Does the Ketogenic Diet Benefit Patients With Heart Failure? – Medscape
The ketogenic ("keto") diet, originally developed in the 1920s as an alternative treatment for pediatric epilepsy, remains a therapeutic strategy for a variety of conditions, including neurodegenerative diseases, diabetes, nonalcoholic fatty liver disease, cancer, and, most notably, weight loss. However, conflicting study results fuel the ongoing debate regarding the role of the ketogenic diet in patients withheart failure (HF). Is the ketogenic diet beneficial in HF in the long term, and how does it affect cardiovascular risk factors? Here's a look at the current research.
The daily caloric intake for people on the ketogenic diet is, approximately, 70%-80% from fats, 10%-20% from proteins, and 5%-10% from carbohydrates. Reducing carbohydrate intake while increasing fat intake results in a metabolic shift in the body called ketosis. In this state, blood ketone levels are increased due to the restriction of carbohydrates, decreased circulating insulin (and consequently, decreased insulin-mediated inhibition of lipolysis and ketogenesis), and a subsequent increase in the mobilization of adipose tissue fatty acids, which are used by the liver for ketogenesis.
Ketones can be utilized by the heart as an energy source in an unregulated manner, raising the question of whether it is possible to increase energy production in cardiac metabolic scenarios such as HF. Studies on patients with advanced HF have shown an increased adaptive reliance of the failing heart on ketones as an alternative energy source. Findings from the EMPA-REG OUTCOME trial, which looked at the effects of empagliflozin on morbidity and mortality in patients with type 2 diabetes at high risk for cardiovascular disease (CVD), led to the hypothesis that higher levels of circulating ketones increase cardiac ketone oxidation and may contribute to improved cardiovascular outcomes. Furthermore, a crossover design study, in which patients with heart failure with reduced ejection fraction (HFrEF) were intravenously infused with either the ketone, beta-hydroxybutyrate, or placebo, demonstrated that increases in circulating ketone levels increased cardiac output by 40% in participants who received beta-hydroxybutyrate. Therefore, the capacity of the heart to oxidize ketones appears to be beneficial in patients with HF.
The American Heart Association's dietary guidelines for cardiovascular disease recommend limiting intake of saturated and trans fat, decreasing sodium, and consuming a variety of fruits and vegetables, whole grains, poultry, and fish. These dietary recommendations contradict the high-fat/low-carb ketogenic diet because fruits and many vegetables are high in carbohydrates. While mono- and polyunsaturated fats may be beneficial in a heart failure setting, many ketogenic diet meal plans result in the consumption of a variety of both saturated and unsaturated fats. Therefore, careful planning to decrease saturated fat consumption and increase unsaturated fat intake is necessary if the ketogenic diet is to be implemented in a heart failure setting.
Studies looking at CVD risks of high-fat, low-carbohydrate diets have shown mixed results. For example, a meta-analysis of 11 randomized controlled trials found that ketogenic diets promoted greater weight loss than low-fat diets, but they also increased low-density lipoprotein cholesterol (LDL-C). In contrast to these findings, multiple studies have reported improvement of CVD risk factors with no adverse effects on lipid profiles in men, women, and adolescents who followed a low-fat diet. Yet, a meta-analysis by Nordmann and colleagues comparing low-fat and low-carbohydrate diets found that ketogenic diets improved triglyceride and high-density lipoprotein cholesterol values but worsened LDL-C. These contradictory findings are probably due to differences between the types of ketogenic diets used (eg. type of fat) and how each study was modeled (eg, variations in specific daily caloric intake, macronutrient composition, and duration). More clinical studies are necessary to determine the impact of a ketogenic diet on CVD risk factors.
While we know that increased ketone metabolism is adaptive in HFrEF, whether ketogenic diets can confer these benefits remains unclear. Findings from a crossover study of 16 healthy participants assigned to a ketogenic or control diet demonstrated a decreased cardiac phosphocreatine-to-adenosine triphosphate ratio in patients on the ketogenic diet; these findings signify an impaired energy production that is similar to that seen in patients with HF. When considering heart failure with preserved ejection fraction (HFpEF), because metabolic inflammation is becoming recognized as the crux of HFpEF pathophysiology and the ketogenic diet exhibits anti-inflammatory properties, the latter may be beneficial in HFpEF. However, whether the ketogenic diet can confer the same benefits in HF as does the direct administration of ketones remains to be determined.
