Schepisi and Teaster: Reduce hands against glass | Commentary – Roanoke Times

When the Commonwealth of Virginia went into lockdown, one reason was to protect the most vulnerable people living in facilities, isolating them from each other and from the outside. An image widely used was of residents and family members pressing their hands against glass. One commentator said that older people were red meat for the virus. From limited information that we think we know, more than 38,000 nursing home residents and staff have died from the virus. Early on, deaths of people in nursing homes were not counted. In the blur of early COVID-19 information, a headline warned that older people in facilities were being airbrushed out of existence.

As the coronavirus spread, caregivers of people with dementia still living at home were faced with two choices: (1) isolate to protect the physical health but risk a decline in cognitive function that could linger for years and physical declines that leave them weaker and at risk for nursing home placement, OR (2) continue in the community enjoying quality of life and functional maintenance but risk contracting a novel disease that could kill them in a matter of weeks. Residents of long-term care facilities didnt get a choice. Administrators locked down under the advisement of state and local government, believing that the drastic measure was critical to stop a potential and horrific spread.

At the beginning of the pandemic, the U.S. and other countries were grossly unprepared. At the foothills of the virus, there may have been a justified/expected time to lockdown and protect vulnerable older adults at all costs. However, like people who are free to walk outside, though physically distant, we suggest that it is time to begin conversations and develop comprehensive and safe strategies to relax the lockdown for (at least some) older adults living in facilities or attending adult day care. They are, after all, residents and citizens, not prisoners.

Like their younger counterparts, Older adults have the right to choose where they live and to direct their own destiny and help others through the pandemicfor example, serving as medical professionals, as grandparents, and as friends to others. For some older adults, the Impact of isolation on mental and physical health can be as dangerous as contracting a respiratory virus. When older adults are isolated from the outside world, a litany of events can occur: falls, general weakness, poor nutrition, lack of communication, depression, cognitive decline, and weight loss.

Older adults with the capacity and/or their appointed caregivers on their behalf should be able to choose quality of life over quantity of life. At least for older participants in our adult day services, most with dementia have designated a Do Not Resuscitate Order (DNR). These older adults recognize that they have more time behind them than in front of them, and that the years ahead will likely continue in a downward progression of dependence. They dont want a life prolonged in a suffering state. We ask the question and invite the conversationcould decline from isolation be prolonging life in a suffering state?

Strict medical models of long-term care provision presume physical safety above all else and take all possible measures to prevent death. We stress that not all older adults want all that can be done for them. Laws, research, and years representing the best thinking possible have evolved to create patient and resident rights providing the freedom to direct personal options for health care and habilitation.

We recognize that far too many people are dying alone in care facilities, deprived of the company of friends and family. We honor and applaud the valor of those providing care at facilities. Some care providers gave their lives to protect older residents. While healthcare teams are dedicated, present, and heroically doing their best, they cannot replace the presence of family and those with whom an older adult has shared a lifetime.

Continued isolation cannot be the new normal; humans, after all, are social creatures. A lockdown cannot continue for months and months on end. Should additional waves of the virus spread across the nation, swift and complete lockdown cannot be the knee jerk reaction. We call for the creation of innovative new plans that are taken seriously, rehearsed, ready, and in place so that we have better options for current and future emergencies. We assert that, by developing better plans now, while we are slowly opening back up, we can reduce the experience of hands pressed against glass.

Mental Health Benefits of Parks Dimmed by Safety Concerns – NYU Langone Health

No matter how close parks are to home, perceptions of park-centered crime may keep New Yorkers from using them.

Researchers at NYU Grossman School of Medicine found that New Yorkers are more likely to exercise in a park if they believe they live very close to it. In turn, they feel less anxious and less depressed the more often they exercise therebut only if they are not concerned about being safe.

