Study shows dramatic shift in gut microbes, their metabolites after weight loss surgery – Arizona State University

Obesity, already a global epidemic, is on the rise. Over one third of the U.S. population is currently afflicted, according to the Centers for Disease Control and the monetary costs alone are approaching $150 billion dollars annually. Causes of the epidemic include changing diets and greater sedentism, though environmental factors may also contribute.
A new study compares the two most common surgical therapies for obesity, known as Roux-en-Y gastric bypass (RYGB), and laparoscopic adjustable gastric banding (LAGB). The results demonstrate that RYGBthe more aggressive of the two surgeries produces profound changes in the composition of microbial communities in the gut, with the resulting gut flora distinct from both obese and normal weigh patients. The results are likely due to the dramatic reorganization of the gut caused by RYGB surgery, which increases microbial diversity. The new research paves the way for new diagnostics and therapies for obesity. Microbial diversity in the human gut: The four images indicate the degree of microbial diversity in the gut in normal patients as well as in obese patients before and after undergoing two types of weight-loss surgery. The normal human gut has a high degree of microbial diversity, considered important for the maintenance of health. Obese patients have lost much of this diversity and while laparascopic band surgery effectively leads to weight loss, the low microbial diversity condition remains. By contrast, gastric bypass surgery results in the restoration of microbial diversity in the gut, though the composition of microbes is distinct from both normal weight and obese patients. The gamut of adverse health effects associated with obesity is broad, including such devastating illnesses as type 2 diabetes, coronary artery disease, stroke and certain forms of cancer. Patients often suffer loss of mobility, social isolation and inability to work. Currently bariatric surgery is the most effective treatment for morbid obesity, in terms of significant and sustained weight loss.
In the new study, appearing in the current issue of the Nature Publishing Group journal International Society for Microbial Ecology (ISME), Zehra Esra Ilhan, Rosa Krajmalnik Brown and their colleagues at the Biodesign Institute at Arizona State Univesity, along with researchers from Mayo Clinic, and Pacific Northwest National Laboratory, explore microbial communities in the human gut following RYBG and LAGB surgeries.
The results confirmed theirearlier researchwith a smaller sample size, showing that in the case of the more aggressive and irreversible RYGB surgery, microbial communities underwent a profound and permanent shift following weight loss. The resulting post-surgical composition of gut microbes observed for RYGB patients was distinct from both normal weight and obese patients, and displayed the high microbial diversity associated with a healthy gut.
The current study also applied the technique of nuclear magnetic resonance (NMR) to examine the metabolome a composite of the metabolites produced by the various microbes in the gut, again noting significant alterations as a result of the RYGB procedure. In the case of the alternate treatment, LAGB, changes in the gut microbiota were mild and accompanying weight loss was less pronounced.
This is one of the first studies to show that anatomically different surgeries with different success rates have different microbiome and microbiome-related outcomes, noted Ilhan, lead author of the new paper. Further, the results indicate that correction of obesity tends to improve related metabolic conditions, including diabetes and high cholesterol.
One of the key findings of the paper confirms what we had already observed inearlier research. RYGP gastric bypass had a huge effect on the microbial community structure, Krajmalnik-Brown said. This fact may have profound implications for both the understanding and management of obesity.
The millions of bacterial microbes in the human gut perform a vast range of critical functions in the body and have even been implicated in mood and behavior. Among their critical responsibilities are the micro-management of nutrients in the food we digest, hence their central place in the regulation of body weight.
A tell-tale indicator of pathology in obese patients has been found in the gut, where a markedly lower diversity of microbial communities is observed. As Krajmalnik-Brown explains, diversity of gut microbes is essential to good health. Diversity is good because of what we call functional redundancy, she said. If you have 10 workers that can do the same job, when one of them gets sick, the job still gets done.
Low microbial diversity in the gut, by contrast, is associated not only with obesity but a range of ailments including inflammatory bowel disease, ulcerative colitis and autism. (Earlier research by Krajmalnik-Brown and her colleagues demonstrated diminished diversity in thegut microbiome of autistic childrenand in a more recent study, improvement in the symptoms of autism was demonstrated followingtransplantation of beneficial microbes.)
Competition in diverse microbial networks in the gut helps provide a system of checks and balances. Should diversity fall, a delicate democracy can be shattered and tyranny may prevail, as populations of microbes like Salmonella or Clostridium difficile usually subsisting at low levels in the gut expand and take over.
The study sought to explore long term changes in the gut in patients who had undergone either of the two surgeries at least 9 months prior, comparing them with normal weight and pre-bariatric obese patients. While the reasons for the sharp disparity of results between RYGB and gastric banding are not entirely clear, the results indicate that simply reducing the size of the stomach through gastric banding is not sufficient to induce the large changes in microbial communities observed for the RYGB group.
One hypothesis the authors put forward is that RYGB alters the physiology of the gut to such a degree that microbes formerly unable to survive conditions in the obese gut are able to flourish in their surgically-modified surroundings.
One of the things we observe from the literature is that the oral microbiome community composition is very similar to the colon microbiome composition after bariatric surgery, Ilhan said. Most of the species are acid sensitive, which supports the idea that changes in stomach pH levels may permit these microbes to survive and make it to the colon.
According to John DiBaise, a gastroenterologist at Mayo Clinic, Scottsdale and co-author of the new study, These new data on microbial community structure and function significantly expand our knowledge on how the microbiome is associated with weight loss following bariatric surgery.
While it seems clear that RYGB surgery produced permanent changes in bacterial communities in the gut, the resulting microbial community may also act to help maintain weight loss over the long term. Experiments have shown that transplantation of beneficial microbes from mice that have undergone RYGB surgery into obese mice induces dramatic weight loss. While these results have yet to be replicated in humans, the findings open the door to the eventual use of healthy microbial communities to treat obesity.
Although the RYGB surgery has been quite successful for many patients suffering from morbid obesity, it is a serious, invasive procedure that is not without risks. Further, some patients are not successful and regain the weight they have lost post-surgery, perhaps because they lack the favorable microbes necessary for permanent weight loss.
As Ilhan said, a probiotic that would replace surgery would be great. Another positive outcome would be if we can find a microbial biomarker that will identify the best candidates for surgery and sustained weight loss.

