DEAR DR. ROACH: We are very fond of our family doctor. I was in to see him the other day, and it came up in conversation that he was going to be getting married.

We would like to acknowledge this event in his life with either a lovely card or a small gift. I know the old-school way of thinking is to never do things that are so personal when it comes to your physician. Do you think that still holds true today? N.W.

ANSWER: Physicians are human beings, and a thoughtful card is always appreciated. Gifts can sometimes be OK, but the American Medical Association has some guidelines for physicians on accepting gifts. I agree with them and you might find them helpful:

Be sensitive to the size of the gift: Inappropriately large gifts should not be accepted. Do not let a gift influence patient care.

Decline a gift that would present an emotional or financial hardship to the patients family.

Consider suggesting a charitable contribution in lieu of a gift.

I would add that sometimes rejecting a gift can be hurtful, and if accepting a gift conforms to the guidelines, I generally accept it with thanks.

DEAR DR. ROACH: Hi. Im a very healthy, 68-year-old male. My only issue is almost nonexistent testosterone. What type of doctor should I see? It seems to be a problem for many senior men. A.

ANSWER: Low testosterone is a concern for many men as they get older. In some cases, the concern is justified. Some men may have a problem making testosterone due to a condition in the brain (the hypothalamus and pituitary gland both need to be working properly for testosterone production) or in the testes, where testosterone is actually made. In these cases, testosterone replacement therapy is clearly indicated.

What is still controversial is whether to treat healthy older men who have a low testosterone level and no symptoms. Its not clear that treatment of low testosterone will prevent problems in the future.

Its possible thats the case, but I am very cautious about prescribing a drug with potential harms for someone without symptoms, when it has not been proven to be of benefit.

However, men with symptoms of low testosterone, such as low sex drive, depressed mood and loss of body hair, and who have clearly low testosterone levels may benefit from treatment, and a trial of testosterone treatment is appropriate.

Men should learn about potential risk (prostate cancer remains a concern, despite reassuring early studies) and have regular laboratory evaluation. Low energy by itself is not clearly a symptom of low testosterone and may not improve with testosterone treatment.

DR. ROACH WRITES: A recent column on easy bruising in older adults prompted several readers to recommend specialized makeup products to cover the discolored areas. That is not something I had considered, and I appreciate the recommendation. Another reader recommended dry skin brushing, but I could not find good evidence that this helps with skin discoloration.

Shoulder acne: Types, causes, and how to get rid of it – Medical News Today

Acne occurs when oil, dead skin cells, and bacteria become trapped inside hair follicles, resulting in infection and inflammation.

Although acne often develops on the face, it can affect hair follicles on other parts of the body, such as the chest, back, and shoulders.

Below, we describe what acne is and the types of acne that may develop on the shoulders. We also explore the symptoms and appearance of shoulder acne and list some treatment options.

Pores are small openings in the skin. Each pore contains a single hair that sits inside a hair follicle. These hair follicles connect to oil-producing glands, called sebaceous glands.

Sebaceous glands secrete an oil called sebum, which carries dead skin cells to the surface of the skin.

Sometimes, sebum and skin cells form a plug within the hair follicle. This plug traps dirt and bacteria inside the skin, resulting in an infection. The infection triggers inflammation in the form of a pimple.

Two types of acne can develop on the shoulders: acne vulgaris and acne mechanica.

Acne vulgaris is the medical term for common acne. It typically occurs when oil and dead skin cells become trapped inside a pore or hair follicle.

Also, Cutibacterium acnes bacteria can trigger or worsen the symptoms.

In addition, changes in the levels of certain hormones can trigger breakouts. Some hormones that may influence acne vulgaris are:

Testosterone regulates sebum production, and rising testosterone levels can trigger acne breakouts in anyone.

Adolescent males may, for example, experience acne during puberty, when testosterone levels naturally increase.

Females with polycystic ovary syndrome may also develop acne due to high testosterone levels.

The hormone progesterone can also contribute to acne.

Progesterone levels peak during the luteal phase of the menstrual cycle, which begins after ovulation and ends just before a period. As a result, acne breakouts may occur around this time.

In addition, some people experience acne while using progesterone-only hormonal birth control, sometimes called the mini-pill.

Meanwhile, the hormone estrogen has a converse effect it inhibits sebum production and shrinks the sebaceous glands.

Over-the-counter topical treatments usually creams, gels, or foams can help clear acne breakouts. Look for products that contain:

Benzoyl peroxide is an antiseptic that kills C. acnes bacteria on the surface of the skin and inside hair follicles.

In 2017, a 12-week clinical trial investigated the effectiveness of this antiseptic as a treatment for acne vulgaris among 607 participants.