Living near a park may not be enough to improve your physical and mental wellbeing through exercise, says study lead author Stephanie L. Orstad, PhD, a research assistant professor in the Department of Medicine at NYU Langone Health. If we want to make the most of the abundant health benefits parks offer, then we need to make them not only accessible, but also safe for everyone.

Many past studies have linked the availability of urban green spaces to lower stress levels, weight, and risk of heart disease, the study authors say. Other work has shown that living closer to a park leads to fewer days of anxiety and depression.

The new study, published online July 7 in the International Journal of Environmental Research and Public Health, is the latest to suggest that safety concerns could interfere with mental health advantages that park proximity offers New Yorkers, Dr. Orstad says.

For the investigation, the researchers analyzed responses from more than 3,800 New Yorkers who completed the citys 20102011 Physical Activity and Transit Survey. The assessment tracked the participants mental health, as well as how long they estimated it would take them to walk from home to the nearest park. The survey also asked participants to estimate how often they used the park to exercise or play sports.

Nearly twice as many people said they exercised in the nearest park sometimes or often if they lived less than a 5-minute walk away, compared with people who estimated living more than 30 minutes away, the researchers say. In addition, people who described themselves as frequent park exercisers reported having one fewer day of mental health issues a month compared with people who said they rarely or never were active in their local park.

However, the closeness of a local park made no difference in park use for those who worried about crime in the area. According to Dr. Orstad, improving cleanliness and lighting along paths, offering more park-based programs, and fostering a sense of community could help make parks feel safer. She emphasized that the coronavirus pandemic has highlighted the importance of such communal programs because parks are one of the few remaining places where people can get out of their homes, be active, and connect (from a distance) with their neighbors.

Investing in park safety offers a practical way of improving physical and mental health in different communities in the city, especially in areas where there are stigmas associated with seeking help, says senior study author Melanie R. Jay, MD, MS, an associate professor in the Departments of Medicine and Population Health at NYU Langone. It takes advantage of resources that may already exist in the neighborhood.

Next, the research team plans to investigate ways to improve a communitys perceptions of its neighborhood park to increase opportunities for exercise and mental health.

Funding for the study and its publication was provided by NYU Langones Comprehensive Program on Obesity.

FITNESS: Trainer becomes accustomed to giving online workouts – Odessa American

Fitness trainer Mike Ruiz is used to giving online workouts.

But with COVID-19 driving people away from gyms for now, he has had to adjust to doing workouts from a virtual setting. Ruiz, who has been a trainer since 2010, admits that its taken some time to get used to the changes that a lot of other trainers have gone through.

People are looking for the online training because they feel like thats the best way to work out right now since some gyms are closed, Ruiz said. Having access to a trainer and the ability to do it on their own schedule and own time and from the safety of their own homes has been a huge factor.

Separately from the online workout programs, Mike and his wife, Kyley also own Dash Mobile Fitness and he says that they both fill their van with equipment and train people from their garage and shops so that its more personal and more private.

They still get the benefit of using the equipment, Mike said.

Ruiz says his mobile business has been extremely busy ever since the coronavirus became more widespread back in March. The biggest change hasnt been moving training sessions online. Instead, the biggest adjustment has been able to handle the influx of clients.

That has been consistent throughout this entire ordeal, Ruiz said. Once things started getting crazy, people started contacting us because they still wanted access to gyms and to equipments. That way, they can control who, what, when and where.

Personal trainers have different ways of doing their online workouts.

Ruiz says that hell create a workout for the client and go through questions about their fitness level and other health questions. From there, the program is tailor-made based on what the clients want to do and what equipment they have access to.

Ruiz typically has between 16 and 20 clients and personalizes the regiments that focus on everything from nutrition, coaching, counseling, habit changing and wellness coaching. In addition, Ruiz also works with some clients on dealing with sleep, hygiene and stress management.

Theyll log on to that and go to the program that I have created for them and itll have their workouts scheduled for the day, whether its resistance training or stretching or cardiovascular training, Ruiz said. It just depends on what theyre supposed to do.