‘You feel shocking’: Star young jockey Dylan Dunn tells why he had to quit – The New Daily


Dylan Dunns efforts in 2016 all pointed to him being a star of the future.
The young jockey won Abu Dhabis Apprentice World Championship Final, was crowned Melbournes best apprentice and even won on a horse owned by Her Majesty Queen Elizabeth II her first Australian winner.
But just five months into 2017, the 22-year-old has announced his retirement, citing a never-ending battle with his weight.
At 178cm, Dunn the son of famous jockey Dwayne is far taller than most jockeys and, as a result, he found it extremely difficult to weigh as little as 55kg for some of his rides.
He said that constant wasting [a term used in the racing industry to describe losing weight quickly] was the reason he contracted pneumonia on his left lung in 2015 and glandular fever earlier this year a diagnosis that led to his retirement.
There were days I just didnt want to get out of bed because I knew it was going to be a shocking day of wasting, Dunn told The New Daily.
The wasting brought on the sickness and the glandular fever its why Im giving it up.
I travelled around Australia watching dad, and as a kid I always rode horses. It was my dream [to be a jockey].
Its come to an end a lot sooner than I expected but I probably always knew it was coming because of my size.
Dunn was initially turned down by Racing Victorias apprentice academy because of concerns about how he would manage his weight a decision that killed him at the time.
Determined to make a career of it, he moved to South Australia, got his licence, and, before long, started to rack up winners, leading to a return to Victoria.
Dunns life was far from healthy, though, something he now admits in hindsight.
Theres been weeks where Ive lost 5kg from Monday to Saturday through limiting food, fluid, sweating, he said.
I might have had some lollies every now and then, because I felt like I was going to pass out its bad a lot of jockeys are anorexic and underweight.
Sweating in saunas, baths, cars, running in full-length wetsuits Ive done it all.
When driving to the races, Id put on a plastic sauna suit, ski pants, two skivvies, jackets and then wind the windows up and crank the heat up. I could lose one to two kilos doing that.
Theres plenty of days youll be sitting in the car, 40 degrees (Celsius) outside and its so hot in the car. Youre sweating and you feel shocking. About an hour is all I could deal with.
Theres people who resort to throwing up. I was lucky enough not to do that. Thats another reason why I was willing to walk away.
Ive seen what it does to people, who just look sick, and long term, its really not good.
Dunn, who said he basically completely blacked out after a race at Caulfield after having to lose four kilograms in one day, also says too much is demanded of many jockeys, citing regular days that begin with track work at 4am and often dont end until 8pm after a day at the races.
But despite all of this, Dunn who says he would consider making a return if weight limits were lifted is far from bitter.
He is an impressive young man who still loves racing, says riding in races against his father was very cool, had 133 career winners and can say Queen Elizabeth II knows who he is.
Dunn, who rode her Bold Sniper to victory at Sandown at 19-1 last May, said: She loves her horse racing and watched the race.
She wanted to speak to me on the phone but she got [trainer] David Hayes and they spoke about me thats pretty cool.
Dunn will now turn his attention to his mortgage-broking business, Dunn Financial Services, and is expected to work in the racing media.

