A friend related to me that his home recently had been broken into and that he went to the door with gun in hand to face the robber. How he is in his 80s with many health issues, has never taken classes in gun management nor has he fired a weapon in 20 years.

I laughed and asked him why such a smart guy could make such a thoughtless decision.

Sometimes discretion is the better part of valor. While growing up, I watched foolish men on the screen take on Matt Dillon and John Wayne, and they received a tombstone for their efforts. They were driven to destruction by too much testosterone and an overabundance of ego. They believed as do many young people, including myself that they were indestructible.

All of us have been distressed by the loss of life on the streets of America. Those horrific acts define race, creed or status in life. We grieve for those on both sides of the law and wonder how and when the destruction will stop.

Wildlife specialists warn us all that when faced with a carnivore such as a bear, lion or tiger, never run but simply back away cautiously. That clear message applies to the streets of America and if followed would have prevented most of the deaths that have occurred during confrontations.

Do not run. Do not fight! Save your life. By not resisting, jail could be the result. Dont feel bad, because some of the greatest people in the world have ended up behind bars. We have seen time and again that isolation and the quietness of incarceration gives time for thought and freedom from drug addiction. This could be the magic ingredients to a better life.

Fortress Reports Positive Data And Other News: The Good, Bad And Ugly Of Biopharma – Seeking Alpha

Fortress Biotech reports positive data from CUTX-101

Fortress Biotech Inc. (FBIO) reported positive topline clinical efficacy data for CUTX-101. The trial is being conducted by its partner company Cyprium Therapeutics. The data showed the drug candidate brought about statistically significant improvement in overall survival for Menkes disease. CUTX-101 has been given Orphan Drug, Fast Track and Rare Pediatric Disease Designations by the FDA.

The primary efficacy endpoint for the trial is overall survival measured from birth. The primary efficacy analysis compared overall survival in Menkes disease patients who were administered daily dose of CUTX-101 beginning within four weeks of age to a historical control cohort of patients who were not given copper therapy. Primary efficacy analysis included 31 Menkes disease patients who received ET with CUTX-101 and 18 HC Menkes disease patients. The trial met the primary endpoint of overall survival.

The drug candidate brought about nearly 80 percent reduction in the risk of death whereas median survival for the ET cohort was 14.8 years or 177.1 months compared to 1.3 years 15.9 months for the untreated HC cohort. S. Yam, CEO of Cyprium said, These positive topline clinical efficacy data highlight the potential of CUTX-101 as an effective therapy for Menkes disease patients. With no currently approved U.S. Food and Drug Administration (FDA) treatments, Menkes disease is a serious condition with a significant unmet medical need.

The ET cohort participants carried a severe pathogenic mutation of the ATP7A gene and were born within the past 20 years. The treatment with CUTX-101 was initiated within four weeks of age, adjusted for prematurity. The participants survived at least four weeks after birth and were asymptomatic for significant neurological signs and symptoms during the first four weeks.

HC cohort participants carried a severe pathogenic mutation of the ATP7A gene and were also born within the past 20 years. These participants have not received CUTX-101 therapy and have survived at least four weeks after birth. The patients were asymptomatic for significant neurological signs and symptoms during the first four weeks.

CUTX-101 is a subcutaneous injectable formulation of Copper Histidinate. It has been manufactured to offer improved tolerability due to physiological pH and to circumvent the oral absorption of copper, which is diminished in patients suffering from Menkes disease. The drug candidate is being clinically developed for treating Menkes disease. It aims to treat the ailment by reloading Copper Histidinate, repairing copper homeostasis, and retaining serum copper levels in the normal age appropriate range. The enrollment for a Phase 3 trial of CUTX-101 has been completed. Currently, a Cypriot-sponsored expanded access protocol for Menkes disease patients is being carried out.

Cyprium Therapeutics is mainly engaged in developing therapies for treating Menkes disease and related copper metabolism disorders. It has been founded by Fortress Biotech Inc., which is mainly invested in acquiring, developing and commercializing high-potential marketed pharmaceutical products. It also deals in development-stage pharmaceutical product candidates. Fortress Biotech has five marketed prescription pharmaceutical products in its portfolio. It has robust development pipeline with over 25 programs spanning a wide range of market segments including oncology and gene therapy.