The program also comes with demonstration videos that show his clients how to do the exercises with the proper form. The technology adds an extra layer of accountability.

Ill get a notification that lets me know when the clients are doing the actual training and make sure they stay on top, Ruiz said. With technology and apps, my clients can connect their fit bit, or their certain weight scales and anything they want.

Numerous people have been stressed out due to the impact from the virus and Ruiz says his its been no different from his clients.

Stress affects everything, Ruiz said. Figuring out a way to manage your stress through exercise or clean eating, whether its a hobby of some sort, its a huge component of wellness. Its a small piece of a big picture.

Education Before Implementation: The CCPA Requirement That Should Be First on Your List, Not Last – CPO Magazine

CCPA enforcement began on July 1, 2020 and many businesses are still ramping up their compliance efforts. These efforts include meeting the CCPAs requirement to provide privacy training to all employees who either handle consumer inquiries or who are responsible for CCPA compliance. To maximize the value of CCPA compliance efforts, businesses should address the training requirement at the outset of the compliance process.

Too often, as companies begin to tackle their obligations under a new privacy law, the natural inclination is to turn immediate (and sometimes exclusive) attention to amending their privacy policies. This focus on external disclosures is not surprising, as privacy regulations are principally about providing transparency to consumers, and the privacy policy is the primary mechanism by which companies accomplish these goals. Additionally, an updated privacy policy is sometimes the only piece of tangible work product a company can point to when trying to justify the time and expense of reviewing its data practices.

However, as many privacy professionals have learned in the post-GDPR worldand much to the chagrin of budget-conscious business managersbusinesses must expend considerable effort before they can put pen to paper on a new privacy policy. The biggest to-do is data mapping (i.e., documenting the what, where, who, why, and how of the data collection, use and sharing in the business). Data-mapping involves answering, among other questions, the following:

To answer these questions accurately with a view towards updating disclosures, businesses need to engage employees at multiple levels of the organization. Although most employees will have no experience with interpreting privacy regulations, many will have the critical knowledge necessary to map the personal information that flows through every aspect of the business accurately (e.g., website visitor data, customer data, HR, payment card information). Although data-mapping can be a herculean exerciseespecially for large, complex organizationsdoing so efficiently and cost-consciously is critical in our data-driven economy.

While the importance of data-mapping message has propagated, what many businesses have yet to embrace is that to map data efficiently, businesses should begin with effective privacy training. Although the CCPA requires that businesses adopt some form of formal privacy training, training often appears as the very last item on compliance to-do lists. Training is generally thought of as a perfunctory check-the-box item and not a useful or worthwhile investment. This relegation is unwise. Organizations should, instead, make training the first compliance requirement they address because it can make addressing data mapping and all other CCPA requirements (including updating a privacy policy) easier and more efficient. And training can also help a business become more privacy forward in an environment marked by both increased consumer awareness of data privacy and regulatory complexity.

Training business managers and other employees makes addressing all of the other CCPA requirements easier and more efficient. The core requirement under the CCPA, and privacy laws like it, is accurate disclosure of the types of personal information collected by a business, the use of that information, and the types of third parties with whom a business shares or sells that information. As businesses address these disclosures, the language in the CCPA, as any new law or regulation would tend to do, demands the interpretation of ambiguities such as: what qualifies as personal information and what does it mean to sell it? As many privacy professionals understand, the meanings of these terms under the CCPA do not align with their plain meaning. Unless a business trains their employees before asking them to participate in a data mapping exercise, these employees are likely to confuse concepts and definitions, provide incomplete categorizations and descriptions, and may need to update their data maps later to supplement or correct their initial feedback. Without training to understand what qualifies as personal information under the law, addressing disclosures and updating privacy policies can become more costly, inefficient and less accurate.