Under a new version of the citys proposal, diet drinks would not be taxed, in addition to other changes. A final vote by the full council likely will be scheduled for Monday.
Seattle City Council members moved a proposed soda tax Wednesday toward a final vote and amended it to exclude diet drinks.
Mayor Ed Murray sent legislation to the council in April calling for a tax of 1.75 cents per ounce on distributors of sweetened beverages, including sugary and diet soft drinks, among others.
Under the new version of the proposed tax, diet drinks would not be taxed, small manufacturers would be exempted and medium-sized manufacturers would pay a reduced rate.
The amended plan would raise about $8 million less annually than Murrays plan, according to city estimates.
The councils finance committee made the changes Wednesday before voting 5-2 to move the ordinance forward.
For me, the main object of this legislation is to reduce the consumption of this unhealthy product, Councilmember Mike OBrien said.
Councilmembers Lisa Herbold and Kshama Sawant cast the dissenting votes, while Councilmembers Tim Burgess and M. Lorena Gonzlez were behind the major changes.
Herbold proposed lowering the tax rate to 1 cent per ounce and keeping diet drinks in the tax, but her amendments were rejected by the committee.
Councilmember Rob Johnson proposed taxing syrups and powders based on their dry weight rather than their weight once mixed into drinks, saying that would be simpler. His amendment also was rejected.
Herbold and Sawant said they couldnt support a tax that would disproportionately burden low-income people and people of color. Herbold called the existing proposal unfairly regressive and warned a rate set too high could expose the tax to repeal via citizen ballot measure.
A final vote by the full council likely will be scheduled for Monday.
Many health advocates are supporting the tax, while soft-drink companies, some business owners and unions with workers in the beverage industry oppose it.
The Martin Luther King County Labor Council and the Seattle Metropolitan Chamber of Commerce issued a joint statement Wednesday against the tax, saying it would harm the local economy.
Revenue from the tax would be used by the city to support healthful eating and education.
Seattle leaders have been having trouble agreeing whether to tax diet drinks.
Diet drinks werent included in Murrays initial plan. The science on sugary drinks being unhealthful is solid, but evidence of artificially sweetened drinks being harmful is less clear.
The mayor added diet drinks to his plan after a racial-equity analysis showed their popularity among wealthy people and white people. And now council members have taken them back out.
In pushing for diet drinks to be excluded from the tax, Burgess pointed to unclear health findings on artificial sweeteners and to concerns about the tax leading to job losses.
Leaving diet drinks untaxed gives consumers an alternative, he said. And Im concerned about what weve heard from labor.
Taxing sugary and diet drinks at 1.75 cents per ounce, as the mayor proposed, would initially raise about $23 million annually, while taxing only sugary drinks at that rate would raise about $15 million, according to estimates Herbold obtained from the citys budget office.
Herbold argued for taxing sugary and diet drinks at 1 cent per ounce in order to raise 15.5 million annually.