The researchers found that using a benzoyl peroxide cream resulted in a 72.775.0% reduction in inflammatory lesions, while the use of a placebo cream resulted in a 41.6% reduction in inflammatory lesions.

Salicylic acid is a beta-hydroxy acid that reduces sebum production and skin inflammation.

It also reduces acne lesions by exfoliating the skin removing oil, dirt, dead skin cells, and bacteria.

A 2018 clinical trial compared the effectiveness of a salicylic acid cream and a benzoyl peroxide cream for the treatment of mild to moderate acne. The trial included 31 participants, and all applied each cream to separate sides of their faces for 28 days.

The researchers found that the 2% salicylic acid cream reduced the number of papules and pustules by 47.9% and reduced the number of noninflammatory lesions by 43.1%. These results were comparable to those of the benzoyl peroxide cream.

The team concluded that the two creams were similarly effective in treating acne.

Retinoids are a class of chemical compound derived from vitamin A. They increase skin cell turnover, keeping dead cells, oil, and bacteria out of pores.

A person needs to use a retinoid consistently to see results. This is not a spot treatment.

Azelaic acid exists naturally in grains, such as wheat, barley, and rye. Topical application helps decrease redness and swelling of the skin.

A small 2017 pilot study investigated the safety and efficacy of a 15% azelaic acid foam in the treatment of mild acne on the torso.

The results showed that 44% of participants who applied the foam twice daily experienced a significant reduction in their acne.

The following prescribed medications, which come in pill form, can help treat the underlying causes of acne.

A common treatment for men with intermediate-risk prostate cancer is to combine radiation with drugs that block testosterone a hormone that makes the tumors grow faster. (This type of treatment is also called androgen deprivation therapy, or ADT).

New research is suggesting the sequence of these treatments may be crucially important.

Dr. Dan Spratt, a professor of radiation oncology at the University of Michigan, led the research. He and his colleagues pooled data from two previously published clinical trials (here and here). Taken together, the studies enrolled just over 1,000 men who had been randomly assigned to one of two groups:

By themselves, the individual studies didnt show a significant difference in outcomes after an average follow-up of nearly 15 years. But by pooling the data, Spratts team produced a dataset with enough statistical power to show that men who started on hormonal therapy either during or after radiation did significantly better in all respects: compared to the men who were treated with hormone therapy first, they had lesser odds of experiencing a rise in prostate-specific antigen levels (suggesting the cancer might be returning); they lived longer without a progression of their disease; and they were less likely to have cancer spread to other parts of the body. The results also suggested they had a lower risk of actually dying from prostate cancer, although this particular finding wasnt statistically significant.

Researchers have already devoted a lot of attention to how long hormonal therapy should last when its given with radiation. This is now the first study to show that sequence also matters.

Why would that be the case? Possible explanations center on testosterones capacity to fix genetic damage in irradiated cancer cells. Just how sequencing plays into this repair mechanism isnt known, but Dr. Spratt says the new results point to avenues for further study.

Importantly, the results apply specifically to men with unfavorable intermediate-risk prostate cancer who typically get four to six months of hormonal therapy. According to Spratt, sequencing may not be as relevant to men with high-risk tumors who can get hormonal treatments for several years.

Dr. Marc Garnick, the Gorman Brothers Professor of Medicine at Harvard Medical School and Beth Israel Deaconess Medical Center, editor of the Harvard Health Publishing Annual Report on Prostate Diseases, and editor in chief of HarvardProstateKnowledge.org, says there are circumstances in which some men will need hormonal therapy before radiation. Describing the new results as compelling, he adds Efforts to investigate sequencing prospectively (i.e., forward in time) should become an important component of future research.

Review: ‘Call of Duty: Black Ops Cold War’ is zany, action-packed game that takes few risks – Fox17

Call of Duty: Black Ops Cold War takes a dramatic tonal shift from its super-serious 2019 predecessor, making it one of the goofier entries in the annualized franchise. While Modern Warfare was committed to realism, especially in its campaign, this new game is soaked in conspiracy theories, explosions, and cheesy 80s haircuts. Despite this sharp pivot, Cold War quickly finds its footing by committing to its wacky tendencies, and out-there plot twists. It never quite reaches the heights of the previous game, but its far from a misfire.

If this games campaign was turned into a feature film, it would be a contender for the most dad-friendly action flick of the year. All the ingredients are there, a Robert Redford look-alike, rock n roll music, Russian spies, and a villain posing a global threat that only a special team can prevent from happening. Maybe it was the testosterone radiating from my screen, or maybe it was too much coffee, but I was on-board with the ridiculousness from the get-go, even if its a tad too much off the rails at times.