Lipocine Inc. (LPCN) provided regulatory update for its lead drug candidate Tlando. The company has been informed by the FDA that it required additional time to complete its review of the New Drug Application. The anticipated Prescription Drug User Food Act goal date was set at August 28, 2020. The FDA has not provided any alternate timeline or a new action date. The regulator has not demanded any additional data.

TLANDO is an oral testosterone replacement therapy product candidate. It comprises Testosterone Undecanoate, which helps in restoring normal testosterone levels in males for conditions associated with a deficiency or absence of endogenous testosterone. Lipocine has faced several roadblocks for Tlando. The company had received its third Complete Response Letter in November 2019. It had previously received CRLs in June 2016 and May 2018 as well.

The company had recently reported its second quarter results. Lipocine suffered a net loss of $6.4 million or $0.13 per diluted shares for the second quarter. The company had reported $3.4 million in net loss for the corresponding quarter of the previous year. Its research and development expenses for the quarter increased from $2 million to $2.3 million on year over year basis. Its General and Administrative expenses also jumped from $1.4 million to $2 million for the second quarter.

Lipocine Inc. has a robust developmental pipeline with various candidates such as TLANDO, LPCN 1144, TLANDO XR, LPCN 1148 and LPCN 1107. LPCN 1144 is a product of bioidentical testosterone. The drug candidate recently completed a proof-of-concept clinical study showing its potential effectiveness in treating non-cirrhotic NASH. TLANDO XR is a novel oral prodrug of testosterone and LPCN 1148 is an oral prodrug of bioidentical testosterone aimed to treat cirrhosis.

Enanta Pharmaceuticals Inc. (ENTA) reported updates for its non-alcoholic steatohepatitis (NASH) and hepatitis B virus (HBV) development programs. The company is currently working on EDP-305 and EDP-297 for treating NASH. Its EDP-514 is being developed for treating HBV.

Phase 2a ARGON-1 study of EDP-305, its lead Farnesoid Xreceptor (FXR) agonist, showed that the drug candidate met its primary endpoint with statistically significant reduction in alanine transaminase of 28 U/L in comparison to 15 U/L in the placebo cohort at week 12. EDP-305 at the 2.5mg dose also showed statistically significant reduction in liver fat content as measured by MRI-PDFF. The drug candidate also showed robust target engagement as evidenced by decrease in C4 and the increase in FGF-19.

EDP-297 substantially decreased fibrosis progression and enhanced liver function as measured by key biomarkers in a rat model. Rats were randomized to be administered either vehicle control (0.5% methylcellulose), 0.1 mg/kg EDP-297, or 0.3 mg/kg EDP-297 by once-daily oral gavage at the first signs of fibrosis. The data showed that the drug candidate may have a potent anti-fibrotic effect in NASH patients, including those with late-stage F3/4 fibrosis.

The data from first in human, Phase 1 study of EDP-514 showed that it was rapidly absorbed. Its exposure magnified with escalating single and multiple dosing of 600 mg and 400 mg, respectively. The drug candidate was found to be generally safe and well tolerated over a broad spectrum of single and multiple doses for up to 14 days. The trial did not report any discontinuations due to adverse events. EDP-514 demonstrated pharmacokinetics suitable for once daily oral dosing. The geometric mean plasma concentration at 24 hours was found to be significantly higher after multiple dosing with and without a standard meal. The drug candidate is currently being evaluate in two Phase 1b clinical studies.

Tom Keane: Hello, everybody. This is Tom Keane coming to you from Charleston, South Carolina, and the Medical University of South Carolina. But today I'm going to talk about a paper that was recently published in European Urology. It's entitled, "The Oral Gonadotropin-releasing Hormone Receptor Antagonist Relugolix as Neoadjuvant or Adjuvant Androgen Deprivation Therapy to External Beam Radiotherapy in Patients with Localized Intermediate-risk Prostate Cancer". This is a randomized, open-label, parallel-group, Phase II trial. Now the background to this is that there has up till now only been one LHRH agonist or antagonist, I should say. Relugolix represents the second such agent to have been developed. Degarelix was the original, which the one that got approval in 2008. Before that, in the early 2000s, there was abarelix.