Theo Riddick, recovering from wrist injury, cut junk food from his diet … – ESPN (blog)


ALLEN PARK, Mich. -- With protection over his surgically-repaired right wrist, and off away from the rest of his position group, Theo Riddick started to run. Then, the Detroit Lions running back began to cut.
He was doing it without a helmet and he was doing it without cones or any sort of drill expectation, but there seemed to be progress during Wednesday's OTA. Riddick was moving. He looks like he hasn't lost any of his speed or his elusiveness after a 2016 season marked with injuries -- including wrist injuries that ended his season and required surgery.
"I'm getting a lot of mental reps right now," Riddick said. "And I'm just trying to sharpen my game in terms of reading defenses, seeing stunts, fronts and all things around that nature."
When he's healthy, Riddick is one of the more valuable players in the Lions offense. He is one of the more dynamic pass-catching backs in the NFL with 133 receptions for 1,068 yards over the past two seasons. That's almost equal to his rushing totals of 135 carries for 490 yards over the same span.
It's tough to gauge much from the minimal work Riddick did on the side, but entering his fifth season he looks a bit more built than in years past. When he was asked about that, he fired back, jokingly, "You trying to say I'm fat? Nah, I'm good."
Actually, Riddick said he's been eating better. He said he cut junk food from his diet -- although he wouldn't say exactly what -- but that "it's helping out."
Helping out is what he does best with the Lions. He's the change-of-pace back Detroit has been looking for and he showed some inconsistent flashes of being an improved runner last season, averaging 3.88 yards per carry. But he's best-used as a 2-minute back who can make players miss with good cuts and reliable hands.
The 26-year-old is expected to continue in that role as a change-of-pace back for starter Ameer Abdullah. Lions coach Jim Caldwell once again said Abdullah is the team's starting running back and he took the majority of first-team reps during Wednesday's open OTA.
Of course, Riddick wasn't participating. He was vague about his potential participation in the future. He wouldn't confirm either surgery and shrugged off any questions about his health -- changing the focus to mental reps and the playbook.
When asked if he would be ready for training camp, Riddick was also non-committal.
"Guess you'll see," Riddick said, smiling.
Until then, all the Lions -- and their fans -- can do is watch him work out on the side and wait.

In particular, a new small-scaled study from Stanford University suggests, fitness trackers are not great at measuring calories burned.
A team of researchers tested the accuracy of seven wrist devicesApple Watch, Fitbit Surge, Basis Peak, Microsoft Band, PulseOn, Samsung Gear S2 and MIP Alpha 2on a pool of 60 volunteers (29 men and 31 women), who were told to walk, run and cycle on treadmills and stationary cycles in the lab. Their goal was to see how accurate heart rate and energy expenditure (aka calories burned) measurements were for each device.
The heart rate measurements performed far better than we expected, said Euan Ashley, an associate professor at Stanford University Medical Center and co-author of the study, but the energy expenditure measures were way off the mark. The magnitude of just how bad they were surprised me.
The researchers gathered heart rate data from the wearables and compared it to results from an electrocardiograph. The analysis found that six of the seven devices were relatively on point, boasting an error rate under 5 percent. Only the Samsung Gear S2 had a higher error rate of 6.8 percent, while the most accurate device was the Apple Watch.

But when it came to calories lostwhich means tracking energy used during a workoutthe results were quite different. The researchers compared data from the tracker to that from an instrument that measures oxygen and carbon dioxide in breath to estimate metabolic rate. Of the five devices that offered this additional feature, the range of error was anywhere from 20 percent to 93 percent.
According to the scientists, the devices should ideally have an error rate lower than 10 percent in non-medical settings. None of the tested devices came close. Even the most accurate device, the Apple Watch, was off by an average of 27 percent. The biggest offender was the PulseOn.
The study found specific groups of people to receive a higher rate of error than others, reports NPR. For instance, darker skinned toned people, those with higher BMIs and men had higher error rates than Caucasian women with a healthy weight. The research team doesnt know why this pattern was seen, but for consumers, this means non-Caucasian individuals, men, and those with larger BMIs should be skeptical of their readings.
Additionally, data for cycling was the most accurate and data for walking was the least accurate.
The study, published in the Journal of Personalized Medicine, has an important takeaway for users of fitness trackersand its not to eliminate them entirely.
According to Ashley, the findings spotlight the importance of understanding that fitness trackers give rough estimates when it comes to calories burned. Part of the problem is that body types and metabolism are so different from person to persona two-mile jog for two people can result in completely different energy expenditure based on factors like gender, age, metabolic rate, height and weight.
The findings are particularly valuable information if you actively use your fitness tracker to monitor caloric intake and base your days diet off of the data. So just because your fitness tracker says you burned 900 calories doesnt mean you can afford to eat an entire pizza. And if youre debating between one of the aforementioned seven devices, the Apple Watch appears to be the way to go.