From a length and difficulty perspective, Cold War follows the same pattern of being short and sweet as previous games, but its in the difficulty that I found a new appreciation for this series. While Im no stranger to lowering things down to easy mode, in Cold War I played the campaign on the highest difficulty and found it to be surprisingly balanced and fair. Firefights are just the right amount of tense, without making the player feel like they need to duck after every shot, but if you make the wrong move, a game over is still inevitable. Thanks to the games friendly checkpoint system, a restart is never the end-of-the-world,but just a small delay in progression. Giving this mode a try is bound to help you when you take your skills online.

On the multiplayer side, things are not quite as unorthodox as you might have come to expect, but its still solid. Each year, when I dabble in Call of Duty multiplayer, I expect quick, snappy action and maps that are memorable. Cold War checks both those boxes. All the classic modes are here, including Team Deathmatch, Domination, and Search and Destroy. If youre looking for larger-scale battles, the battle royale like Warzone is included in the package, and for the horde mode enthusiasts, Zombies is back too. Best of all, crossplay is back as a feature, bringing gamers that much closer to the end of the console wars. After more than a decade of these games, seeing experience points increase, guns unlock, and stats pop-up on-screen still elicits a rush of excitement like few shooters can create, which explains why the series continues to be on-top each year.

If theres anything to critique when thinking about Cold War, its that it takes few risks. Maybe its riding the coattails of last years formula, but I would have liked to see more zany qualities instilled in the multiplayer mode to match the campaigns tone. Including just the slightest bit of 80s themes would have been enough to separate itself from Modern Warfare. Just give me an eye-rolling one liner, maybe a Rambo-like headband, anything at all! Instead, it just feels like the same game with new maps, making for the same old Call of Duty atmosphere. I was never confused about what a perk does, button layout, or rules of a game mode. The series perfected multiplayer long ago, its time to take more risks.

Call of Duty: Black Ops Cold War is another successful entry in the franchise. Although with each preceding year the series hinges on becoming stale, this game brings over what made last years game so beloved, and adds just enough goofiness to the campaign to differentiate itself, making it a contender for one of the best first-person shooters of 2020.

The DASH diet is a low fat, low sodium diet that healthcare providers often recommend to help reduce high blood pressure.

Additionally, one review of studies on several commercially available diets found that people on Nutrisystem lost about 4% more weight after 3 months on the program than people who received only diet education or counseling (5).

However, the review could not look at long-term results because all the studies it investigated were less than 1 year in duration.

Its also important to note that these studies looked at the results of other Nutrisystem programs, including Nutrisystem D. They did not specifically investigate the results of Nutrisystem for Men, though these programs are similar.

Nutrisystem appears to help people lose weight over the short term, but these studies didnt investigate long-term results and may have some bias. They also didnt specifically investigate Nutrisystem for Men.

There are several drawbacks to the Nutrisystem for Men program.

One of the Nutrisystem-funded studies noted that men on the program consumed an average of only 1,500 calories per day (4).

Some studies looked at the results of following Nutrisystem for 46 months. However, many people may find it too expensive or unrealistic to stay on Nutrisystem for Men for that long. That said, you can complete as many cycles of the 4-week program as youd like (3, 4).

Finally, while Nutrisystem-branded foods are formulated to be low in calories and rich in fiber, theyre highly processed. Choosing more whole, minimally processed foods may be better for your overall health (10).

Whats intermittent fasting? The science behind it – ZME Science

One of the worlds most popular health (and even fitness trends) is intermittent fasting. Intermittent fasting is an eating pattern that cycles between periods of fasting and periods of eating. It focuses more on when you eat than on what or how much.

People use intermittent fasting to lose weight and improve their health, with several studies showing that it can have powerful effects on your body and brain.

Its not a diet in the conventional sense, but more accurately described as an eating pattern.

Fasting has been a practice throughout much of human history and, as a result, we evolved to be able to function without food for extended periods of time. Using it from time to time is just as natural as eating three meals a day if not more so.

Theres more than one way of doing intermittent fasting. There are several methods that involve splitting the day or week into eating and fasting periods. Essentially, you introduce a daily or weekly fasting period.

These are the most popular methods:

During the fasting periods, you eat either very little or nothing at all (no drinks with calories, either). By reducing your calorie intake, all of these methods can cause weight loss as long as you dont compensate by eating much when you do eat. Many people find the 16/8 method to be the simplest, most sustainable, and the easiest to stick to. Its by far the most popular.

Fasting leads to a set of reactions in your body on the cellular and molecular level. Your body adjusts hormone levels to make stored body fat more accessible, while your cells also initiate important repair processes and change the expression of genes.

The levels of growth hormone can also skyrocket with intermittent fasting, increasing as much as 5-fold. This has benefits for fat loss and muscle gain, among other things. Insulin sensitivity improves and levels of insulin drop dramatically. Lower insulin levels make stored body fat more accessible.