Now, this is, as I mentioned, a Phase II trial and on the background, external beam radiotherapy with neoadjuvant or adjuvant androgen deprivation therapy is an established treatment option to prolong survival for patients with intermediate and high-risk prostate cancer. However, most of these trials have been done with an LHRH agonist. Relugolix is an oral gonadotropin-releasing hormone, receptor antagonist, and was evaluated in this clinical setting in comparison with degarelix, which is an injectable GnRH antagonist. So, both these drugs have the same mechanism of action, but one is an oral formulation, the other is a subcutaneous injection formulation. The objective was to evaluate the safety and efficacy of relugolix to achieve and maintain castration. Now, the design was a Phase II open-label study which was conducted in 103 intermediate-risk prostate cancer patients who were undergoing primary external beam radiation therapy and neoadjuvant or adjuvant androgen deprivation therapy. The trial was conducted between the time of June 2014 and December 2015. Patients were randomly assigned on a three to two basis to 24-week treatment with either daily oral relugolix or four weeks subcutaneous depot degarelix, which was the reference control.

The outcome measurements and the statistical analysis were as follows. The primary endpoint was the rate of effect of castration, which was defined in European terms as 1.73 nanograms per mole per liter. And in the US we would call this 50 nanograms per deciliter. And that was used in both the relugolix patients between four and 24 weeks of treatment and also in the degarelix treatment. The secondary endpoints included the rate of profound castration, which in Europe is less than 0.7 nanomoles per liter, and in the US it's a testosterone level of 20 or less. PSA levels were also monitored. Prostate volume, quality of life was assessed using two different tools. The first was the EORTC Aging Males' Symptoms scale and also there was an EORTC prostate cancer module which was a 25 item module. No formal statistical comparisons with degarelix were planned.

So castration rates during treatment were 95 and 82% with relugolix and 89 and 68% with degarelix. That's looking at the castration achieved level and the profound castration level achieved. The median time to castration in the relugolix arm was four days. The median time for castration with degarelix is generally two to three days. During treatment, PSA levels and prostate volumes were reduced in both groups. Three months after discontinuing treatment, 52% of men on relugolix and 16% on degarelix experienced testosterone recovery. The statistical significance of the differences were not tested. The mean and median quality of life scores improved following treatment discontinuation, which is what we would expect to see. The most common adverse event was hot flashes at relugolix at 57%, degarelix at 61%.

Lack of blinding is a potential limitation of this study. But the conclusions were that relugolix achieve testosterone suppression to castrate level left within days and maintained at over the 24-week period with a safety profile which is consistent with its mechanism of action. The patient's summary was that oral once-daily relugolix may be a novel oral alternative to injectable androgen deprivation therapy.

Now there's a couple of points about this paper. Again, it is not disturbing but it's frustrating to see that we are still using the 50 nanograms per deciliter are in Europe, the 1.73 nanomoles per liter level as a level of castration. If we look at the guidelines, the idea of testosterone levels to be achieved during androgen deprivation therapy in 2012 that passed the US consensus, recommended a level of 29 milligrams per deciliter for serum testosterone during ADT in patients with advanced prostate cancer as levels between 20 to 50 have had a poor clinical outcome. EAU guidelines define testosterone during ADT as reaching less than 20 nanograms per deciliter or the equivalent European value.

In 2018, the Canadian Urological Association consensus recommended the adoption of a value of 20 or less as the castrate level to be achieved, and regular monitoring of testosterone and PSA levels should be undertaken every three to six months during ADT. However, European Union Regulatory Authorities and the FDA continue to define castrate levels for testosterone at 50 nanograms per deciliter. Again, there is good data both in non-randomized trials and in the Canadian data, which was a randomized trial, that getting down to a level of 20 is significantly better than reaching a level of 50. And this paper is very interesting because for the first time, it gives us an alternative to the injectable form of an LHRH antagonist.

Having an oral form may have its benefits, may also have its drawbacks, but it's a very interesting study. It certainly, there is a Phase III study called the HERO trial, which has been presented but as that data is currently at the FDA, I think you should watch this space clearly. I do think that there will hopefully be an option between the injectable form as compared to the oral form, and we will await the results of the Phase III trials once they're published and we will be happy to bring them to you on UroToday.