Addressing disclosures and updating a privacy policy are not the only instances where training can be useful. Another CCPA pain point is the Do Not Sell requirement. The Do Not Sell rule requires businesses that sell personal information to provide consumers with a mechanism to opt-out of such sales. This requirement has led some businesses across industries to change their business practices to avoid making sales as defined under the CCPA. Otherseven in cases where required by the CCPAhave opted not to include the Do Not Sell button on their websites or mobile apps, and in certain industries, such as digital advertising, businesses have questioned whether the requirement spells the end for some of their core products and services. Needless to say, determining if a business sells personal information under the CCPA (and implementing Do Not Sell compliance for consumer requests if it is selling) should not be taken lightly, and certainly should not be taken with an incomplete or inaccurate understanding of how a business shares personal information. As with disclosures, without proper training before being asked to participate in a data mapping exercise, most employees are likely to provide incorrect or incomplete information to their in-house or external lawyers. This kind of garbage-in, garbage-out mapping may result in longer, more costly compliance reviews and, even worse, an incorrect or incomplete analysis.

Training early in the compliance process can also help make a business and more critically, its workforcemore privacy forward overall. While the GDPR and CCPA have dominated privacy headlines, there are no fewer than 24 states in the US seeking to pass CCPA-like comprehensive state privacy laws. And non-EU foreign privacy regimes will likely come into focus for many businesses as the dust continues to settle on the GDPR. Privacy compliance is here to stay, and many data privacy management principles and practices that business managers and employees are encountering with the CCPA will be relevant under other state and foreign privacy laws. Training enables business managers and employees to develop foundational knowledge that will make the learning curve less steep when tackling future laws, and makes a business more proactive and less reactive in its approach to privacy.

An effective privacy training program balances relevant, engaging and practical content that (1) meets the minimum compliance requirements, (2) educates its audiences with a view towards addressing privacy compliance holistically, and (3) reinforces a privacy forward business culture; in each case without consuming too many resources (e.g., cost or time) or creating administrative burdens (e.g., scheduling issues). At one end of the training spectrum are customized privacy training programs that can be tailored at the industry, company, and even department levels. Privacy professionals at law firms are best suited to deliver these programs to increase the benefit these programs can bring to an overall privacy compliance exercise. Program aims to get adult foster care home residents in Copper Country moving more and eating better – Rapid Growth

This article is part of Stories of Change, a series of inspirational articles of the people who deliver evidence-based programs and strategies that empower communities to eat healthy and move more. It is made possible with funding from Michigan Fitness Foundation.

Many of the residents with cognitive impairments at adult foster care homes served by Copper Country Community Mental Health Services were surprised to learn that burritos don't have to contain meat, but can instead be filled with black beans and vegetables. And that's just one of the many healthy lessons they've learned through a program called Linking Lessons for People with Cognitive Disabilities, which offers nutrition and physical activity education and encouragement tailored to the needs of clients with cognitive impairments.

A highlight of the program in this setting is that the foster home staff attend the lessons and have made positive changes in shopping and food preparation. Besides implementing some of the nutrition recommendations, they can also remind and encourage the residents to choose healthy snacks, eat more fruits and vegetables, and be more active inside and outdoors. This program is a wonderful example of a teach-learn-apply approach involving both the staff and residents of these adult foster care homes.

Teresa Robins, a direct care staff member in one of those residential group homes, has been involved with the Linking Lessons program for about two years and has seen long-lasting changes in the residents she serves. She says residents are now more willing to try fruits and vegetables, and they're moving more as a result of the physical activities in Linking Lesson sessions, which are offered in the home where they live.

"Some of the guys that participate in those physical activities don't normally exercise at all," Robins says. "It's really nice to see the difference in how the program got our guys more active." By applying the nutrition and physical activity recommendations, this important population can decrease their risk for obesity and some chronic illnesses.

A Linking Lessons kit.