Overall, assessment of the ketogenic diet's long-term effects on cardiac function in the setting of HF are limited. In fact, most studies to date have evaluated only short-term effects of a ketogenic diet in patients without HF or other cardiovascular risk factors. Moreover, because ketogenic diets vary by recommendation (eg, amount of daily protein intake), there is no general consensus on cardiovascular implications, treatment duration, and mode of implementation. On the basis of current evidence, future high-quality clinical trials that assess the long-term effects of the ketogenic diet in the setting of HFrEF and HFpEF are needed.

DEAR DR. ROACH: Im 64 and thin, on no medications and had my bloodwork done. Cholesterol showed 226 total, HDL 77, triglycerides 79, LDL 131, glucose 94. My blood pressure is usually about 90/60, and I have never smoked.
My doctor wants me to go on a statin, which I know is not good. I want to try adjusting my diet. Are these levels so high that they warrant a statin? Should his first course of action be to suggest changing diet? Your opinion is important. Thank you. J.F.
ANSWER: Statin drugs are a powerful way to reduce the risk of heart attack and stroke in people at higher risk for developing them; however, like all drugs, they have the potential for adverse effects. They should be used only when the benefit of taking them is expected to be greater than the risks. Nearly everyone with known heart disease or history of stroke due to blockages in the arteries should take a statin if they can tolerate it. For people without a history of cardiovascular disease or cerebrovascular disease, a good tool is a risk calculator.
These are not perfect, but they give a good starting place to estimate risk. Using the American Heart Association calculator at cvdrisk.com, the best guess of you developing heart disease or stroke in the next 10 years is 2.3%. Based on this result, a statin would generally not be recommended, although there are conditions that increase risk of heart disease that are not considered in the calculator. Other medical conditions and a patients preferences need to be considered before recommending medication therapy.
Almost everyone can reduce their risk of many diseases, not just vascular disease, by eating more plants and less meat, reducing simple sugars and starches, and having a diverse diet.
I recommend thinking of statins as a way to decrease heart disease risk. Blood cholesterol is just one part of risk. Your age and sex, and low blood pressure are helping to protect you, but you may someday still benefit from a statin. If your cholesterol and blood pressure numbers stay the same as they are now, a statin may be in your future in 10 years.

Chief planner hints at backtrack on Section 106 reforms – Housing Today
The government appears set to row back on radical proposals set out last summer to ditch the current Section 106 system in favour of a flat-rate infrastructure levy across England.
In comments to an online event hosted by the Campaign to Protect Rural England, housing ministry chief planner Joanna Averley said the government will instead proceed incredibly carefully in bringing in any reforms. She said it was very aware of the micro nature of landscapes and housing markets in England.
Her comments follow widespread criticism that the governments proposals to replace the system of developer contributions from Section 106 agreements and the Community Infrastructure Levy (CIL) with a new flat-rate levy risk stalling developments on brownfield sites and jeopardising the delivery of affordable housing.
Averley (pictured, left) said developers had also sent a very strong message to government that they valued the direct link provided by Section 106 agreements between developments and the local communities in which they take place.
Despite the bold proposals outlined in last Augusts planning white paper, Averley said the government was only at the start of thinking through how to reform the system of developer contributions. Any system would take account of the unique circumstances of individual communities, she added.
She said: In changing any form of what is basically development taxation, we need to do that incredibly carefully, we need to make sure that there arent consequences that arent anticipated.
We are very aware, as people have said, that the English landscape is very unique and very micro in its character, and that goes for housing market and property values.

She said a lot of developers had told the government that the connectedness of Section 106 and CIL to the local community is something that they value, and added that we need to understand how that will be reflected in any new system.
Critics of last summers proposals had suggested that proposals for a flat-rate financial contribution by developers to local infrastructure would simply serve to make more marginal brownfield sites, or those with significant infrastructure requirements, unviable, pushing development on to green fields.
Consultant Roger Hepher, founder of HGH Consulting, said he now expected the government to ditch its idealistic and inadequately thought through plans to reform developer contributions. He said the government was now more likely to amend the existing CIL regime.
This could be quite attractive from the industrys point of view - CIL hasnt been an unmitigated success, but at least it is accepted and generally understood, he said.
The government has said it will publish a response in the spring to its own consultation, setting out how it intends to proceed with its reforms, ahead of legislation in the autumn.
The comments came as a group of environmental charities, led by the CPRE, published an alternative vision for planning which called on the government to row back on plans to remove powers to determine individual planning applications from new designated growth zones.
Tom Fyans, deputy chief executive of the CPRE, said the government needed to create a planning system that put people, and tackling the climate and ecological emergencies, at its heart.