New research suggests that smartphones could use their various sensors to collect information about a persons movement, specifically their gait, to measure their intoxication level.

The researchers recruited 22 adults who were given enough vodka, under laboratory conditions, to increase their breath alcohol level to 0.2%, more than twice the U.S. legal limit of 0.08%. The researchers then asked the participants to walk in a straight line for 10 steps once an hour for 7 hours.

Analysis of the resulting data allowed researchers to say when a participant was over the legal limit with 92.5% accuracy. Though only a proof of concept study, it could lead to future smartphones alerting the owner if it detects that they may be more susceptible to engaging in risky behavior after drinking.

Couple lost a combined 31st and their marriages – but found love with each other – Mirror Online

Sucking in her tummy, it did not matter which way Jenna Watts turned there was no escaping her 30st reflection in the mirror.

The newlywed was booked in for gastric bypass surgery to help her realise her dream of starting a family.

But as the weight fell away after her successful op, so did Jennas love for her husband.

After meeting fellow slimmer Rob online, sparks flew and now the pair are madly in love.

Jenna, 32, said: Its like a fairytale. Robs the perfect guy, we have so much in common.

Losing the weight has been amazing but finding Rob and having him cheering me on has been even better.

Between us, weve lost 31st, countless dress sizes and two spouses but what weve found is worth so much more true love.

Ditching all that dead weight was the best decision we ever made. Losing everything led us to so much more.

Lead support worker Jenna had struggled with her weight all her life and at the start of 2019, was 30st and wore size 34 clothes.

That March she had a gastric bypass on the NHS at the Spire Manchester Hospital.

The August before, Jenna had wed her long-term boyfriend and was keen to start a family.

But she said: I knew that at my size, there was no way I could conceive so thats when I decided to have the bypass.

After the op, Jenna shed the pounds but her marriage hit the rocks. She said: I thought my weight loss would make us stronger but my husband became jealous of my shrinking body. We quickly fell out of love.

It was a shock. Id had the operation for our future family but instead, it just highlighted all the problems in our marriage.

Devastated Jenna turned to Rob, 30, who she had met a month earlier in an online support group for gastric surgery patients, The Friendliest Gastric Sleeve/Bypass Group UK.

And Rob, who also lived in Manchester, was in a failing marriage, too.

Jenna said: I wanted to know if the weight loss had affected anyone elses relationship in the same way.

When he replied to say his surgery had affected his marriage too, I was relieved. It felt good to know I wasnt alone.

She said: Id never pursue someone while Im in a relationship. But my marriage had hit rock-bottom. I was slimmer than ever and I loved my new body, but I just didnt feel like I could be myself with my husband.

Leaving was a hard decision but we didnt love each other and I knew it was for the best.

Afterwards, Jenna told Rob her marriage was over. She said: It felt like fate. I messaged Rob as a friend but when he told me hed split from his wife and was newly-single, it felt like a sign.

The pair confessed they had feelings for each other and agreed to meet for the first time two weeks later on Valentines Day.

Jenna said: I was so nervous but as our eyes met, everything else disappeared. We had an instant connection.

Were perfect for each other, and from the moment we met, we hit it off.

Since finding Rob, Jenna has lost 16st and shrunk to a healthy size 16. She said: I couldnt be happier and I love my new figure. Doctors told me I wouldnt lose as much weight as I have but with Rob cheering me on, Ive lost more than they expected.

At 14st Im healthier than ever and I get to enjoy my new lease of life with the man I love.

Rob, a Royal Mail driver, had a gastric sleeve op and has shed 15st, slimming down from a 48 to a 34-inch waist.

He said: From the age of six, I put on a stone every year of my life. By 29, I was 30st.

The surgery changed his life. He said: Im one of those people who will give it 110 per cent, so I did. I now weigh 15st and feel amazing.

I really love wearing suits now and flaunting my slimmer frame. But thanks to the surgery, I found Jen.

Nothing happened between the pair before their marriages ended, but Rob felt a connection with Jenna online.

He said: We connected over losing weight but we never spoke out of turn to each other we were both still in relationships.

"Before the surgery I always said to my wife, Im never going to change, but I wasnt prepared for how much you do change.