The program is funded in part by a federal Supplemental Nutrition Assistance Program Education (SNAP-Ed) grant awarded by Michigan Fitness Foundation (MFF) and implemented through dedicated staff at Copper Country Mental Health Services. SNAP-Ed is an education program of the U.S. Department of Agriculture that teaches people eligible for SNAP how to choose healthier foods and be more physically active. As a State Implementing Agency for the Michigan Department of Health and Human Services, MFF offers competitive grant funding for local and regional organizations to conduct SNAP-Ed programming throughout Michigan.

The Linking Lessons nutrition program was developed by MFF using principles known to be effective with people who have cognitive disabilities. Messages are simple and are repeated and reviewed, with suggestions for how they can be implemented in everyday life. Each lesson includes a recipe, a food tasting, and a physical activity. Lessons cover topics including fruits and vegetables, healthy snacks, water and other healthy beverages, portion size, and the USDA's MyPlate food guide. Clients use recipes to prepare simple foods that don't involve turning on a stove, like cold corn salad and fruit slushies.

Liz Holden, training and prevention specialist with the Copper Country Community Mental Health Institute, says each lesson is flexible and can last 20 to 45 minutes.

"About half of that is the lesson, and then we chat a little about a particular nutritional topic, and focus on a colorful poster," she says. "We talk about familiarity with different vegetables, drinking more water, and portion size."

The physical activity components of Linking Lessons are also tailored to clients' unique needs. Holden notes that some of the clients have physical disabilities in addition to their cognitive disabilities, so chair yoga exercises are a great way to get clients with all types of abilities moving. She also emphasizes that physical activity can be made fun by putting on some music and having a dance party for the movement portion of the lesson.

Group home staff are always present at the lessons so they can learn as well and reinforce lessons after Holden has left. Karen Fooce, a residential team leader at two of the adult foster care homes served by the program, says she appreciates that the lessons and physical activity are adapted not just for the people with cognitive disabilities but also can be adapted for special needs from home to home.

For instance, she says the strength bands Holden brings to some of the physical activity sessions are good for stretching leg muscles or for range-of-motion exercises for wheelchair users' arms.

"One of my residents got down on the floor and did stretching and thought that was pretty cool," Fooce says. "Liz coming to the home was something they would really look forward to every week."

Robins says it's not just clients who are getting healthier. She says staff have also benefited from trying new fruits and vegetables, and she has used some of the strategies with her own son, who is a picky eater.

"It's a really good thing and has taught the residents a lot more about healthier choices," Fooce says. "It has turned them on to different vegetables, and they're learning what their likes and dislikes are."

The original program's lessons spanned two weeks, with a series of four lessons for a total of eight weeks, but both staff and residents at all locations agreed to expanding the program to five lessons for a total of 10 weeks.

"We've done surveying of our residents and staff involved in the program, and we've seen really positive results," Holden says, including that 76% of residents were more physically active after going through the program than they were before.

She says she has good conversations while she's running the program, and staff say the effect has been lasting.

"Our residents are eating more vegetables, drinking more water, and having more conversations about cutting down on sugary drinks," Holden says. "One of the biggest things is that residents talk more about food choices after having this class. One fellow started not salting his food anymore because of this class and talking about the effects of having too much salt in the diet."

The COVID-19 pandemic and stay-at-home orders meant that Holden had to discontinue visiting the homes while the order was in place, but she continued to provide resource packets to staff with recipes and ideas for staying physically active.

This summer, Holden says she and other staff would like to try out new warm-weather activities.

"We're hoping for more lawn games like lawn bowling, ring toss, tossing the beanbag," she says. "At one group home, the staff plays Yahtzee with residents every day, and they ordered big inflatable beach ball dice. That little change gets them up and outdoors and standing while they play."

Dom Sibley inspired to lose weight by work ethic of senior England players – The Guardian

Dom Sibley was greeted with a Hello, Fridge from Joe Root when he first joined the England setup last winter. But now a more slimline version of the Warwickshire opener heads into his first Test on home soil after shedding 12kg during lockdown.

