In an effort to reduce and prevent the incidence of heart disease, researchers in Europe have developed a new tool that designs personalized workouts for patients at risk or living with the condition.
Called the European Association of Preventive Cardiology Exercise Prescription in Everyday Practice and Rehabilitative Training (EXPERT) tool, the program creates exercise routines with varying intensities and frequencies based on the severity of a patients risk factors. Research on the development of the program was published April 19 in the European Journal of Preventive Cardiology.
"Exercise reduces cardiovascular risk, improves body composition and physical fitness, and lowers mortality and morbidity," said Dominique Hansen, the lead author on the study and an associate professor in exercise physiology and rehabilitation of internal diseases at Hasselt University in Diepenbeek, Belgium, in a statement. "But surveys have shown that many clinicians experience great difficulties in prescribing specific exercise programs for patients with multiple cardiovascular diseases and risk factors."
Physicians can input patients cardiovascular risk factors, diseases and other chronic conditions into the program and it will automatically design a personalized exercise program for the patients. The program also takes into account best practices for patients who are overweight, or are struggling with diabetes and hypertension.
"EXPERT generates an exercise prescription and safety precautions since certain patients are not allowed to do certain exercises, Hansen said. For example, a diabetic patient with retinopathy should not do high-intensity exercise. This tool is the first of its kind. It integrates all the international recommendations on exercise to calculate the optimum training program for an individual patient. It really is personalized medicine."
Looking ahead, Hansen wants to launch clinical trials that test the programs effect on patient outcomes.

There are no treadmills humming. There are no weights clanking to the ground. There are no medicine balls smacking against the walls.
The only sound in the room is the smooth swish of water coming from a line of rowing machines and music blaring from the speakers.
Five minutes into the workout, 11 rowers were breathing heavily.
As exercises ramped up in intensity, their instructor offered words of encouragement: Dont give in. Remember, were all on the same boat.
By the end of the 30-minute class at First Row Fitness, participants were covered in sweat. They groaned as they clambered off the machines, still trying to find their footing on wobbly legs.
Rowing isnt new, but its slowly making a splash in the local fitness scene.
The exercise machines have long been left in the dust at big box gyms, surpassed in popularity by treadmills, the weight room and group exercise classes.
But in recent years, rowing machines have been reintroduced through fitness trends like CrossFit and Orangetheory the latter offers circuit-style fitness classes.
And within the last year, at least two local studios have debuted classes built around the rowing machine.
The exercise, which looks as if users are rowing a boat, is low-impact, increases the heart rate and works several muscle groups.
It was the right format for Carrie McDonalds Council Bluffs facility, First Row Fitness. McDonald, a longtime fitness instructor, wanted a workout for all fitness levels.
First Row Fitness, which opened at 623 W. Broadway in June, offers kettle bell and yoga classes, too. But rowing is the focus.
Rowing classes, offered three days a week, are always full with 11 participants. Attendees are a mix of men and women. The oldest client is 78.
I usually have a wait list on those classes, which is a good problem to have, McDonald said.
Over the past two years, rowing studios have popped up regularly in cities like Chicago and New York.
After visiting a New York studio, Trevor Fleming was inspired to open his own in Omaha. Dynamic Rowing, which opened a year ago at 14505 Grover St., offers seven rowing classes a week.
Like the Council Bluffs studio, Fleming uses water rowers at his facility. A water rower doesnt jerk or jolt the joints, he said. Thats good for individuals who are overweight, have arthritis, joint pain or injuries.
The concept behind water rowers is its ... replicating what you would actually get on the water: that full, smooth, consistent stroke, Fleming said.
Rowers sit on the machines with their backs straight, knees bent and fingers grasping a handle attached to the front of the machine. They kick off the rower, straightening their legs, while simultaneously pulling the handle to their chests. Rowers repeat the motion over and over, at different speeds and intensities, depending on the workout.
If using proper form, the exercise works 80 percent of the bodys muscles. The majority of the muscles worked are in the legs. It also helps strengthen the core, arms and back.
Fleming said his clients often find that its harder than it looks.
Fleming mixes in resistance training in his classes, which can accommodate between two and 10 participants. He said most classes fall somewhere in the middle.
During a Tuesday evening class, Kathy Rains and her classmate used the rowers and a TRX system, a pair of straps anchored to the wall that use gravity and body weight for resistance training.
The two women leaned back, holding onto the straps as they performed lunges, squats and pushups. The more reps they did, the more their arms started to quiver.
While Rains and a classmate rowed, Fleming crouched between their machines to offer advice: Catch your breath. Nice and slow.
Rains had been in boot camps on and off over the years. Once she tried rowing, she was hooked.
I like getting more bang for your buck, Rains, 51, said. It was something that really made me work, but I can go at my own pace.
In the eight months the Papillion woman has been rowing, shes lost 30 pounds. She also has more energy.
At the University of Nebraska at Omaha, rowing is a popular alternative to high-impact exercises, said Skyler Brooke, assistant director for fitness and strength programs. He said the machines are so in-demand on campus it can be hard to find one available.
Brooke has clients use the rowers while warming up because it works so many muscle groups. He said the exercise also can supplement other training.
Brooke also incorporates the rower into his high-intensity interval class on campus. Sometimes participants race on the machines or track how many calories they burn.