One day, Jen messaged me and asked if I was having trouble in my marriage. Thats when we started talking and realised we had all these crazy weird connections. Rob now describes his weight loss as a rebirth.

He said: Unfortunately when Jenna and I were bigger, no disrespect to our exes, we sort of settled for people. I used to think I was in love with my ex, but I wasnt.

The amount of love and care from Jen is incredible.

Shes accepted me for who I am, and I do the same for her. Were so lucky to find each other.

The smitten couple have swapped promise rings and will wed when their divorces are finalised. Jenna said: I know Rob and I will be together forever, so I dont mind the wait.

Boris Johnson faces winter of discontent for national crises – The Hill

Few world leaders have had such an active six months as Boris Johnson, who recently marked his first anniversary of becoming prime minister of Britain. In late March, as the country was in the grips of the first wave of the pandemic, Johnson fell sick to the novel coronavirus. At first, he and his officials strenuously claimed that his symptoms were mild and would swiftly disappear. Ten days later, in early April, he became the first world leader to be admitted to the intensive care unit of Saint Thomas Hospital in London where he was in need of liters and liters of oxygen.

While the government denied rumors, some emanating from Russia, that the prime minister was on a ventilator, plans were made to announce his death, marking how serious his illness was. But Johnson and his partner, Carrie Symonds, would announce the birth of their son, his sixth child and his first with her. Four months later, the press have been consumed with images of Johnson working out with his new fitness trainer as he attempts to lose weight while launching a strategy to tackle obesity in Britain, where 28 percent of the population suffers from obesity.

Despite the personal ups and downs of the relatively new prime minister, his political fortunes have taken a nosedive as the rest of 2020 presents challenges for his leadership and his government. The pandemic shows no signs of abating and, while several vaccines are moving along, the coming winter is likely to pose massive problems for the National Health Service as the government tries to reopen schools.

Britain has recorded more than 330,000 coronavirus cases and over 41,000 people have died from the pandemic. Britain has the dubious honor of ranking first with the highest death rate ahead of the United States and the Group of Seven countries. Many have blamed the initial hands off approach by Johnson for tackling the pandemic and the delay in lockdowns when the coronavirus first reached Europe in late winter.

While many countries in Europe, like Italy and Norway, went into lockdowns before the middle of March, Britain waited almost two more weeks before it introduced mandated restrictions. In late July, Johnson admitted that the response could have been handled differently. An official inquiry has been promised into the management of the pandemic, which is bound to cause a certain degree of embarrassment for both Johnson and his government, with many questions left unanswered.

While t coronavirus has dominated the daily state of affairs and has convulsed much of the news coverage in Britain and elsewhere, the Brexit question has not disappeared. Britain officially left the European Union in January, and its transition period expires at the end of December. The United Kingdom still enjoys all the trade benefits it had with the European Union before exiting. But starting next year, if no trade deal is concluded, Britain will be locked out of the European Union economy of over 440 million people.

While negotiations have continued irregularly, no measurable degree of progress has been made. Many in Brussels remain deeply suspicious of the real motives of Johnson, with some senior European Union officials believing that the prime minister would prefer to let the clock run out and, with that, cut off all ties with the European Union without a trade deal, parts of which might not sit well with some of the staunch Brexit supporters in his Conservative Party.

His government enjoys a majority in Parliament that will give him some political cover as the House of Commons returns from its summer recess and as questions will inevitably mount over his response to the coronavirus and attitude toward future trade relations with the European Union. Yet as Britain edges closer to the December deadline, the many members of the Conservative Party still expect Johnson to deliver on his promise to negotiate a trade deal with Brussels. The downward spiral of the economy might also show that closer ties with the European Union might not be a bad thing.

Any prevarication by Johnson could result in a challenge to his leadership and bring to an end his short tenure as prime minister. The coronavirus and the decisions that his government has been forced to take on public health has exposed the myth of Johnson as some kind of a political genius. The Brexit trade talks will also test the false narrative that, despite the blond bombshell bluster, he has a master plan for the future of Britain outside the largest free trade area in the world. Johnson has been exposed as a man without a detailed plan as he navigates Britain into a winter of discontent dominated by a raging pandemic and the toxic issue of Brexit.