Sibley has never found his bulky 6ft 3in frame to be a hindrance when it comes to run-scoring and bar this nickname around the squad a likely nod to William Perry, the former American footballer it has scarcely been mentioned in dispatches.

A maiden Test century compiled over eight hours in Cape Town during January certainly proved as much, but when Sibley was in Sri Lanka for Englands aborted tour two months later he decided to make a change that will become more obvious when he takes to the field against West Indies on Wednesday.

For the first time I felt a little bit self-conscious about my physique and my weight, the 24-year-old said. The 18 first-class counties have agreed to play both red-ball and white-ball cricket during this summer's shortened domestic season.

A majority vote was reached at a meeting of county chairmen on Tuesday. The agreement will now be passed on for approval from the England and Wales Cricket Board (ECB), after which a new fixture schedule will be drawn up and announced.

County cricket has been given the go-ahead to start on 1 August, having been on hold since April due to the coronavirus pandemic. The ECB insists health matters are of paramount importance and says counties will face further assessments to ensure it is safe for play to resume.

"The safety and wellbeing of all players, staff and officials remains the priority of the ECB," read a statement. "All first-class counties will undergo further medical risk assessments and venue compliance approval as planning progresses to ensure safe environments for the start of the men's domestic season."

It is a shame he and Zak Crawley, another to make his debut during the winter, will not experience a home crowd this year. England have now confirmed dates for the Tests in August against Pakistan, which will similarly be behind closed doors at the two biosecure venues, the Rose Bowl and Old Trafford.

Ive played the pull shot since a young age, but in South Africa, with the bouncy wickets, I didnt feel in control, so I chose to get out of the way. But in that area around the nipple to hip its a case of being in control, choosing when to play and when not to play. Its going to be a case of me managing those risks.

Growing up, I had a very rocky relationship with my weight. I was overweight most of my childhood, but when I was 16, I lost over 50 pounds. Unfortunately, the way I lost the weight wasnt healthy, and I was diagnosed with anorexia shortly after.

My relationship with food was extremely sensitive. I was in a binge and over-restrict cycle, which was really hard on me. I went to therapy for a while and that helped a lot. I also moved out of a toxic environment, which helped me heal and overcome my disordered eating habits.

Fast-forward to August 3rd, 2016, when I had my son. I had a lot of health issues during my pregnancy, including high blood pressure, IBS, and a high resting heart rate while I was pregnant. I gained 60 pounds, and after my sons birth, I was diagnosed with postpartum depression and anxiety.

I gained another 10 pounds over the following two years, putting me at 246 pounds.

Overall, I was just miserable and suffering constantly. I couldnt even carry my son around for more than two minutes without being completely out of breath.

On August 12th, 2018, I decided to start making changes for my health.

When I had lost weight previously (in a healthy mindset!), counting calories worked for me. A calorie deficit diet, or calories-in-calories-out (CICO) diet, is easy to track and maintain for me.

I also had originally cut dairy out of my diet to help with my stomach issues and acne, and eventually I started intermittent fasting along with calorie counting. I really like fasting because it eliminated late-night snacking.

Im usually never hungry in the morning anyway, so Ill drink my coffee and then wait until noon to eat my first meal. I enjoy the idea of having a window of time I can eat.

I tried picking up running, but it was really harsh on my knees and back. I ended up buying a cheap cycling machine off Amazon and would use it for 30 minutes a day for the first two months of my weight loss journey.

Then, I switched to running because it was less harsh on my body after losing 30 pounds. I got into running through an app called Couch To 5K, which taught me how to build my endurance, and I never looked back.

Now my typical week of exercise is usually four to six days of running. I love it so much. Three of those days are short runs for one to three miles followed by lower body strength training, upper body strength training, or core training.

Losing weight gave me the confidence I never had but always should have had. I shouldve always believed in myself, but it took doing something as difficult as this to recognize my potential. Progress isnt always linear and sometimes we have ups and downs, but the most important part is to keep pushing and stay strong. You've got this.