We all deserve a home we can genuinely afford to live in, and to have a say in shaping the communities around us. But, as things stand under the governments current proposals, the opportunity to influence what happens and where in our communities would be halved.

"When it is dark, you taste charcoal, the same charcoal that's on a piece of toast," noted another. But those who thumb their noses at dark roast coffee may change their tune if they were fully aware of its outsize health benefits.
A cup of dark roast comes packed with plenty of vitamins and minerals, including riboflavin, which helps your cells flourish and your body better process amino acids. At the end of the four-week study, the dark roast-drinking participants lost more bodyweight overall than the light-roast drinkers. "The dark roast coffee has a special substance called N-methylpyridinium ions, which is responsible for the decline in weight. And this substance is formed only when the coffee is roasted."
The difference between light roasts and dark roasts has everything to do with the way the roasters heat the coffee beans during the roasting process. Light roasted coffees, as the name indicates, aren't exposed to as much heat and aren't roasted as long. As a result, they retain more moisture, are denser, and retain more caffeine. (If you really need to wake up, always choose light.) Dark roasts, meanwhile, are exposed to greater periods of stronger heat, so they emerge from the process with a darker complexion, less density, less caffeine, that undeniably smoky (or "burnt") taste, andif you're a true coffee snobless flavor. That being said, they've got a higher concentration of N-methylpyridinium ions and a lower concentration of chlorogenic acids than lighter roasts, and those factors, say scientists, allow dark roast coffee to help you lose weight.
Of course, lighter roasted coffee is no slouch in the health department. Given the higher amount of antioxidants in lighter roastschiefly the aforementioned chlorogenic acidthe lighter variety of coffee is better at lowering inflammation, according to the National Coffee Association.
"We know that antioxidants protect against many health problems, and we also know that inflammation is the basis of many chronic diseases, whether it is diabetes, heart disease, cancer, or neurological diseases likeAlzheimer's," Sampath Parthasarathy, Ph.D., interim associate dean at the University of Central Florida School of Medicine, explained to Delish. When I Finally Have a Wedding After COVID-19, I’m Not Going to Care How Much I Weigh – POPSUGAR
At 35 years old, I'm one of the last of my close friends to get married. I remember watching each and every one of them whittle themselves down to their thinnest selves for their weddings. So when I finally got engaged a little more than a year ago, I relished the idea of planning my workouts and meals and getting in the best shape of my life before the big day. I would shed as I've never shed before! I had a month-by-month plan to do so. Then, the COVID-19 pandemic hit.
I'm not overweight, and I've always been a relatively healthy person. However, once I got engaged, I went down a rabbit hole. I spent hours staring at willowy Instagram models in formfitting wedding dresses and googling terms like "wedding diet" and "bridal boot camp." Even when I went wedding-dress shopping, both I and the woman working there held the assumption that I would lose weight before the wedding. I liked the way the dress looked when I tried it on, but I wanted to love the way it looked months later.
I started off 2020 trying to outline what my weight-loss plan would be. I chose a workout studio I'd go to religiously and splurged on a pricey unlimited membership to get the results I desired. Then, my shedding-for-the-wedding plan came to a halt. My favorite workout studios shuttered, and even the wedding planning itself was put on pause.
Months before our scheduled September wedding, I knew in my gut we would have to reschedule. My concerns shifted from planning a wedding to finding a new date that would work for our vendors and letting our family and friends know about the change. I wondered when normal life would resume. As my anxiety about the future grew, the progress I had made on my fitness goals came undone.
We pushed our wedding to Memorial Day weekend of 2021. Now that the new year is here, there's still uncertainty about whether we can have the wedding we want. When I envision my wedding, I imagine myself looking toned and beautiful. However, what I want so much more now is to be able to hug my friends and family. To be able to dance close with everyone I love. To be able to take photos with my arms wrapped around my friends. I want to celebrate finding my person and be able to do so with the many special people in my life. I'd gain weight if it meant we could have the pre-COVID wedding we imagined.
I'm starting this new year focused on my health, as opposed to weight loss. I've always used exercise as a means of stress relief, and I need that now more than ever. I work out to feel better. I lift weights so I can get stronger, prepping to one day lift up the children I hope to have. Instead of restricting calories, I'm eating well so I can stay healthy during this continued pandemic.
I'm nowhere near my initial goal weight, but just yesterday, my fianc turned to me and told me I looked beautiful. Sweatpants on, hair undone, makeup-free. Isn't that what getting married is truly about? Feeling loved and beautiful, no matter the number on the scale.
