for once, not wanting to drink three pints of oblivion in the pub, I remembered the pool round the corner. It was 8pm would it be open? Yes it was, and so a little later I found myself back in the water.

Before I took the plunge, though, it was time for my evening call to Nick. Standing in the echoing corridor by the changing room, I told him I was about to go for a dip.

I miss swimming, he said, sadly.

I used to be really good at it.

Nick hasnt been in a pool since the accident. At first, he was too mad and ill. Then the pool at the hospital was out of service. Then his incontinence was the issue. Then, once he moved to a care home, the nearest pool was being refurbished. Then he moved again, and was told he needed to lose weight before they could safely get him doing hydrotherapy.

Hes managed to lose a stone and a half since Christmas, and I am going to start nagging the home about getting him into a pool. Because since I took to the water recently, Ive been returning often. I mentioned it to a friend who had a baby a few months back. A blissed-out look came across her face. I managed to have a swim the other day, she said. It felt wonderful to feel my body again.

I cant stop thinking about how wonderful it would be for Nick to feel his body doing something different from lying in bed or sitting in his wheelchair.

I asked him this morning how he thought it would feel.

If I could go in the water, I could float and feel free. Ill feel freer than ever.

Usually, the burden falls on family members to recognize the signs that an aging parent might need some extra help with daily living tasks, but this doesnt necessarily mean that your loved one has to move into a care home. They may just need some extra help in their home.

Here are some questions to ask to determine if your elderly parents may need some extra care?

Are your parents able to take care of themselves? Pay attention to your parents appearance. Failure to keep up with daily routines such as bathing, tooth brushing, and good hygiene is a sign that they may be experiencing feelings of depression or have a physical ailment.

Are your parents able to take care of their home? Are the lights working or is the laundry piling up? Is the garden overgrown? Any changes in the way your parents do things around the house could provide clues to their health.

Are your parents safe on the road? Driving can be challenging for older adults. If your parents become confused while driving or youre concerned about their ability to drive safely, it might be time to stop driving.

Have your parents lost weight? Losing weight without trying could be a sign that somethings wrong. Weight loss could be related to many factors, including difficulty cooking, loss of taste or smell or perhaps it is the signal of an underlying condition such as malnutrition, dementia, or depression.

Are your parents still social? Its important to talk to your parents about their activities. Are they connecting with friends? Have they maintained interest in hobbies and other daily activities? Are they involved in organisations or clubs? If a parent gives up on being with others, it could be a sign of a problem.

Even if you dont live near-by to your parents but are concerned about their health and wellbeing, there are many things you can do to ensure your parents are getting the support they need. Share your concerns with them and address any issues that you feel may become bigger if they arent dealt with early on. Encourage regular medical check-ups, consider home care services, and seek help from local agencies.

I run five miles every day, Id tell myself. But it wasnt until I had my second son a year ago that it hit me: I was over-indulging, and it wasnt good for my health or my weight.

When I had my first son, I looked the same way as I did pre-pregnancy within hours after he was born (annoying, I know). But with my second son, I still looked five months pregnant after he vacated the premises. I reassured myself that this was super normal (which it is), but a few weeks later kept wondering why the weight was taking so long to drop off. I started running again two weeks after I gave birth and figured Id lose the weight then, but it still was coming off at a super slow pace.

Finally, it hit me: To lose the weight, I had to pay attention to my diet, too, and cut out all the crap I had been having. It was so obvious, but for some reason I had been kidding myself about what I was really eating. It was ridiculous for me to sit around wondering why I wasnt losing the baby weight while still having a chocolate bar after lunch each day and ice cream at night. And it was more than a weight thingI knew that all of that overindulgence had the potential to be bad for my health, too.

So, I got real with myself. I was mostly eating healthy but completely blowing it with treats every day. And unfortunately, Im not alone. Its a common issue to overestimate good eating habits and underestimate bad eating habits, says registered dietitian Sonya Angelone, a spokeswoman for the Academy of Nutrition and Dietetics. And those bad habits can add up over time, she points out.

Somehow I had convinced myself that my regular treats didnt really count because I run every day, and thats also a problem, says registered dietitian-nutritionist Karen Ansel, coauthor of The Calendar Diet: A Month by Month Guide to Losing Weight. Exercising is one of the most important things you can do to stay healthy, she says. However, the idea that exercising will help women lose a large amount of weight is a myth. Finding a sweet spot between balancing fitness and how much you eat is key to getting and keeping a healthy weight, she says.

Im not into the idea of dieting (Id rather just eat healthyfor real), and I knew that cutting out those extra sweets and rethinking my eating habits would make a difference. So, I did.

I noticed that I tend to grab candy or ice cream when I'm stressed, so I made a point to stop mindlessly eating while Im working. Now, if I have to eat while I work, I make sure its an actual well-rounded meal or piece of fruitnot a bag of chocolate chips. I also cut out those late-night ice cream benders, started eating more slowly, and paid more attention to how much I eat at meals.

Ice cream is a huge weakness for me, and its usually best if I steer clear of it. But Im also working on having a bite or two of it if I really want it. Ive learned that I can feel just as satisfied by eating it that way as if I had a bunch of ice cream at onceand thats worked out really well for me. Weirdly, the less Ive eaten, the less I crave it.

Not surprisingly, the baby weight melted off after I made those changes. I even lost an extra seven pounds that Ive been carrying around for years that I'm pretty sure is due to all those treats I've been eating.

Im not super hung up on the number on the scale, but I like the way my body looks and feels now. Im also happy with my eating habits and the fact that Im actually being honest with myself about my bad habits. Its taken three decades and some brutal truth, but I think Ive finally figured it out.

When she moved in with her husband in her early 20s, Destiny said she stopped caring about her weight.

The woman from Sarasota, Florida added: "I was very comfortable being overweight.

"But it didn't completely spiral out of control until I got married, and unfortunately my husband and I both share the same love for food. I am an emotional eater and I'm also a binge eater, and at the time I had no self-control.

"In 2014 I reached 263 pounds, my heaviest weight at the time, and at that point I did start working out and losing weight. I lost 53 pounds at first, but then I plateaued and lost motivation. I didn't want to keep working out. I wanted to eat what everyone else ate.

"So I quickly gained my weight back and then some, and in February 2016 I was diagnosed with polycystic ovarian syndrome (PCOS) and endometriosis. And then in March 2016 my little brother passed away from Duchenne muscular dystrophy, and I started struggling badly with depression."

"I didn't have a nutritionist or trainer, I just went to the gym and started on the elliptical for the first couple of weeks for just an hour a day. Then, as silly and pathetic as I probably looked, I started incorporating weights, and I started eating cleaner and limiting my portions.

"I did some research on how to lose weight, but I didn't have any tricks up my sleeve. There were no diet pills, no wraps, no shakes and no surgery.

"I go to bed when I start getting hungry, and I wake up at 4:30 am to go to the gym. I even bring my own snacks everywhere I go to avoid temptation.

The hardest part for Destiny has been watching her family and friends continue to eat junk food and not be able to eat any of it bit its been worth it. As well as the weight loss, Destiny has gone from a size 26 to a size 16.

Destiny added: "I am now able to do things I've never done. I am going skydiving in a week, and I've already signed up for my first 5k. I even plan on going zip lining soon!

"Being overweight really limited the activities I could participate in. Now I feel so confident. No one and nothing can get in my way now. I'm unstoppable."