Rowing hasnt caught on everywhere. While most local gyms have rowing machines, few offer rowing-centered classes.
Many instructors credit the CrossFit craze with boosting rowings growing popularity. CrossFit gyms use the rowers daily for warm-up and cool-down routines. The machines are incorporated into different interval workouts about three times a week, said Jonathan Pingel, owner of CrossFit Papio.
The machines appearance on the Netflix series House of Cards helped, too.
Sales spiked at the water rower company after the shows protagonist Frank Underwood started using the machine, said David Jones, director of sales and marketing at WaterRower.
The company has been producing more than 1,000 machines a week to keep up with demand.
Patty Scarpello, who works out at First Row Fitness, spent nearly three decades doing the same workout: running and weight lifting. The Omaha woman now rows two to four times a week and takes other fitness classes. Since changing things up, shes gone from 48 percent body fat to 32 percent.
Josee Beier was new to the sport, too. The Glenwood, Iowa, woman used to do competitive weightlifting and bodybuilding in her 20s. She then took up bike riding and Jazzercise, although the latter bothered her knees.
Since she started rowing last summer, the 55-year-old hasnt had any injuries.
At first, I thought I didnt know if I could do this, Beier said. You get in a groove. Its a sport that anyone can do.

Muscle weighs more than fat. Youve probably heard that phrase a million times before. And although it's false (a pound is a pound), you may have taken it to mean you should stay far, far away from the weight rack if youre trying to drop pounds. We get where youre coming from, but its not exactly that cut and dry.
At the most basic level, losing weight comes down to being at a caloric deficit, which means you take in fewer calories than you burn, says Nick Tuminello, a personal trainer and author of Strength Training for Fat Loss. You could do that by skipping your afternoon vending machine visit or by jogging for an hour after work oryepby lifting weights. While some studies show cardio is king when it comes to the best exercise for weight loss, theres something to be said for strength training. But more importantly, theres what happens after youve built muscle. Muscle burns more energy (read: calories) throughout the day than fat does, so having more muscle stokes your metabolism.A recent study found that nine months of resistance training increased study participants resting metabolic rate by an average of 5 percent. Think of it this way: Even if youve committed to the couch for a Big Little Lies marathon, the more muscle you have, the more calories your body will burn while youre just vegging out. Basically, you have a bigger engine that needs more fuel, Tuminello says. Were not talking massive amounts of caloriesadding one pound of muscle will burn an extra five to 10 calories per day, Tuminello saysbut every little bit helps you inch closer to the calorie deficit needed to lose weight.
Even though exercise (no matter what type) is always a good idea, intensity is key. More challenging workouts will have a greater impact on your metabolism, whichhelps you burn more fat during and after exercise,Tuminello says.
You can tweak your strength-training sessions to maximize the number of calories you burn. Tuminello suggests focusing on metabolic resistance training (MRT), which is basically high-intensity interval training (HIIT) with resistance added. He suggests performing circuits (completing 3 rounds of 3 to 4total-body exercises back to back)and complexes (circuits using the same piece of equipment to minimize down time). For example, you might grab a pair of dumbbells and perform bent-over rows and then go straight into Romanian deadlifts and then dumbbell squats without resting in-between.
The idea is to get your heart rate elevated and keep it there, but you also want to make sure the dumbbells you pick up are challenging without compromising your form. Lifting heavier weightsfor fewer reps burns significantly more calories for two hours after a workout than more reps with lighter weights. Heres the catch: Building muscle might have you looking and feeling fitter and more toned, but those changes may not be so obvious when you hop on the scale. Thats because muscle is denser than fat, and one pound of fat takes up about four times as much space as muscle. If the mirror is looking good, but the scale isnt necessarily changing, what youre really doing is changing the composition of your body, Tuminello says. And that's a great thing! Youre losing fat and gaining muscle, which resistance training does more effectively than endurance training, according to recent research. This post from a fitness Instagrammer sums it up: Shes clearly her fittest at 140 pounds, even though thats 18 pounds heavier than her lowest weight.
Yes, exercise, including resistance training, is essential for general health, weight loss, and weight maintenance, but dont forget that diet is No. 1 when it comes to weight loss. Research shows thata combination of diet and exercise is the way to go for sustaining it. Pay attention to your diet to better reveal your shapeand use strength training to improve that shape, Tuminello says. Besides,thebenefits of a healthy diet and daily exercisego way beyond just weight loss. Sorather than getting hung up on numbersespecially if its causing you to obsess and step on the scale every single dayditch the weigh-ins and focus on how your clothes fit and how you feel. Kardashian West doesnt always rely on such peculiar diets, though. The mother of two revealed to PEOPLE in October that she had lost nearly 70 pounds after welcoming herson Saint almost a year earlier.
I think dieting is so important to weight loss, whereas, I didnt really ever think that before, she said then. I thought, Oh, I can work out, I can just eat whatever I want. But you have to work out all the time.