When you fast, your cells can initiate or accelerate cellular repair processes. This includes autophagy, where cells digest and remove old and dysfunctional proteins that build up inside cells. Changes are also observed in the function of genes related to longevity and protection against disease.

Its also been suggested that periods of intermittent fasting increase cellular stress resistance and defense, repairing important pathways that improve the resistance of cells to stressors, promoting cellular growth. However, the science around intermittent fasting is not entirely clear.

When thinking about fasting, weight loss is the most common reason given. By making you eat fewer meals, it can lead to an automatic reduction in calorie intake. Additionally, it changes hormone levels to facilitate weight loss.

Studies show that intermittent fasting can be a very powerful weight-loss tool. A 2014 review study found that this eating pattern can cause 38% weight loss over 324 weeks. According to the same study, people also lost 47% of their waist circumference.

It should be said that researchers studying fasting are calling for more human studies, especially large-scale human studies. Many of the purported benefits of intermittent fasting come from animal studies or small-scale animal studies and have not been confirmed in larger cohorts. In addition, while weight loss seems like the most appealing upside, other claims are debatable.

For instance, decades of studies on rodents have shown that intermittent fasting can help them remain lean, develop fewer aging-related diseases, and live 30-40% longer. A 2019review of studies in theNew England Journal of Medicine showed that intermittent fasting can lower blood pressure and cholesterol levels, improve brain health, reduce inflammation, and boost endurance. But studies on humans have been less conclusive. It seems to be good for losing extra pounds, but anything else is less clear.

However, even then, keep in mind that the main reason for its success is that intermittent fasting helps you eat fewer calories overall. If you binge and eat massive amounts during your eating periods, you may not lose any weight at all.

Intermittent fasting is probably not for everyone. This is actually one of the reasons why its so hard to thoroughly confirm any nutritional study: theres so much variety between different individuals, diets, and a million other factors that can have an influence. It may even be detrimental.

If youre underweight or have a history of eating disorders, you should not fast without consulting with a health professional first. In these cases, it can be downright harmful.

There is also some evidence that intermittent fasting may not be as beneficial for women as it is for men. For example, one study showed that it improved insulin sensitivity in men, but worsened blood sugar control in women. Another study suggested that it can lead to shedding muscles, not fat, which would be counterproductive.

There are a number of anecdotal reports of women whose menstrual period stopped when they started doing fasting and went back to normal when they resumed their previous eating patterns. For these reasons, women should be especially careful with intermittent fasting. They should follow separate guidelines, like easing into the practice and stopping immediately if they have any problems like amenorrhea (absence of menstruation). This eating pattern is likely also a bad idea if youre pregnant or breastfeeding. As always, consult with a professional before embarking on any diet.

Among the side effects, hunger is considered the main one the bane of all diets. People also feel weak and their brains may not perform as well as they are used to. This may only be temporary, as it can take some time for the body to adapt to the new meal schedule. Most people seem to get on and adapt after a couple of weeks.

If you have a medical condition, you should consult with your doctor before trying intermittent fasting. This is particularly important if you have diabetes, problems with blood sugar regulation, low blood pressure, take medications, are underweight or have a history with eating disorders.

All that being said, intermittent fasting has a rather remarkable safety profile. There is nothing dangerous about not eating for a while if youre healthy and well-nourished overall. Unlike other diets which can be extreme, it doesnt seem to produce any major risks.

Its likely that at some point in your life, youve inadvertently done some intermittent fasting. If youve ever eaten dinner, then slept late and not eaten until lunch the next day, then youve probably already fasted for 16+ hours. Ever skipped breakfast or dinner? Same thing. Some people instinctively eat this way. They simply dont feel hungry in the morning.

Many people consider the 16/8 method the simplest and most sustainable way of intermittent fasting, so this one might be the best one to try first. If you find it easy and feel good during the fast, then maybe try moving on to more advanced fasts like 24-hour fasts 12 times per week.

Another approach is to simply fast whenever its convenient simply skip meals from time to time when youre not hungry or dont have time to cook. There is no need to follow a structured intermittent fasting plan to derive at least some of the benefits.

Intermittent fasting is not something that anyone needs to do. Its simply one of many lifestyle strategies that can improve your health. Eating quality food, exercising, and taking care of your sleep are still the most important factors to focus on.

If you dont like the idea of fasting, then you can safely ignore this article and continue to do what works for you. At the end of the day, there is no one-size-fits-all solution when it comes to nutrition. The best diet for you is the one you can stick to in the long run.

Intermittent fasting is great for some people, not others. The only way to find out which group you belong to is to try it out. If you feel good when fasting and find it to be a sustainable way of eating, it can be a very powerful tool to lose weight and improve your health.