When we think about what we are going to eat for a meal, we may not take into consideration the way the diet affects our brain, good or bad. However, its something we should be aware of.
There are a variety of things that contribute to overall good health, including brain health, says Jill Kilhefner, registered dietitian at Porter Regional Hospital in Valparaiso. Its important to emphasize that its not just one thing you can do.
Kilhefner notes that regular mental activity and social activity go hand in hand with food choice for optimum health. Theres a lot of research on Alzheimers and brain-related disease that (suggests) nutrition and a good diet can prolong brain function and keep it functioning at a high level, she says.
Kilhefner says that its important to incorporate foods into your diet that are high in antioxidants and phytochemicals, especially B vitamins and protein with omega-3s, such as salmon. This translates into a diet heavy on leafy greens, fruits, nuts and healthy fats, with little dairy and red meat.
Falling into the what-not-to-eat category would be butter and stick margarine, cheese, fried or fast foods, pastries and sweets, and red meatsespecially heavily marbled fatty red meats, says Kilhefner. If you cant eliminate them entirely, it is recommended that they be limited.
Kim Kramer, registered dietitian and certified diabetes educator with Ingalls Health System, notes that it takes 100 to 120 grams of carbohydrates a day for minimal brain function, but its important to choose complex carbs, such as whole grains, beans and fruits, and to spread them out throughout the day. Foods that are high in folic acid are also important to include, and Kramer says that fortified cereals, spinach, orange juice, broccoli, beans and eggs are good sources.
Vitamin E is also key for good brain health and can be found in almonds, blueberries, avocados, plant oils, pine nuts and pumpkins, Kramer says.
At the top of Kramers list of suggested brain foods are broccoli, salmon and pumpkin (which is low in calories, but rich in vitamins A, C and E as well as high in fiber and antioxidants).
In order to get all these nutrients, youve got to plan and make an effort to include plant foods. You cant get all those nutrients in one meal, Kilhefner says. Its recommended to work them into three larger meals or five smaller meals.
Kramer suggests making a pumpkin puree to add to oatmeal or pancakes, or to use in a cake in place of oil or eggs.
Kilhefner recommends two diets you can look to for recipe inspiration: The DASH (Dietary Approaches to Stop Hypertension) diet follows a heart-healthy eating style full of brain-boosting foods, and the Mediterranean diet emphasizes fish, vegetables, beans, whole grains and olive oils, all foods that promote good brain health.
Kramer adds that the Japanese diet, which is low in saturated fats and meats, but high in whole grain, fruits, vegetables and legumes, decreases the risk of depression by 25 to 35 percent compared to a Western diet.

Cutting out certain amino acids - the building blocks of animal proteins - extended the lives of mice by starving tumours.
Removing two in particular - known as serine and glycine - from the lab rodents' diets slowed the development of lymphoma and intestinal cancer.
Glasgow University researchers also found the special diet made some cancer cells more susceptible to reactive oxygen species.
Chemotherapy and radiotherapy boost levels of these chemicals in the cells - so a specially formulated vegan diet could make conventional cancer treatments more effective.
The next stage would be to set up clinical trials with patients to assess the feasibility and safety of such a treatment.
Dr Oliver Maddocks said: Our findings suggest restricting specific amino acids through a controlled diet plan could be an additional part of treatment for some cancer patients in future, helping to make other treatments more effective.

Amino acids are the building blocks that cells need to make proteins.
While healthy cells are able to make sufficient serine and glycine, cancer cells are much more dependent on getting these vital amino acids from the diet.
Co-author Professor Karen Vousden, Cancer Research UK's chief scientist, said: This kind of restricted diet would be a short term measure and must be carefully controlled and monitored by doctors for safety.
Our diet is complex and protein - the main source of all amino acids - is vital for our health and well-being.
This means that patients cannot safely cut out these specific amino acids simply by following some form of home-made diet.

The study published in Nature also found the diet was less effective in tumours fuelled by a faulty gene known as Kras, such as most pancreatic cancer.
This is because it boosted the ability of the cancer cells to make their own serine and glycine.
This could help to select which tumours could be best targeted by diet therapy.
Dr Emma Smith, science communication manager at Cancer Research UK, said: This is a really interesting look at how cutting off the supply of nutrients essential to cancer cell growth and division could help restrain tumours.
The next steps are clinical trials in people to see if giving a specialised diet that lacks these amino acids is safe and helps slow tumour growth as seen in mice.
We'd also need to work out which patients are most likely to benefit, depending on the characteristics of their cancer.
Three years ago a study found eating too much protein could be as dangerous as smoking for middle-aged people.
Research which tracked thousands of adults for nearly 20 years found those who ate a diet rich in animal protein were four times more likely to die of cancer than someone with a low protein diet.
The risk is nearly as high as the danger of developing cancer by smoking 20 cigarettes each day.
